Resumo: Leprosy, secular disease, still challenges and moves mankind to control it globally. Despite having been eliminated in several regions of the world, it remains as a serious problem in other regions. Brazil as being only second to India in notifying new cases. The main strategy for Leprosy control has been the launching of the multidrug therapy by the World Health Organization. This has been a successful strategy, but it has not managed to carry out the medium-term elimination of the disease, thus pointing to other disease-related features that play a role in this process and have not yet been unveiled. This current study aimed at analyzing the knowledge of primary health care users diagnosed with Leprosy. It has also analyzed what they actually know about it, and what has not yet been incorporated in their care by the Municipal Leprosy Control Program, and how this may help those affected by the disease in approaching the diagnosis and treatment. Knowing what it is and what keeps them away from it, is empowering in this process, it may reduce the gaps between Hansen Disease and the program. It is a qualitative research accomplished in the APS of the Municipality of Eunápolis, Bahia, having been adopted as technique for data collection the semi-structured interview and social-demographic survey. 14 interviews were accomplished with Hansen Disease diagnosed users. Data collection took place between September through December, 2017. To carry out treatment and data analysis is has been used the technique of Thematic-Categorical Content Analysis. It has been verified that the lack of knowledge about the disease and the health professionals’ difficulties in performing definitive diagnosis has compromised the control of the disease. Multidrug therapy-related side-effects were also analyzed, along with beliefs surrounding the disease, the lack of bonds of trust in the relationship between users, family and health care teams, and the apparent improvement of signs and symptoms, after the beginning of the referred therapy have contributed to treatment irregularity and its abandonment. It has been concluded that the access to a Primary Care Facility for the Hansen Disease program adherence, has stimulated users in taking part with the Family Health Care. It has been realised that it is difficult to live with the disease without family support, and that the lack of formal education makes it difficult for the fluency of communication among health professionals and users, about the disease and treatment. Until the time of diagnosis, many were unaware of the disease and that some still preferred to hide the diagnosis, and so, the psychological support is important to help them to maintain their self-esteem raised, and to perform treatment and to stimulate autonomy and integration between users and close people who collaborate in the treatment. It has been recommended, that the Municipality managers, and Municipality coordinators of Leprosy Control Programs, along with the support of the National and State Leprosy Program, perform local planning through partnerships with the civil society's nongovernmental organizations, universities and federal institutes of education, towards contributing to strengthening integral care to these users, as well as to support them both emotionally and socially.

Aunque se ha eliminado en varias regiones del mundo, en otros es un problema grave. Brasil es el segundo país que más notifica nuevos casos de lepra, perdiendo sólo a la India. La principal estrategia para su control fue el lanzamiento de la poliquimioterapia por la Organización Mundial de la Salud. Se trataba de una estrategia exitosa, pero no alcanzó la eliminación de la enfermedad a mediano plazo, lo que indica que otros aspectos que lo involucran implican, se insertan en este proceso, pero aún no se han revelado. El presente estudio tuvo como objetivo analizar el conocimiento de los usuarios con un diagnóstico de Lepra asistida en atención primaria de salud. También analizó lo que estos usuarios conocen acerca de la enfermedad y que aún no se ha incorporado a su cuidado con el programa municipal de Lepra y cómo esto puede ayudar a los afectados por la enfermedad para acercarse al diagnóstico y tratamiento. Saber lo que es y lo que los aleja, es empoderar en este proceso puede reducir las brechas que existen entre los usuarios con lepra y el programa. Se trata de una investigación cualitativa, llevada a cabo en el APS de la ciudad de Eunápolis, Bahia, trato de adoptar como técnica de recolección de datos la entrevista semiestructurada, y la encuesta sociodemográfica. Se realizaron catorce entrevistas con los usuarios diagnosticados con lepra. La recolección de datos se produjo entre septiembre y diciembre 2017. Para proceder con el tratamiento y análisis de los datos, se utilizó la técnica de análisis de contenido temático-categorial. Se descubrió que la falta de conocimiento sobre la enfermedad y las dificultades de los profesionales sanitarios en la realización del diagnóstico definitivo comprometen el control de la enfermedad. También se analizaron los efectos indeseables correlacionados con la poliquimioterapia, las creencias que involucran la enfermedad, la ausencia de vínculos fiduciarios en las relaciones entre los usuarios, los equipos familiares y de salud y la aparente mejoría de los signos y síntomas después de la aparición de tratamiento y abandono irregulares. Se concluyó que el acceso a la unidad de salud básica para la adherencia al programa de la lepra, despertó a los usuarios a participar en el cuidado con la salud de la familia. Se percibe que es difícil vivir con la enfermedad sin el apoyo de la familia y que la ausencia de educación formal dificulta la comunicación fluida entre los usuarios y los profesionales de la salud sobre la enfermedad y el tratamiento. Hasta el momento del diagnóstico, muchos desconocían la enfermedad y algunos todavía prefieren ocultar el diagnóstico, y el apoyo psicológico es importante para ayudarles a mantener una alta autoestima, a realizar el tratamiento y a estimular la autonomía y Integración entre usuarios y personas cercanas que colaboran en el tratamiento. Se recomienda que los gerentes municipales y los coordinadores municipales del programa de lepra, junto con el apoyo del programa nacional y estatal de lepra, emprendan la planificación local a través de alianzas con organizaciones civiles no gubernamentales , universidades e institutos federales de educación, un grupo que puede contribuir a fortalecer la atención integral a estos usuarios, así como a apoyarlos emocional y socialmente.A hanseníase, doença secular, ainda desafia e move os homens para o seu controle global. Apesar de ter sido eliminada em diversas regiões do mundo, em outras constitui grave problema. O Brasil é o segundo país em que mais notificam casos novos de hanseníase, perdendo apenas para a Índia. A principal estratégia para o seu controle foi o lançamento da Poliquimioterapia pela Organização Mundial de Saúde. Essa foi uma estratégia exitosa, porém não atingiu a eliminação da doença em médio prazo, o que indica que outros aspectos que a envolvem, estão inseridos nesse processo, mas ainda não foram desvelados. O presente estudo buscou analisar os saberes dos usuários com diagnóstico de hanseníase assistidos na Atenção Primária à Saúde (APS). Analisou também o que esses usuários sabem sobre a enfermidade e o que ainda não está incorporado no seu atendimento junto ao Programa Municipal de Hanseníase e de que modo isso pode ajudar os acometidos pela doença a se aproximarem do diagnóstico e do tratamento. Conhecer o que é e o que os afasta, é fortalecedor nesse processo e pode reduzir lacunas que existem entre usuários com hanseníase e o programa. Trata-se de uma pesquisa qualitativa, realizada na APS do município de Eunápolis, Bahia, tento adotado como técnica para a coleta de dados a entrevista semiestruturada e o levantamento sociodemográfico. Realizaram-se 14 entrevistas junto a usuários com diagnóstico de hanseníase. A coleta de dados ocorreu entre setembro e dezembro de 2017. Para proceder ao tratamento e análise dos dados, utilizou-se a técnica de Análise de Conteúdo Temático-Categorial. Verificou-se que o desconhecimento sobre a doença e as dificuldades dos profissionais de saúde em realizar o diagnóstico definitivo compromete o controle da doença. Analisou-se também a respeito dos efeitos indesejados correlacionados à Poliquimioterapia, as crenças que envolvem a doença, a ausência de vínculos de confiança nas relações entre usuários, família e equipes de saúde e a melhora aparente dos sinais e sintomas após o início da referida terapia contribuem para tratamentos irregulares e para o seu abandono. Conclui-se que o acesso à Unidade Básica de Saúde para adesão ao programa de hanseníase despertou os usuários para participarem do cuidado com a saúde da família. Percebeu-se ser difícil conviver com a doença sem o apoio familiar e que a ausência da educação formal dificulta a comunicação fluente entre usuário e profissionais de saúde sobre a doença e tratamento. Até o momento do diagnóstico, muitos desconheciam sobre a doença e alguns ainda preferem esconder o diagnóstico, sendo que o apoio psicológico é importante para ajudá-los a manter a autoestima elevada, a realizar o tratamento e a estimular a autonomia e a integração entre usuários e pessoas próximas que colaborem no tratamento. description Leprosy, secular disease, still challenges and moves mankind to control it globally. Despite having been eliminated in several regions of the world, it remains as a serious problem in other regions. Brazil as being only second to India in notifying new cases. The main strategy for Leprosy control has been the launching of the multidrug therapy by the World Health Organization. This has been a successful strategy, but it has not managed to carry out the medium-term elimination of the disease, thus pointing to other disease-related features that play a role in this process and have not yet been unveiled. This current study aimed at analyzing the knowledge of primary health care users diagnosed with Leprosy. It has also analyzed what they actually know about it, and what has not yet been incorporated in their care by the Municipal Leprosy Control Program, and how this may help those affected by the disease in approaching the diagnosis and treatment. Knowing what it is and what keeps them away from it, is empowering in this process, it may reduce the gaps between Hansen Disease and the program. It is a qualitative research accomplished in the APS of the Municipality of Eunápolis, Bahia, having been adopted as technique for data collection the semi-structured interview and social-demographic survey. 14 interviews were accomplished with Hansen Disease diagnosed users. Data collection took place between September through December, 2017. To carry out treatment and data analysis is has been used the technique of Thematic-Categorical Content Analysis. It has been verified that the lack of knowledge about the disease and the health professionals’ difficulties in performing definitive diagnosis has compromised the control of the disease. Multidrug therapy-related side-effects were also analyzed, along with beliefs surrounding the disease, the lack of bonds of trust in the relationship between users, family and health care teams, and the apparent improvement of signs and symptoms, after the beginning of the referred therapy have contributed to treatment irregularity and its abandonment. It has been concluded that the access to a Primary Care Facility for the Hansen Disease program adherence, has stimulated users in taking part with the Family Health Care. It has been realised that it is difficult to live with the disease without family support, and that the lack of formal education makes it difficult for the fluency of communication among health professionals and users, about the disease and treatment. Until the time of diagnosis, many were unaware of the disease and that some still preferred to hide the diagnosis, and so, the psychological support is important to help them to maintain their self-esteem raised, and to perform treatment and to stimulate autonomy and integration between users and close people who collaborate in the treatment. It has been recommended, that the Municipality managers, and Municipality coordinators of Leprosy Control Programs, along with the support of the National and State Leprosy Program, perform local planning through partnerships with the civil society's nongovernmental organizations, universities and federal institutes of education, towards contributing to strengthening integral care to these users, as well as to support them both emotionally and socially.
