Do nó ao laço: a trajetória do Fórum de Saúde Mental na cidade do Rio de Janeiro como operador da gestão do cuidado

The model of care which until then was guided assistance to crazy, was based on mental institutions whose treatment provided was confined to psychiatric hospitalization. By taking the asylum as their only destination, remained subject to undergo the only way to treat madness: isolation and exclusion. This was the scenario that the psychiatric reform tried change. Beyond the end of asylums, which was aiming at transposing asylum model for communitary basis. This is the motto of the mental health policy in the city of Rio de Janeiro initiated since the 90s. Therefore, our investigation has put in question the invention of the Mental Health Forum established in 2002 as an arena participatory involving mental health workers in toward building a network of regional attention. Therefore, this study produced a narrative on its path through the bias of its formulation and implementation, from the subjects who participated in this process. We believe that this choice gave us subsidies to understand which paths - or discarded - and the trends that transformed this arena as operator of managed care in mental health. In this direction, the theoretical contribution that serve us leaned on the Structuracion Theory that their study on the production and reproduction of social life by the social agents. The use of this concept helped us in understanding a process from own agents since they hold a reflective capacity, or rather a theoretical understanding of its own shares, including the reasons, motives and needs that instigate to do it. The study identified that the Forum mental health arena worked as an operator of managed care, however, is porous to the set of people who are driving, making delicate as its institutional framework and participatory decision-making arena.
