Resumo: This study sought to understand the changes imposed by Ordinance No. 2,436 of 2017 when compared to the version of Ordinance No. 2,488 of 2011, based on the historical construction of the political meaning of Primary Health Care (APS). The hypothesis is that the transformations imposed with the National Primary Care Policy (PNAB) instituted in 2017 affect the democratic axis of the SUS and are part of an articulated scenario that favors the deconstruction of social policies. When analyzing the historical trajectory from 1960 to 2018, two main strands of Primary Care (AB) - extended and restricted - were influenced by political issues at different times. The 2017 PNAB was instituted in a scenario of fiscal austerity and was the stage for other changes, weakening the universality, integrality, equity and popular participation of the SUS. The analysis of Ordinance No. 2436 of 2017 shows the induction to change the care model and exposes theoretical advances for AB while removing the necessary conditions for its realization. Despite defending the maintenance of the centrality of the Family Health Strategy (ESF), 2017 PNAB extracts the prominence of this model through territorial disarticulation and strong induction for the implementation of the AB teams. Ordinance No. 2436 of 2017 does not conform as a 'reform' or 'revision' of the 2011 publication, it constitutes a structural change for the Brazilian AB and for the SUS, directly affecting the right to health and Brazilian democracy.

A hipótese é que as transformações impostas com a Política Nacional de Atenção Básica (PNAB) instituída em 2017 afetam o eixo democrático do SUS e fazem parte de um cenário articulado que favorece a desconstrução das políticas sociais. Ao realizar a análise da trajetória histórica de 1960 até 2018, duas principais vertentes da Atenção Básica (AB) - ampliada e restrita - foram influenciadas pelas questões políticas em diferentes épocas. A PNAB de 2017 foi instituída em um cenário de austeridade fiscal e foi palco para outras mudanças, enfraquecendo a universalidade, a integralidade, a equidade e a participação popular do SUS. A análise da Portaria nº 2.436 de 2017 mostra a indução para alteração do modelo de atenção e expõe avanços teóricos para a AB ao mesmo tempo em que retira as condições necessárias à sua realização. Apesar de defender a manutenção da centralidade da Estratégia de Saúde da Família (ESF), a PNAB de 2017 extrai o protagonismo deste modelo por meio da desarticulação territorial e da forte indução para a implantação das equipes de AB.
