Resiliência no tratamento de pacientes com insuficiência cardíaca: construção e validação do instrumento RES-IC 15

Resumo: Introduction: Heart failure is a chronic and devastating disease that requires multi-professional care for therapeutic success and a reduction in hospital admissions and years of life lost due to complications and the impact of the disease on activities of daily living. Resilience is a psychological construct that can contribute to positive health behaviors and better outcomes. Generic instruments for measuring resilience have been used to assess this construct in the follow-up of patients with heart failure. Objectives: To propose the construction of an instrument to measure resilience specifically for individuals with chronic heart failure; to carry out a Scoping Review of the main instruments used in research to measure resilience in patients with chronic cardiovascular diseases; to analyze the evidence of content validity of the resilience instrument specifically for patients with chronic heart failure. Method: This is a psychometric study based on Pasquali's methodology, structured around three main poles: theoretical, empirical and analytical. The Delphi technique was used to evaluate the responses of the experts, whose sample was obtained by convenience, in two rounds using the Google Docs tool with five-point Likert-type response options. The statistical analysis was presented in terms of frequency and percentage, and the individual and instrument Content Validity Index (CVI), Content Validity Ratio (CVR) and Cronbach's alpha were evaluated. Results: 16 health professionals took part in the evaluation of the instrument in the first round and 13 in the second round, including doctors, nurses and psychologists from all regions of the country. In the first round, the total CVI was 0.96, Cronbach's alpha was 0.82, but the content validity ratio (CVR=0.37) was not reached for all the criteria of the 15 items. Three items, where there was no consensus among the experts in their preparation, were re-evaluated in the second round, obtaining a CVR ranging from 0.69 to 1.00. Conclusion: The resilience measurement instrument named RES-IC 15, specific for patients with chronic heart failure, showed evidence of validity, however, it is necessary to apply this instrument to the population for which it is intended for its reproducibility.

A resiliência é um constructo psicológico que pode contribuir para comportamentos positivos em saúde e melhores desfechos. Instrumentos genéricos de mensuração da resiliência têm sido empregados para a avaliação deste constructo no seguimento de pacientes com insuficiência cardíaca. Objetivos: Propor a construção de um instrumento de medida da resiliência específico à indivíduos com insuficiência cardíaca crônica; elaborar uma revisão bibliográfica do tipo Scoping Review com os principais instrumentos utilizados nas pesquisas de mensuração da resiliência em pacientes com doenças cardiovasculares crônicas; analisar as evidências de validade de conteúdo do instrumento de resiliência específico para pacientes com insuficiência cardíaca crônica. Método: Trata-se de um estudo psicométrico a partir na metodologia de Pasquali, estruturado em três grandes polos: teórico, empírico e analítico. Para avaliação das respostas dos especialistas, cuja amostra foi obtida por conveniência, foi utilizada a Técnica Delphi em duas rodadas pela ferramenta Google Docs com opções de resposta tipo Likert de cinco pontos. A análise estatística foi apresentada em frequência e percentual, foi avaliado o Índice de Validade de Conteúdo (IVC) individual e do instrumento, a Razão de validade de conteúdo (RVC) e alfa de Cronbach. Resultados: Participaram na avaliação do instrumento 16 profissionais de saúde na primeira rodada e 13 na segunda rodada, incluindo médicos, enfermeiros e psicólogos de todas as regiões do país. Na primeira rodada o IVC total foi de 0,96, Alfa de Cronbach de 0,82, mas não foi atingida a razão de validade de conteúdo (RVC=0,37) em todos os critérios dos 15 itens. Três itens, onde não houve um consenso entre os especialistas na sua elaboração, foram reavaliados a segunda rodada, obtendo uma RVC que variou de 0,69 a 1,00.
