Trajetória histórica da Reforma Psiquiátrica em Goiânia-GO: avanços, desafios e perspectivas

The dissertation investigated the historical trajectory of the Psychiatric Reform in Goiânia-GO, with purpose of mapping and analyzing the constitution of the policies and city's mental health network. The initial bibliographic survey indicated a shortage of material compiled on the subject. Hence the historiographic study was justified by the need to systematize and rescue the main landmarks and the social actors involved in them. The research procedures adopted were the bibliographic review, the documentary survey and the accomplishment of three interviews with significant people in the Psychiatric Reform movement in Goiânia. The time frame covered the period of intensification of transformations in local mental health care from 1992 to late 2016. The results indicated that the Reform movement in Goiânia emerged and was enhanced by the social mobilization, determining the restructuring of practices and policies throughout the 90s. Since 2001, following the national scenario of promulgation of the Law 10.216 / 2001, began the expansion of the network of substitute services and the progressive reduction of the number of psychiatric beds agreed by the public healthcare system in brazil (SUS). Between 2001 and 2004, were implemented most part of the services which compound the psychosocial network in the town. It was observed that such advances were result of the mobilization of social actors and local management committed to the project of the Psychiatric Reform which was not observed in subsequent years. After a period of stagnation, the mental health network in Goiânia returned to progress starting in 2011, driven by the national policy of strengthening RAPS and by a new management of activists of the anti-asylum fight in the town's Mental Health Department (SMS). After 2014, the challenges for the growth of RAPS have intensified and the reality of the network is of the precarious services, inadequate physical structure, lack of materials, inadequate working conditions, network disarticulation, cuts and administrative changes. There was a reduction in the number of psychiatric beds agreed with the SUS, however, we realized that this decrease occurred without a corresponding increase in RAPS because the town does not have beds in general hospitals and we consider that the coverage is insufficient to meet the demands of the city quantitatively and regionally. The demarcated periods were analyzed as rupture showing the predominance of the coherencies or resistance to the process of constitution of the Reformation in Goiânia. With the support of the data collected, we concluded that there is still a long way to go for the implementation of the deinstitutionalization and consolidation of the Psychiatric Reform in Goiânia, historically marked by private and essentially hospital-centered assistance. Some perspectives are pointed out covering mainly the power of social mobilization in the processes of change of reality. The dissertation sought to give visibility to the historical trajectory and to reveal aspects of the current scenario of the mental health network of Goiânia aiming to contribute to the debate in the field of local mental health and in Brazil
