Resumo: The present master's thesis, developed in the Master's course of the Post-Graduate Program in Social Work at the State University of Rio de Janeiro, whose theme is Institutional transphobia to travesti and transsexual and women living with HIV, aims to highlight and to analyze how transphobia, in its structural and institutional character, added to the HIV stigma, directly impact the health and disease process of transsexual and transvestite women living with the virus. Given this, it points out elements that contribute to the debate about institutional transphobia, the stigma related to HIV and how these two issues are summed up and end up generating a series of conditions for the health of this population, in addition to unfolding in a process of violence, which is expressed in the most diverse ways and which crosses the existence of transsexual and transvestite women. The master’s thesis discusses the concepts of gender, sexuality, transsexuality and transvestite, besides historically contextualizing the field of discussion and pointing out some elements to think about this process of violence, also identified as transphobia. It brings some considerations regarding the field of sexual rights, briefly conceptualizes the Human Immunodeficiency Virus (HIV) and discusses the impact of stigma and discrimination around the virus infection. Finally, it addresses the experience of working with transsexual and transvestite women living with the HIV virus, who are followed up through clinical research conducted at an institution in Rio de Janeiro, based on reports recorded in the field diary of the present researcher.

Diante disso, aponta elementos que visam contribuir para o debate acerca da transfobia institucional, do estigma relacionado ao HIV e de como essas duas questões são somatizadas e acabam por gerar uma série de condicionantes à saúde dessa população, além de se desdobrar num processo de violência, que se expressa das mais diversas formas e que atravessa a existência de travestis e mulheres transexuais. A dissertação discute os conceitos de gênero, sexualidade, transexualidade e travestilidade, além de contextualizar historicamente o campo de discussão e apontar alguns elementos para se pensar tal processo de violência, também identificado como transfobia. Traz algumas considerações a respeito do campo dos direitos sexuais, conceitua brevemente o Vírus da Imunodeficiência Humana (HIV) e discute o impacto do estigma e da discriminação em torno da infecção pelo vírus.
