Ser familiar cuidador de doente com câncer em cuidados paliativos: uma análise à luz do interacionismo simbólico

Resumo: Support for the terminally ill patient's family is one of the principles governing palliative care so that they can cope with the patient's illness and survive the grief period. In this sense, the main objective is understanding the experience of family members in the care of a person with cancer in the terminal state in palliative care, and as specific objectives: to characterize the experience of the family in the care for the family member with terminal cancer in palliative care; to describe the meaning of becoming/being/performing as a caretaker of terminal oncological patients in palliative care, and to analyze, in the light of Symbolic Interactionism, the experience of being a family member of a person with cancer in the terminal state in palliative care. It is a descriptive research, with a qualitative approach, through the Narrative of Life method. The 18 participants were 14 women and 4 men. The study scenario was the Palliative Care Unit, which is part of the Aristides Maltez Hospital, a philanthropic state reference for the treatment of cancer in the city of Salvador, Bahia. Data collection was performed through open interviews, between August and October 2018, after submission, approval and appreciation of the Research Ethics Committee (CEP) of the State University of Southwest Bahia and the Maltez Hospital. The project was approved, with Decree N o . 2,717,196, by the CEP of the State University of Southwest Bahia, UESB/BA, as proponent institution, and Decree No. 2,811,728, by the CEP of the Aristides Maltez Hospital, as cooperating institution. In all stages of the development of the study, the ethical principles regulated by Resolution No. 466 of the National Health Council, Ministry of Health, were respected. Data collection was conducted through an open interview with the following question: Tell me about your experience in caring for your relative, terminally ill cancer patient, in palliative care. Thematic analysis enabled, after codification and recodification, the formulation of two main categories with their subcategories. The first category: Caring with affection: meaning the way the family cares in finitude, and the second category: Changing in daily life: meaning looking at finitude. Care is the symbol that permeates the behavior of the relatives of the terminally ill patient, as a result of the actions that integrate them, and each one, with their individuality, with they own way, is acting and reacting on the demands of being caretaker taking care in finitude. Becoming a caregiver involves many changes in the lives of the direct and secondary caregivers, involving changes in their personal and professional life, in their relationships with others, impacting on their daily lives and their family nucleus. Giving voice to the patient's caregivers in palliative care, through attentively listening to their needs, is to broaden the nurse's role in caring for them, valuing the senses from becoming a caretaker with all its implications.

Nesse sentido, o objetivo geral é compreender a experiência de familiares no cuidado de pessoa com doença terminal oncológica em cuidados paliativos, e como objetivos específicos: caracterizar a experência de familiares no cuidar de pessoa com doença terminal oncológica em cuidados paliativos; descrever o sentido de se tornar/fazer/constituir cuidador de familiar com doença oncológica terminal em cuidados paliativos; e, analisar, à luz do interacionismo simbólico, a experiência de ser familiar de pessoa com doença terminal oncológica em cuidados paliativos. Trata-se de pesquisa descritiva, com abordagem qualitativa, através do método da Narrativa de Vida. Os 18 participantes foram 14 mulheres e 4 homens. O cenário do estudo foi a Unidade de Cuidados Paliativos, que integra o Hospital Aristides Maltez, estabelecimento filantrópico de referência estadual para o tratamento do câncer na cidade de Salvador- BA. A coleta de dados foi realizada através da entrevista aberta, entre agosto e outubro de 2018. O projeto foi aprovado, com Parecer de número 2.717. 196 pelo CEP da Universidade Estadual do Sudoeste da Bahia,UESBBA, enquanto instituição proponente, e Parecer de número 2.811.728, pelo CEP do Hospital Aristides Maltez, instituição coparticipante. Em todas as etapas do desenvolvimento do estudo foram respeitados os princípios éticos regulamentados na Resolução nº 466 de 2012, do Conselho Nacional de Saúde, Ministério da Saúde. A coleta de dados foi realizada através da entrevista aberta com a seguinte pergunta: Conte-me da sua experiência no cuidar do seu familiar com doença terminal oncológica em cuidados paliativos. A análise temática possibilitou, após a codificação e recodificação, a formulação de duas categorias principais com suas subcategorias. A primeira categoria: Cuidando com afeto: significando o jeito do familiar cuidar na finitude, e a segunda categoria: Mudando no cotidiano: significando olhar para a finitude. O cuidado é o símbolo que permeia o comportamento dos familiares do doente terminal, fruto das ações que os integra, e cada um, com sua individualidade, com seu jeito próprio vai agindo e reagindo as demandas de se fazer cuidador cuidando na finitude. Tornar-se cuidador cuidando, implica muitas mudanças na vida dos cuidadores diretos e secundários, envolvendo transformações no âmbito pessoal, profissional, nas relações com os outros, impactando no seu cotidiano e do seu núcleo familiar.
