Análise das Internações por Condições Sensíveis à Atenção Primária no âmbito do Sistema Único de Saúde por regiões de saúde do estado do Rio Grande do Sul, de 2009 a 2015.

Resumo: | Ambulatory Care Sensitive Conditions (ACSC) represents a set of health diagnoses which the effective action of Primary Health Care would reduce the risk of hospitalizations. In this sense, disparities in hospitalizations due to a set of diseases considered to be easily prevented or those where early diagnosis and treatment could prevent hospitalization, would reflect inadequate health care for the needs of particular communities. The aim of the study was to study the evolutionary trends of hospitalizations due to ACSC on brazilian Unified Health System in the health regions of the state of Rio Grande do Sul in the period from 2009 to 2015. The list used as base to the study of ACSC overview is the same as presented by the resolution order number 221 of April 17th 2008. The hospitalization data was collected from the Hospital Information System and the study group was only the state residents. During the study period, approximately 4.5 million hospitalizations occurred in the State. The number of hospitalizations of residents of Rio Grande do Sul due to ACSC found was 973,711 that correspond just to 22% of total hospitalizations of residents in the state. Hospitalizations duo to ACSC occurred manly in children and the elderly. Four groups of causes prevailed as the main reasons for ICSAP: pulmonary diseases; cerebrovascular diseases; infections in the kidney and urinary tract, and infectious gastroenteritis and its complications. In all years, unspecified chronic obstructive pulmonary disease (COPD) occupied the first position as a diagnosis that led to these hospitalizations. It was identified a tendency of fall in the gross rates of ICSAP in the studied period. The analysis of the ICSAP gross rates considering the resident population in each health region offered a positive overall picture, with improvement of the indices in 83% of health regions when compared to the values registered in 2009 and 2015. The systematic evaluation of ICSAP rates can be useful as tool to follow the behavior of a regular group of diseases, indirectly detecting the quality conditions of the primary health care and can help to establish the needs of the system and the set of priorities by managers. |

Nesse sentido, disparidades nas internações por um conjunto de doenças consideradas de fácil prevenção, ou por aquelas onde o diagnóstico e tratamento precoces poderia evitar a hospitalização, refletiriam uma inadequação da atenção à saúde às necessidades de determinadas comunidades. O objetivo do estudo foi o de traçar o perfil evolutivo das internações por condições sensíveis à atenção primária no âmbito do SUS nas regiões de saúde do estado do Rio Grande do Sul no período de 2009 a 2015, tendo como população de estudo a de residentes do estado. Utilizou-se a lista de internações por condições sensíveis elencadas pela Portaria SAS/MS n° 221, de 17 de abril de 2008, e aos dados do Sistema de Informações Hospitalares do Sistema Único de Saúde como base. Durante o período estudado, ocorreram aproximadamente 4,5 milhões de internações no Estado. O número de internações de residentes do Rio Grande do Sul por condições sensíveis à atenção primária somou 973.711, correspondendo a pouco menos de 22% do total de hospitalizações de residentes no estado. As hospitalizações por ICSAP predominaram em crianças e idosos. Quatro grupos de causas prevaleceram como principais motivos de ICSAP: doenças pulmonares; doenças cerebrovasculares; infecções no rim e trato urinário, e gastroenterites infecciosas e suas complicações. Em todos os anos, doença pulmonar obstrutiva crônica (DPOC) não especificada ocupou a primeira posição como diagnóstico que levou a essas internações. Identificou-se tendência de queda nas taxas brutas de ICSAP no período estudado. A análise das taxas brutas de ICSAP considerando a população residente em cada região de saúde ofereceu um panorama geral positivo, com melhora dos índices em 83% das regiões de saúde, se comparado os valores registrados de 2009 e 2015.
