Imunoterapia orofaríngea de colostro e ganho ponderal em recém-nascidos prematuros de muito baixo peso

Resumo: | A Imunoterapia Orofaríngea de Colostro (IOC) auxilia o início e progressão da terapia nutricional, ao promover crescimento e maturação intestinal, reduzir eventos de intolerância alimentar e melhorar a evolução clínica de recém-nascidos prematuros. No entanto, o resultado desta imunoterapia sobre a evolução ponderal nos neonatos foi pouco explorado. Por isso, o presente estudo avaliou o efeito da IOC sobre o ganho ponderal de recém-nascidos prematuros (RNPT) com muito baixo peso, admitidos em uma unidade pública de atendimento materno-infantil. Trata-se de um ensaio clínico não-randomizado, com controle histórico. Os RNPT com menos de 1.500g foram alocados em dois grupos de acordo com o período de sua admissão na unidade de terapia intensiva neonatal: no controle os admitidos entre 2015 e 2018 e intervenção os admitidos após a implantação do Protocolo de IOC, entre 2019 e 2020. O esquema de tratamento foi constituído diariamente por 8 administrações de 0,2 ml (quatro gotas) de colostro por dez segundos em mucosa orofaríngea, oferecidos a cada três horas, até o sétimo dia de vida da criança. Os recém-nascidos (RN) foram pesados despidos, uma vez por dia, desde o nascimento, pela equipe de profissionais dos setores envolvidos, previamente treinada pelo grupo de pesquisa, até a alta hospitalar ou óbito do RN. A coleta de dados do grupo controle histórico se deu retrospectivamente mediante registros em prontuário. Para avaliar o efeito da IOC sobre o ganho ponderal de crianças prematuras, realizou-se a modelagem de Equação de Estimação Generalizada (GEE). No período neonatal, identificou-se desaceleração do crescimento nos primeiros 7 dias de vida em ambos os grupos, seguida de maior aumento do ganho ponderal entre 21 e 28 dias no grupo IOC (Pré-IOC: média: 11,79; DP:10,24; IOC: média: 16,29; DP:11,72; p=0,05). Na análise multivariada, após ajuste para fatores de confusão, a IOC aumentou a velocidade de ganho de peso em 1,99 g/kg/dia (p<0,01) e o escore z por idade em 0,33 (p<0,01). Os dados do presente estudo sugerem que a IOC favoreceu o ganho ponderal em RNPT de muito baixo peso, no período neonatal. |

However, the result of this immunotherapy on weight gain in neonates has been little explored. For this reason, the present study evaluated the effect of oropharyngeal colostrum immunotherapy on the growth velocity of very low birth weight premature newborns admitted to a public maternal and child care unit. This is a non-randomized clinical trial with historical control. Premature newborns born weighing less than 1500g were allocated into two groups according to the period of their admission to the neonatal intensive care unit: in the control group, those admitted between 2015 and 2018, and in the intervention group, those admitted after the implantation of a IOC protocol, between 2019 and 2020. The treatment regimen consisted daily of 8 administrations of 0.2 ml (four drops) of colostrum for 10 seconds in the oropharyngeal mucosa, offered every 03 hours, until the seventh day of life of the child. Newborns were weighed naked, once a day, by the team of professionals from the sectors involved, previously trained by the research group, until hospital discharge or death. Data collection for the historical control group was carried out retrospectively through medical records. To assess the effect of the IOC on the weight gain of premature children, Generalized Estimation Equation (GEE) modeling was performed. In the neonatal period, growth deceleration was identified in the first 7 days of life in both groups, followed by a greater increase in weight gain between 21 and 28 days in the IOC group (Pre-IOC: mean: 11.79; SD: 10.24 ; IOC: mean: 16.29; SD: 11.72; p=0.05). In the multivariate analysis, after adjusting for confounding factors, IOC increased the rate of weight gain by 1.99 g/kg/day (p<0.01) and z-score for age by 0.33 (p<0 ,01). A Imunoterapia Orofaríngea de Colostro (IOC) auxilia o início e progressão da terapia nutricional, ao promover crescimento e maturação intestinal, reduzir eventos de intolerância alimentar e melhorar a evolução clínica de recém-nascidos prematuros. No entanto, o resultado desta imunoterapia sobre a evolução ponderal nos neonatos foi pouco explorado. Por isso, o presente estudo avaliou o efeito da IOC sobre o ganho ponderal de recém-nascidos prematuros (RNPT) com muito baixo peso, admitidos em uma unidade pública de atendimento materno-infantil. Trata-se de um ensaio clínico não-randomizado, com controle histórico. Os RNPT com menos de 1.500g foram alocados em dois grupos de acordo com o período de sua admissão na unidade de terapia intensiva neonatal: no controle os admitidos entre 2015 e 2018 e intervenção os admitidos após a implantação do Protocolo de IOC, entre 2019 e 2020. O esquema de tratamento foi constituído diariamente por 8 administrações de 0,2 ml (quatro gotas) de colostro por dez segundos em mucosa orofaríngea, oferecidos a cada três horas, até o sétimo dia de vida da criança. Os recém-nascidos (RN) foram pesados despidos, uma vez por dia, desde o nascimento, pela equipe de profissionais dos setores envolvidos, previamente treinada pelo grupo de pesquisa, até a alta hospitalar ou óbito do RN. A coleta de dados do grupo controle histórico se deu retrospectivamente mediante registros em prontuário. Para avaliar o efeito da IOC sobre o ganho ponderal de crianças prematuras, realizou-se a modelagem de Equação de Estimação Generalizada (GEE). No período neonatal, identificou-se desaceleração do crescimento nos primeiros 7 dias de vida em ambos os grupos, seguida de maior aumento do ganho ponderal entre 21 e 28 dias no grupo IOC (Pré-IOC: média: 11,79; DP:10,24; IOC: média: 16,29; DP:11,72; p=0,05). Na análise multivariada, após ajuste para fatores de confusão, a IOC aumentou a velocidade de ganho de peso em 1,99 g/kg/dia (p<0,01) e o escore z por idade em 0,33 (p<0,01). Os dados do presente estudo sugerem que a IOC favoreceu o ganho ponderal em RNPT de muito baixo peso, no período neonatal. |
