AvaliaÃÃo da cultura de seguranÃa do paciente entre mÃdicos de uma maternidade do nordeste brasileiro

Resumo: This dissertation brings a study about the evaluation of the safety culture of a maternity patient from a Brazilian Northeast, a reference in obstetric and gynecological care. With the evaluation of the safety culture of the patient being carried out from the application of the HSOPSC questionnaire, which brings dimensions considered as keys in the process of maintenance of safe care. The study was set up as a quantitative approach to the results, considering the self-perception of 124 physicians of the institution, among residents, statutory and outsourced. Among the main results are the on-call / shift tickets, internal transfers and the frequency of event notification as dimensions that need improvement to maintain a patient's safety culture. The other dimensions presented strength in relation to the safety culture of the patient (> 50%). Considering the feedback dimension in the communication, a weak point was found in the information received about changes implemented from the event reports since the percentage of Positive responses was below 50%. The reported error rate is higher among outsourced physicians (58.82%), while statutory physicians presented a rate of 29.41% and residents 11.76%. In relation to the occurrence of problems in the exchange of information between the hospital units being frequent, there is also a weak point, since the disagreement of the participants in relation to the affirmation was 40.32%. The fact that the care process was compromised when a patient is transferred from one unit to another was observed as a weakness in the dimension of on-call shifts, since the percentage of disagreement, despite being predominant, did not exceed 50%, there was a greater (44.62%) and statutory (46.43%) when compared to residents, while residents presented a higher percentage of disagreement (35.48%). In general, it was possible to verify the need for improvements in communication among physicians, it is believed that improving this sector with information about error notification as well as information of patients in shift shifts can obtain better results in patient safety.

Com a avaliaÃÃo da cultura de seguranÃa do paciente sendo realizada a partir da aplicaÃÃo do questionÃrio HSOPSC, que traz dimensÃes consideradas como chaves no processo de manutenÃÃo do atendimento seguro. O estudo se configurou como uma abordagem quantitativa dos resultados, sendo considerada a autopercepÃÃo de 124 mÃdicos da instituiÃÃo, entre residentes, estatutÃrios e terceirizados. Dentre os principais resultados cita-se as passagens de plantÃo/turno, transferÃncias internas e a frequÃncia de notificaÃÃo de eventos como dimensÃes que carecem de melhorias para a manutenÃÃo da cultura de seguranÃa do paciente. As demais dimensÃes apresentaram forÃa em relaÃÃo à cultura de seguranÃa do paciente (> 50%).Considerando a dimensÃo de retroalimentaÃÃo na comunicaÃÃo, foi verificado um ponto fraco no recebimento de informaÃÃes sobre mudanÃas implementadas a partir dos relatÃrios de eventos jà que o percentual de respostas positivas ficou abaixo de 50%. o Ãndice de erros relatados à maior entre mÃdicos terceirizados (58,82%), enquanto os mÃdicos estatutÃrios apresentaram Ãndice de 29,41% e os residentes 11,76%. Em relaÃÃo à ocorrÃncia de problemas na troca de informaÃÃes entre as unidades do hospital ser frequente tambÃm se tem ponto fraco, visto que a discordÃncia dos participantes em relaÃÃo à afirmaÃÃo ficou em 40,32%. Na dimensÃo de passagens de plantÃo verificou-se como ponto fraco o fato do processo de cuidado ser comprometido quando um paciente à transferido de uma unidade para outra, visto que o percentual de discordÃncia apesar de ser predominante nÃo ultrapassou 50%, verifica-se maior percentual de concordÃncia entre terceirizados (44,62%) e estatutÃrios (46,43%) quando comparados aos residentes, enquanto os residentes apresentaram maior percentual de discordÃncia (35,48%).
