Resumo: Background: The COPD assessment test (CAT) can provide a simple, efficient and reliable tool for assessing and monitoring COPD patients. Itâs important to perform the validation study of the Brazilian version of the instrument and therefore may become available for studies with the Brazilian population. The literature indicates the existence of different modes of questionnaires administration (selfadministered, interview, telephone), however there are many ways to influence the responses obtained from these instruments, culminating in the appearance of bias, especially in countries with low educational levels. Objectives: to perform the validation and reproducibility of the Brazilian version of CAT (group 1) and to compare psychometric properties of two forms of the questionnaire application: face to face interview and telephone. Methods: Group 1- There were selected patients with stable COPD from two hospitals in Fortaleza-CearÃ. CAT was administered in 50 patients on two occasions with an interval of 30 minutes, and a third time after one week. CAT has eight components: coughing, mucus production, chest tightness, capacity for exercise and activities, confidence, sleep quality and energy levels. Also the following tests were performed: Saint George questionnaire for respiratory diseases (SGRQ), pulmonary function tests, six-minute walk distance test (6MWD), modified index of dyspnea (MMRC) and hospital anxiety and depression scale (HADS). Group 2 â 100 patients were randomly assigned to either the face-to-face interviewer administered CAT (CAT-FFIA) or telephone interviewer administered CAT (CAT-TIA) group to asses internal consistency reliability, cross-sectional validity and test-retest reliability of the questionnaires. The patients answered the questionnaires twice, one week apart. In the same occasion, the patients answered the following questionnaires: SGRQ, HADS and MMRC. Results: Group 1 â CAT demonstrated excellent reproducibility, with an inter-rater ICC of 0.96 (95% CI: 0.93-0.97; p< 0.001) and an intra-rater ICC coefficient of 0.98 (95% CI: 0.96-0.98; p< 0.001). Bland-Altman plots showed good test-retest agreement for the CAT. The total score presented a significant correlation with SGRQ, espirometric values, 6MWDT, MRC and depression scores of HADS (p< 0.05). Group 2: For both administration formats, we found good internal consistency reliability (Crohnbach's alpha between 0,89 and 0.74 for the CAT-FFIA and CAT-TIA, respectively). In general, moderate-to-high correlations of CAT with SGRQ were observed, independent of the format of administration. For the test-retest reliability, the intraclass correlation coefficients were very similar for both CAT-FFIA and CAT-TIA group : 0,96 (95%CI 0,93-0,97) vs 0,98 (95%CI 0,96-0,98) respectively. In relation to discriminative properties of the CAT, it was observed that the questionnaire is able to assess the severity of COPD. The results showed significant differences in the scores of CAT in patients with moderate to severe COPD in both CAT-FFI and CAT-TIA. Conclusions: the Brazilian Portuguese version of CAT is a valid, reproducible and reliable instrument to evaluate the impact of COPD on patientâs lives. There were similarities in the psychometric properties when compared the format application of CAT (face to face interview versus telephone). Keywords:

ContextualizaÃÃo: o Teste de avaliaÃÃo da DPOC (CAT) à considerado uma ferramenta simples, eficiente e confiÃvel para a avaliaÃÃo e monitoramento de pacientes com doenÃa pulmonar obstrutiva crÃnica (DPOC). Desta feita à importante a realizaÃÃo do estudo de validaÃÃo da versÃo brasileira deste instrumento, podendo assim tornar-se disponÃvel para estudos com a populaÃÃo brasileira. A literatura aponta a existÃncia de diferentes modos de aplicaÃÃo de questionÃrios (auto-aplicÃvel, entrevista, telefone), portanto existem inÃmeras formas de influenciar as respostas obtidas a partir destes instrumentos, culminando no aparecimento de vieses, principalmente nos paÃses com nÃveis educacionais reduzidos. Objetivos: realizar a validaÃÃo e a reprodutibilidade da versÃo brasileira do CAT (grupo 1) e comparar as propriedades psicomÃtricas de duas formas de aplicaÃÃo do questionÃrio: entrevista face-aface e telefone. (grupo 2). MÃtodos: Grupo 1 - foram selecionados pacientes com DPOC estÃveis em dois hospitais de ensino na cidade de Fortaleza-CearÃ. O CAT foi aplicado em 50 pacientes em dois momentos, com intervalo de 30 minutos, e em um terceiro momento, apÃs uma semana. O CAT à composto de 8 itens: Tosse, Catarro, Aperto no peito, Falta de ar, LimitaÃÃes nas atividades domiciliares, ConfianÃa em sair de casa, Sono e Energia. Adicionalmente foi avaliado: questionÃrio Saint George para doenÃas respiratÃrias (SGRQ), teste de funÃÃo pulmonar, Teste da caminha de seis minutos (TC6), Ãndice modificado de dispneia (MMRC) e a escala hospitalar de ansiedade e depressÃo (HADS). Grupo 2 - 100 pacientes foram randomizados para receber o CAT no formato entrevista (CAT-EFF) e no formato telefone (CAT-ET), sendo avaliado a consistÃncia interna, validade e reprodutibilidade do questionÃrio. Os pacientes responderam ao questionÃrio duas vezes, com uma semana de intervalo. Na mesma ocasiÃo os pacientes responderam aos questionÃrios: SGRQ, HADS e MMRC. Resultados: Grupo 1 - O CAT evidenciou excelente reprodutibilidade tanto inter-examinador quanto intra-examinador, com um CCI de 0,98 (IC95% 0,96-0,98; p<0,001) e 0,96 (IC95% 0,93-0,97; p< 0,001)), respectivamente. Os grÃficos de Bland-Altman demonstraram boa concordÃncia no teste re-teste com o CAT. A pontuaÃÃo total do CAT apresentou correlaÃÃo significativa com o SGRQ, espirometria, TC6, MMRC e escores de depressÃo do HADS. Grupo 2 â Em ambos os formatos de aplicaÃÃo do CAT evidenciou-se boa consistÃncia interna (Coeficiente Alfa de Cronbach entre 0,89 e 0,74 para o CAT-EFF e CAT-ET, respectivamente). Houve correlaÃÃo de moderada a alta do CAT com o SGRQ em ambos os grupos. Em relaÃÃo a reprodutibilidade, o CCI de ambos os grupos foram similares: 0,96 (IC95% 0,93-0,97) no CAT-EFF e 0,98 (IC95% 0,96-0,98) no CAT-ET. Em relaÃÃo as propriedades discriminativas do CAT, observou-se que o questionÃrio à capaz de avaliar a gravidade da DPOC. Os resultados apontaram diferenÃa significativa da pontuaÃÃo do CAT nos pacientes com DPOC moderado e grave tanto no grupo CAT-EFF como no CAT-ET. ConclusÃo: A versÃo brasileira do CAT à vÃlida, reprodutÃvel e confiÃvel no tocante a avaliaÃÃo do impacto da DPOC na vida dos pacientes. Group 2 â 100 patients were randomly assigned to either the face-to-face interviewer administered CAT (CAT-FFIA) or telephone interviewer administered CAT (CAT-TIA) group to asses internal consistency reliability, cross-sectional validity and test-retest reliability of the questionnaires. The patients answered the questionnaires twice, one week apart. In the same occasion, the patients answered the following questionnaires: SGRQ, HADS and MMRC. Results: Group 1 â CAT demonstrated excellent reproducibility, with an inter-rater ICC of 0.96 (95% CI: 0.93-0.97; p< 0.001) and an intra-rater ICC coefficient of 0.98 (95% CI: 0.96-0.98; p< 0.001). Bland-Altman plots showed good test-retest agreement for the CAT. The total score presented a significant correlation with SGRQ, espirometric values, 6MWDT, MRC and depression scores of HADS (p< 0.05). Group 2: For both administration formats, we found good internal consistency reliability (Crohnbach's alpha between 0,89 and 0.74 for the CAT-FFIA and CAT-TIA, respectively). In general, moderate-to-high correlations of CAT with SGRQ were observed, independent of the format of administration. For the test-retest reliability, the intraclass correlation coefficients were very similar for both CAT-FFIA and CAT-TIA group : 0,96 (95%CI 0,93-0,97) vs 0,98 (95%CI 0,96-0,98) respectively. In relation to discriminative properties of the CAT, it was observed that the questionnaire is able to assess the severity of COPD. The results showed significant differences in the scores of CAT in patients with moderate to severe COPD in both CAT-FFI and CAT-TIA. Conclusions: the Brazilian Portuguese version of CAT is a valid, reproducible and reliable instrument to evaluate the impact of COPD on patientâs lives.
