Education to healthy as a nursing intervention strategy to people who are carriers of diabetes

Resumo: As a nurse in the program called “Programa Saúde da Família” it was possible to observe that carriers of diabetes, from Nova Aurora-GO, could not reduce their glycemic levels, even if they frequently receive orientations from the healthy staff. This study aimed to analyze an education health process with the carriers of diabetes, using one approach with Paulo Freire’s theoretical purposes and Arco de Maguerez’s methodological purposes adapted by Bordenave and Pereira. Besides that, we intend to evaluate the results from glycosylated hemoglobinbefore and after the implementation of this educational plan. It is an evaluative processual research with a qualitative approach and approved by Comitê de Ética em Pesquisa da Universidade Federal de Goiás. Nineteen carriers of diabetes took part in this educational action and seventeen took part in the glycosylated hemoglobin test.All the participants signed the informed consent and were informed about the intervention.  The educational action was carried out in six meetings. The glycosylated hemoglobin dosage was done in the beginning of the educational action and five months after its conclusion. What the group said, during the meetings, was recorded and analysed according to Bardin. In the investigation of the reality the emerging categories were related to nourishment, how people face the disease and feelings as triumph and frustration with the disease.  The topics diet, hypoglycemia / hyperglyecemia and the physiopathology of diabetes melitus were elected as key points to be theorized. In this stage, these categories emerged: dependency of the health professional and autonomy / independence to choose their food; lack of knowledge and knowledge in common sense level. When we were raising solution hypothesis the group created educational posters that showed clearly two categories: specific preventive actions to promote health to the group and health promotion and early diagnosis pointed toward community. The application to reality was done according to reports from carriers of diabetes about changes in their abilities and attitudes, besides autonomy and independence acquired by knowledge. The of the glycosylated hemoglobin results, the “t” test, shows a difference statistically significant with the decrease of plasma levels from dosed element.This study showed that a dialogical educational activity contributed to stimulate group’s reflective, critic and creative potential, as an important intervention strategy to the nurse who works in order to emancipate your clients.Key words: Health education; Communitarian health nursing; Diabetes mellitus.

El estudio objetivo analizar un proceso de educación para la salud junto a portadores de diabetes utilizando una aproximación entrwe los presupuestos teoricos de Paulo Freire metodológicos del Arco de Maguerez, adaptado por Bordenave y Pereira y avaluar los resultados de hemoglobina glicada antes y después de la implantación de este plano educativo. Se trata de una búsqueda avaliativa del procesal, con abordaje cualitativa y aprobada por el comité de Ética en búsqueda de la Universidad Federal de Goiás. Diecinueve portadores de diabetes participaron de la acción educativa y diecisiete del teste de hemoglobina glicada. Todos firmaron el documento de consentimiento libre y esclarecido y fueron informados sobre la propuesta de intervención. La acción educativa fue realizada en seis encuentros. La dosificación de la hemoglobina glicada ocurrió en el inicio de la acción educativa y cinco meses después de su encerramiento. Las conversaciones del grupo durante los encuentros, fueron gravadas y analizadas de acuerdo con Bardin. En la observación de la realidad las categorías emergentes se relacionaban a la alimentación, al enfrentamiento y a los sentimientos de éxito y frustración frente a la enfermedad. Los temas dieta hipoglicemica-hiperglicemica y la fisiopatología de diabetes melitus fueron elegidos como puntos clave a ser teorizados. En esta etapa emergieron las categorías: dependencia del profesional de salud y autonomía- independencia para que escojan sus alimentos; ausencia de conocimiento y el conocimiento en nivel de censo común. En la fase de hipótesis de solución, el grupo construyo carteles educativos que evidenciaron dos categorías: medidas preventivas de promoción de salud especificas para el grupo y promoción de salud y diagnostico precoz diseccionado a la comunidad. La aplicación a la realidad fue realizada mediante relatos de los portadores de diabetes cuando las mudanzas en sus habilidades y actitudes y el alcance de la autonomía e independencia conquistados por medio de dominio de conocimientos. En cuanto a los resultados de la hemoglobina glicada, el Tes. “t”, muestra una diferencia estadísticamente significativa, con disminución de los niveles plasmáticos de los elementos dosificados. El estudio evidencio que la actividad educativa dialógica, fue capaz de contribuir para el despertar del potencial reflexivo, critico y creativo del grupo,  presentándose como una importante estrategia e intervención para el enfermo que trabaja en la perspectiva de emancipación de sus clientes.Palabras clave: Educación en salud; Enfermería en salud comunitaria; Diabetes Mellitus.Enquanto enfermeira do Programa Saúde da Família foi possível observar que os portadores de diabetes, do município de Nova Aurora-GO, não conseguiam reduzir seus níveis glicêmicos, ainda que recebessem orientações freqüentes da equipe de saúde. O estudo objetivou analisar um processo de educação para a saúde junto a portadores de diabetes utilizando uma aproximação entre os pressupostos teóricos de Paulo Freire e metodológicos do Arco de Maguerez adaptado por Bordenave e Pereira e avaliar os resultados de hemoglobina glicada antes e após a implementação deste plano educativo. Trata-se de pesquisa avaliativa processual, com abordagem qualitativa e aprovada pelo Comitê de Ética em Pesquisa da Universidade Federal de Goiás. Dezenove portadores de diabetes participaram da ação educativa e dezessete do teste de hemoglobina glicada. Todos assinaram o termo de consentimento livre e esclarecido e foram informados acerca da proposta de intervenção. A ação educativa foi realizada em seis encontros. A dosagem de hemoglobina glicada ocorreu no início da ação educativa e cinco meses após seu encerramento. As falas do grupo, durante os encontros, foram gravadas e analisadas de acordo com Bardin. Na observação da realidade as categorias emergentes se relacionavam à alimentação, ao enfrentamento e aos sentimentos de êxito e frustração frente à doença. Os temas dieta, hipoglicemia / hiperglicemia e a fisiopatologia do diabetes melitus foram eleitos como pontos-chave a serem teorizados. Nesta etapa emergiram as categorias: dependência do profissional de saúde e autonomia / independência para a escolha de seus alimentos; ausência de conhecimento e o conhecimento em nível do senso comum. Na fase de hipóteses de solução o grupo construiu cartazes educativos que evidenciaram duas categorias: medidas preventivas de promoção da saúde específicas para o grupo e promoção da saúde e diagnóstico precoce direcionado à comunidade. A aplicação à realidade foi efetivada mediante relatos dos portadores de diabetes quanto às mudanças em suas habilidades e atitudes e o alcance da autonomia e independência conquistadas por meio do domínio de conhecimentos. Quanto aos resultados da hemoglobina glicada, o test “t”, mostra diferença estatisticamente significativa, com diminuição dos níveis plasmáticos do elemento dosado.
