NURSING ASSISTANCE SYSTEMATIZATION IN A MENTAL HEALTH CLINIC

Resumo: ABSTRACT: The aim of this work was to elaborate a model of nursing evaluation to be used in a clinic for mental insanity people; to describe the positive and negative aspects of the use of this model; and to identify the most frequent nursing diagnosis in a clinic for mental insanity people in a city of São Paulo state. A qualitative methodology approach was chosen, but some quantitative descriptions were used when analyzing the nursing diagnosis found frequency. From a bibliographical study, a checklist was used to know about insanity people’s health status. The data collection was initiated with the nursing consultation guided by this instrument, through physical and psychical examinations, after signing the informed consent term. The presentation and the discussion of the nursing diagnosis occurred according to the frequency of their appearance during the analysis of the insanity people’s health status; the criterion of discussion of these nursing diagnoses was its appearance with a frequency of 10 or more times in the nursing evaluation. The most frequent diagnosis were: hopelessness related to stresses, characterized by verbal indications (75,00%); social Isolation related to alterations in the mental conditions, characterized by expressions of rejection feelings (70,00%); and harmed verbal communication related to psychological barriers, characterized by absence of visual contact (65,00%). The experiencing with the nursing assistance systematization and this specific approach for insanity persons, based in the NANDA human reaction standards, has shown positive results. New additions to the instrument used in the psychosocial rehabilitation will promote greater benefits to such individuals. KEYWORDS: Psychiatric Nursing; Nursing Diagnosis; Mental Health.

KEYWORDS: Psychiatric Nursing; Nursing Diagnosis; Mental Health.RESUMEN: Este trabajo se objetivo en elaborar un modelo del histórico en enfermería, para ser utilizado en un ambulatorio de salud mental; hay que describir los puntos positivos y negativos de la utilización de este modelo; y levantar los diagnósticos de enfermería mas frecuentes en un ambulatorio de la salud mental, en el municipio del interior paulista. El método utilizado fue la abordaje cualitativo, con descripciones de cantidad en el análisis de la frecuencia de los diagnósticos encontrados en enfermería. A partir de un estudio bibliográfico se estableció un instrumento de colecta de datos (checklist) a respecto del estado de la salud de los enfermos mentales. Se inició la colecta de datos por intermedio de consultas de enfermería siendo guiados por este instrumento, por medio de exámenes físicos y psíquicos, después de la anuencia y la firma del término de consentimiento libre y esclarecido. La presentación y el debate de los diagnósticos de enfermería ocurrieron de acuerdo con la frecuencia con que aparecían durante el análisis del estado de salud de los enfermos mentales, siendo que el criterio del debate de los diagnósticos de enfermería fue lo que se presentó con frecuencia de 10 o más veces en los históricos de enfermería. Los diagnósticos de enfermería más frecuentes fueron: Desesperación relacionada al estrés prolongado, caracterizado por indicaciones verbales (75,00%); Aislado socialmente relacionado a las alteraciones en el estado mental, caracterizado por expresiones y sentimientos de rechazo (70,00%); y Comunicación verbal perjudicada relacionada a las barreras psicológicas, caracterizada por ausencia de contacto visual (65,00%). Las experiencias con la sistematización de la asistencia de enfermería y el modelo abordaje a los enfermos mentales se consiste en los padrones de respuesta humanas y tienen demostrado resultados positivos. Nuevos acrecimos al instrumento utilizado en la rehabilitación psico-social promoverán mayores beneficios a los individuos abordados. PALABRAS CLAVES: Enfermería Psiquiátrica; Diagnóstico de Enfermería; Salud Mental.RESUMO: Objetivou-se neste trabalho elaborar um modelo de histórico de enfermagem para ser utilizado em um ambulatório de saúde mental; descrever os pontos positivos e negativos da utilização deste modelo; e levantar os diagnósticos de enfermagem mais freqüentes num ambulatório de saúde mental em um município do interior paulista. O método utilizado foi a abordagem qualitativa, com algumas descrições quantitativas na análise da freqüência dos diagnósticos de enfermagem encontrados. A partir de um estudo bibliográfico estabeleceu-se um instrumento de coleta de dados (checklist) acerca do estado de saúde dos doentes mentais. Iniciou-se a coleta de dados através da consulta de enfermagem guiada por este instrumento, através de exames físico e psíquico, após anuência através da assinatura do termo de consentimento livre e esclarecido. A apresentação e a discussão dos diagnósticos de enfermagem ocorreram de acordo com a freqüência com que apareceram durante a análise do estado de saúde dos doentes mentais, sendo que o critério de discussão desses diagnósticos de enfermagem foi o de se apresentarem com freqüência de 10 ou mais vezes nos históricos de enfermagem. Os diagnósticos de enfermagem mais freqüentes foram: Desesperança relacionada a estresse prolongado, caracterizada por indicações verbais (75,00%); Isolamento social relacionado a alterações no estado mental, caracterizado por expressões e sentimentos de rejeição (70,00%); e Comunicação verbal prejudicada relacionada a barreiras psicológicas, caracterizada por ausência de contato visual (65,00%). As experiências com a sistematização da assistência de enfermagem e o modelo de abordagem do doente mental baseado nos padrões de respostas humanas têm mostrado resultados positivos. Novos acréscimos ao instrumento utilizado na reabilitação psico-social promoverão maiores benefícios aos indivíduos abordados.
