OS SENTIDOS DA MATERNIDADE PARA MULHERES-MÃES COM HIV/AIDS INVESTINDO NA VIDA PARA CONTINUAR VIVENDO

Resumo: HIV/Aids is found in an expressive way among women in Brazil, specially in the reproductive age group. After the discovery of the pregnancy with the HIV/AIDS diagnosis, the woman routine includes respecting medication schedule, consultations, exams, medical follow-up and general self-care. Furthermore, there is the likely identification of side affects originated by the anti-retroviral use. It is confirmed that the ARV decreases the risk of vertical transmission of the virus, although the consequences of the ARV for the child and the pregnancy have not been much discussed. The seropositive woman, who has or does not have this kind of information, builds her way inspired by the accomplishment of the mother status. The purpose of this investigation was to understand the relations between the process health-disease and the pregnancy meanings attributed by women with HIV. Because of this, we accomplished a qualitative approach study, following the bases of Social Strategic Research, in a non-governmental institution that aids mainly children, adolescents and women with HIV/AIDS, in Goiânia. Ten women with HIV/AIDS who got pregnant after the discovery of the seropositivity for HIV voluntarily took part in this study, according to the resolution 196/96, with the approval of the CEP/HC under the protocol 187/2007. These women were between 28 and 42 years old, were housewives, and had low formal educational level. Among these women, 5 were single and their families, government and/or non-governmental organizations helped them to support themselves. They admitted have been contaminated by the virus for more than two years, and the last delivery was about 3.7 years ago. In order to collect the data, we used semi-structured interviews, participative observation and photograph images. The data was analyzed with the Meaning Interpretation Method, according to the categories Believing in Life and Being a Mother with HIV. In general, the meanings of pregnancy for women with HIV/AIDS alter among the ideas of being source of worry, achieving a desired live phase, and continuing the family. The women admitted that the pregnancy was not planned, except for one. Four women revealed that the pregnancy was a result of sexual intercourse with a non-steady partner. Besides the meanings described, it was possible to notice that women, after pregnancy, were capable of adhering the antiretroviral treatment, even when it had side effects. The pregnancy made it possible to find a new meaning for their lives, promoting changes in the world view and in their behavior, as an educational model for their children. Because of this, we verified that the meaning of the pregnancy for a woman with HIV/AIDS is surrounded by autonomy feelings and the child is a synonym of hope for the reconstruction of a trajectory characterized by prejudice and discrimination

Após a descoberta da gravidez com o diagnóstico do HIV/Aids, o cotidiano da mulher inclui o atendimento aos horários de medicamentos, consultas, exames de acompanhamento e auto-cuidados em geral. Ainda se adiciona a isso a provável identificação de efeitos colaterais advindos do uso dos anti-retrovirais. Os anti-retrovirais (ARV) comprovadamente diminuem o risco de transmissão vertical do vírus, apesar de serem pouco discutidas as repercussões dos ARV na criança e na gravidez. Assim, o risco-benefício deve ser considerado para a escolha no uso de medicamentos. A mulher soropositiva, com posse ou não destas informações, constrói sua trajetória inspirada na realização de seu status de mãe. O objetivo desta investigação foi compreender as relações do processo saúde-doença com os significados da gravidez atribuídos por mulheres com HIV. Desse modo, realizamos estudo de abordagem qualitativa, seguindo os pressupostos da Pesquisa Social Estratégica, numa instituição não-governamental que assiste principalmente crianças, adolescentes e mulheres com HIV/Aids, no município de Goiânia. Participaram deste estudo 10 mulheres com HIV/Aids que engravidaram após a descoberta da soropositividade para HIV e que concordaram em participar de modo voluntário, conforme a Resolução 196/96, com aprovação do CEP/HC sob o protocolo 187/2007. Essas mulheres apresentavam idade entre 28 e 42 anos, desenvolviam atividades do lar, possuíam baixa escolaridade. Cinco delas eram solteiras e, para seu sustento, as mulheres eram auxiliadas pela família, governo e /ou ONG. Reconheceram estar contaminadas pelo HIV há mais de dois anos e, pelo último parto, em média de 3,7 anos. Para coleta de dados, utilizamos entrevistas semiestruturadas, observação participante e imagens fotográficas. Os dados foram analisados pelo Método de Interpretação de Sentidos, segundo as categorias Acreditando na Vida e Ser Mãe com HIV. De modo geral, os significados da gravidez para mulher com HIV/Aids transitam entre ser fonte de preocupação, concretização de uma etapa desejada da vida e dar continuidade à sua família. Com exceção de uma, as mulheres relataram que não planejaram a gravidez. Quatro delas revelaram que a gravidez adveio de relações sexuais com parceiro eventual. Além dos significados descritos, foi possível perceber que as mulheres, após a gravidez, foram capazes de persistir na adesão aos anti-retrovirais, mesmo quando estes ocasionam mal-estar. A gravidez possibilitou ressignificar suas vidas no sentido de promover mudanças de visão de mundo e de comportamento como modelo de educação para suas crianças.
