Resumo: | INTRODUCTION: Psychiatric Reform is being built in Brazil for over 20 years and is one of its main pillars deinstitutionalization. In the country, the National Mental Health (PNSM) follows a global trend the organization of community practices and home care in place procedures institutionalized for treatment. The mental health work represents great challenges because of its complexity, which are in the restructuring of the quality of care, through the labor relations and the adoption of new technologies, knowledge and work practices. This research is part of a multicenter project coordinated by the School of Nursing of Ribeirão Preto, University of Sao Paulo in partnership with the School of Nursing, Federal University of Goiás, which proposes to perform diagnostic studies on mental health care in Brazil. GENERAL PURPOSE: To present an overview of mental health services in the state of Goias METHODOLOGY: This is a descriptive-exploratory quantitative and qualitative approach, carried out in all municipalities in the state of Goias who provide public mental health, totaling 19 municipalities and 22 services. The subjects were professionals who have higher education level, as well as those who act as coordinators in services. Data were collected through the use of self-administered forms and screenplay for oral interview. For quantitative analysis were used relative and absolute frequencies in order to characterize the profiles of professionals, nurses and coordinators of Mental Health services. The data obtained through the interviews were submitted to thematic content analysis as proposed by Bardin, with the help of ATLAS.ti. RESULTS: The general practitioners, nurses and coordinators of Mental Health services within the State of Goiás have a multidisciplinary character, the majority are young, with little time for completion of graduation and have limited experience in Mental Health, with rare exceptions. Few have expertise in the area, employment relationships are precarious and wages are low. As for nurses, most of the actions that they develop the services are predominantly focused on general nursing duties, with little contribution to the therapeutic actions proposed by the model of psychosocial care. The coordinators of services have an understanding consistent with the PNSM, although there is still some conflicts regarding the real meaning of deinstitutionalization. The main challenges posed by them are related to the interest of municipal management by mental health services to the structure, to overcome the prejudice, the need for construction and expansion of a network of social services and resources, and the basic preparation and qualification. CONCLUSION: The interior of Goiás State does not have a network of psychosocial care services because they lack the articulation between existing ones are insufficient. Professionals and engineers who work in services still lack adequate training to work in mental health. Engineers pointed challenges to implementation of PNSM corroborate the national scene. |

No país, a Política Nacional de Saúde Mental (PNSM) segue uma tendência mundial de organização de práticas comunitárias e de cuidado domiciliar em substituição aos procedimentos institucionalizados para tratamento. O trabalho em saúde mental representa grandes desafios, em virtude de sua complexidade, os quais vão da reestruturação dos serviços à qualidade da assistência prestada, passando pelas relações de trabalho e pela adoção de novas tecnologias, conhecimentos e práticas de trabalho. Esta pesquisa faz parte um do projeto multicêntrico coordenado pela Escola de Enfermagem de Ribeirão Preto da Universidade de São Paulo em parceria com a Faculdade de Enfermagem da Universidade Federal de Goiás, que propõe realizar estudos diagnósticos sobre assistência em saúde mental no Brasil. OBJETIVO GERAL: Apresentar o panorama dos serviços de Saúde Mental do interior do Estado de Goiás. METODOLOGIA: Pesquisa descritivo-exploratória de abordagem quantitativa e qualitativa, realizado em todos os municípios do interior do Estado de Goiás que possuem serviços públicos de Saúde Mental, totalizando 19 municípios e 22 serviços. Foram sujeitos os profissionais que possuem nível superior de escolaridade, assim como os que atuam como coordenadores nos serviços. Os dados foram coletados por meio da utilização de formulários auto-aplicáveis e roteiro para entrevista oral. Para a análise quantitativa foram utilizadas frequências relativas e absolutas a fim de caracterizar os perfis dos profissionais, enfermeiros e coordenadores dos serviços de Saúde Mental. Os dados obtidos por meio das entrevistas foram submetidos à análise temática de conteúdo conforme propõe Bardin, com auxílio do ATLAS.ti. RESULTADOS: Os profissionais em geral, enfermeiros e coordenadores dos serviços de Saúde Mental no interior do Estado de Goiás possuem característica multidisciplinar, a maioria é jovem, com pouco tempo de conclusão da graduação e possuem restrita experiência na área de Saúde Mental, com raras exceções. Poucos possuem especialização na área, os vínculos empregatícios são precários e os salários são baixos. Quanto aos enfermeiros, a maioria das ações que eles desenvolvem nos serviços são predominantemente centradas nas atribuições gerais de enfermagem, com pouca contribuição com as ações terapêuticas propostas pelo modelo de atenção psicossocial. Os coordenadores dos serviços possuem uma compreensão condizente com a PNSM, apesar de ainda haver alguns conflitos com relação ao real sentido da desinstitucionalização. Os principais desafios apresentados por eles estão relacionados ao interesse da gestão municipal pela saúde mental; à estrutura dos serviços; à superação do preconceito; à necessidade de construção e ampliação de uma rede de serviços e recursos sociais; e a fundamental preparação e qualificação dos profissionais. CONCLUSÃO: O interior Estado de Goiás ainda não possui uma rede de atenção psicossocial porque faltam serviços e a articulação entre os já existentes é insuficiente. Os profissionais e coordenadores que atuam nos serviços ainda não possuem uma formação adequada para atuar na saúde mental.
