Temporal trend and the epidemiological profile of congenital syphilis in Ceará (2009-2018)

Resumo: Introduction: In the last decade, there has been an increase in the prevalence of congenital syphilis (SC) in some regions, and Brazil led the number of cases in Latin America. Objective: To analyze the time trend and the clinical-epidemiological profile of Congenital Syphilis in Ceará between 2009 and 2018. Material and Methods: This is an ecological type epidemiological study with a time series approach, with the state of Ceará as a scenario. The data came from the Notifiable Diseases Information System (SINAN), Mortality Information System (SIM), Live Birth Information System (SINASC) and the Department of Diseases of Chronic Conditions and Sexually Transmitted Infections. Data analysis was performed using the Statistical Program for Social Sciences (SPSS) 20.0 statistical package and Microsoft Excel 2013®, proceeding with descriptive and inferential analysis of the data, using simple linear regression, which was called as crescent, stationary and decreasing, according to the slope. To verify normality, the Shapiro-Wilk test was performed. Results: In the analyzed time series, 10,392 cases of SC were recorded, with an increase of 91.63% in the comparison between the years 2009 and 2018, as well as an increase in the incidence rate, going from 5 to 9.57 cases / 1,000 born in the same period. The number of treated partners (15.25%) and mothers who performed the treatment adequately (4.72%) remained low. Mothers who taught higher education accounted for only 0.55% of total notifications; while almost 90% of the notified mothers were brown. Conclusion: The results demonstrate an increase in congenital syphilis stroke accompanied by inadequate therapy, highlighting the need to improve the quality of prenatal care and other scenarios of comprehensive sexual and reproductive health care.

Objetivo: Analisar a tendência temporal e o perfil clínico-epidemiológico da sífilis congênita no Ceará entre 2009 e 2018. Materiais e Métodos: Trata-se de um estudo epidemiológico do tipo ecológico com abordagem de séries temporais, tendo como cenário o estado do Ceará. Os dados foram provenientes do Sistema de Informação de Agravos de Notificação (SINAN), Sistema de Informações sobre Mortalidade (SIM), Sistema de Informações sobre Nascidos Vivos (SINASC) e Departamento de Doenças de Condições Crônicas e Infecções Sexualmente Transmissíveis. A análise dos dados foi realizada no pacote estatístico Statistical Program For Social Sciences (SPSS) 20.0 e no Microsoft Excel 2013®, procedendo-se análise descritiva e inferencial dos dados, por meio de regressão linear simples, a qual foi classificada como crescente, estacionária e decrescente, de acordo com o coeficiente angular. Para verificar a normalidade foi realizado o teste de Shapiro-Wilk. Resultados: Na série temporal analisada contabilizou-se 10.392 casos de SC, havendo um aumento de 91,63% na comparação entre os anos de 2009 e 2018, assim como aumento da taxa de incidência, passando de 5 para 9,57 casos/1.000 nascidos no mesmo período. O número de parceiros tratados (15,25%) e de mães que realizaram o tratamento de forma adequada (4,72%) permaneceram baixos. Mães que tinham ensino superior corresponderam apenas a 0,55% do total das notificações; enquanto que quase 90% das mães notificadas eram pardas.
