Evaluation of the efficacy of the arthrocentesis associated with viscosupplementation in the treatment of the temporomandibular joint disorders: case report

Introduction: Arthrocentesis is a minimally invasive technique used for the treatment of temporomandibular disorders. Consists at the lavage of the upper articular space, increasing articular mobility and reducing pain. This technique can be associated with viscossuplementation, that can be defined as the injection of hyaluronic acid in the temporomandibular joint (TMJ), decreasing joint cartilage wear. Objective: To analysis the effectiveness of the arthrocentesis and viscossuplementation, highlighting its benefits, through a case report. Case Report: Pacient, E.O.M, female, 30 years, reported to Santa Casa de Misericórdia, Juiz de Fora, with main complain of pain and difficulty of mouth opening.  The DC/TMD Axis I questionnaire was conducted, and was verified hypomobility of the TMJ, with maximal mouth opening of 25 mm, clicking, crepitation and pain of the pre-auricular region. The diagnostic hypothesis was disc displacement with reduction confirmed by Magnetic Resonance Imaging (MRI). It was initial treatment with occlusal splint and farmacological therapy, but no improvement of the symptoms was noticed. Thus, arthrocentesis associated with viscossuplementation was indicated, in both joints. Results: it was observed improvement of the symptoms of the patient. The maximal mouth opening increased from 25mm to 36 mm, after 30 days of the procedure. The length of the follow-up was two years. There were no signs of relapses. Conclusion: The artrocenthesis associated with viscossuplementation is an effective method for treatment of temporomandibular disorders, mainly for refractory patients for conventional treatment being considered a safe technique.
