Resumo: Asthma is a complex syndrome, of difficult diagnosis in children. The mechanisms involved in the etiopathogeny of the disease have not been completely elucidated. Environmental exposures during the first 1000 days of life (270 days of gestation plus the 365 days of the first year of life and 365 days of the second of life) appear to have a strong influence on epigenetic regulation, increasing predisposition to childhood asthma. The association between early exposures of the prenatal period, birth and the first years of life with asthma symptoms in children involves multicausality and temporality relationships between these variables that can be better studied by structural equation modeling (SEM). Chapter I of this thesis was the original article: The First 1000 Days of Life Factors Associated with “Childhood Asthma Symptoms”: Brisa Cohort, São Luís, Brazil, which aimed to analyze the associations between the environmental factors up to the first 1000 days of life and “Childhood Asthma Symptoms”. It is a prospective study involving three moments of the BRISA cohort in Sao Luis (n = 1140), where the outcome was the latent variable “Childhood Asthma Symptoms”, formed by four indicators with convergent loads: medical diagnosis of asthma, number of wheezing episodes, emergency care visit due to wheezing and medical diagnosis of rhinitis. A theoretical model that included prenatal factors, birth factors and of the second year of life, were analyzed in association with “Childhood Asthma Symptoms” using SEM. It was observed that the higher the pre-gestational BMI, high soft drink consumption in gestation, cesarean section without labor, chill in the first three months of life, carpeted floor and child´s exposure to tobacco were associated with higher values of “Childhood Asthma Symptoms”. In contrast, high birth weight, the age of the child and children exclusively breastfed for six months were associated with lower values of “Childhood Asthma Symptoms”. Chapter II of this thesis was the original article: Environmental factors linked microbiota hypothesis and “Childhood Asthma Symptoms”: BRISA cohort, Ribeirão Preto, Brazil, which aimed to analyze environmental factors linked to the microbiota hypothesis and “Childhood Asthma Symptoms”. It is a prospective study involving population-based sample of the BRISA birth cohort in Ribeirão Preto (n = 3694). Birth and second year life factors linked to the microbiota hypothesis were analyzed on a theoretical model having as outcome the latent variable “Childhood Asthma Symptoms” was formed by three indicators (medical diagnosis of asthma, number of wheezing episodes and emergency care visit due to wheezing) using SEM. It was observed that children with higher BMI values, belonging to the black race, with diarrhea and older presented higher values for “Childhood Asthma Symptoms”; while better economic situation, female gender and longer breastfeeding presented lower values for “Childhood Asthma Symptoms”. In the present thesis, the analyzes of the variable “Childhood Asthma Symptoms” as a continuous latent formed by clinical indicators of convergent loads reduced the error of measurement of this outcome difficult to diagnose in children. Our findings suggest that environmental exposures during the first 1000 days of life, such as: maternal and child overweight, consumption of soft drinks in gestation, cesarean delivery, would be involved in the early programming of asthma children, while breastfeeding would protect from this condition.

Os mecanismos envolvidos na etiopatogenia da doença ainda não foram completamente elucidados. Exposições ambientais durante os primeiros 1000 dias de vida (270 dias da gestação mais os 365 dias do primeiro ano de vida e os 365 dias do segundo de vida) parecem ter forte influência na regulação epigenética, aumentando a predisposição à asma na infância. A associação entre exposições precoces do período pré-natal, do nascimento e dos primeiros anos de vida com sintomas de asma em crianças envolvem relações de multicausalidade e temporalidade entre essas variáveis que podem ser melhor estudadas por modelagem de equações estruturais (structural equation modeling - SEM). O capítulo I desta tese foi o artigo original: “Fatores associados aos primeiros 1000 dias de vida e “Sintomas de Asma na Infância”: coorte BRISA, São Luís, Brasil”, que teve como objetivo analisar as associações entre fatores ambientais até os primeiros 1000 dias de vida e “Sintomas de Asma na Infância”. Trata-se de um estudo prospectivo envolvendo três momentos da coorte de pré-natal BRISA de São Luís (n=1140), onde o desfecho foi a variável latente “Sintomas de Asma na Infância”, formada por quatro indicadores com cargas convergentes: diagnóstico médico de asma, número de episódios de chiado, emergência médica devido chiado intenso e diagnóstico médico de rinite. Um modelo teórico que incluiu fatores do pré-natal, do nascimento e do segundo ano foram analisados em associação com “Sintomas de Asma na Infância” através da SEM. Como resultados, observou-se que maiores valores de IMC pré-gestacional, consumo elevado de refrigerantes durante à gestação, crianças nascidas por cesárea sem trabalho de parto, presença de resfriado nos três primeiros meses de vida, piso revestido de carpete e exposição da criança ao tabagismo passivo foram associados a maiores valores de “Sintomas de Asma na Infância”. Por outro lado, o maior peso ao nascer, a maior idade da criança e crianças amamentadas exclusivamente por seis meses foram associados com menores valores de “Sintomas de Asma na Infância”. O capítulo II desta tese foi o artigo original: “Fatores ambientais ligados à Hipótese da Microbiota e “Sintomas de Asma na Infância”: coorte BRISA, Ribeirão Preto, Brasil”, teve como objetivo analisar fatores ambientais ligados à hipótese da microbiota e “Sintomas de Asma na Infância”. Estudo prospectivo envolvendo uma amostra de base populacional da coorte de nascimento BRISA em Ribeirão Preto (n=3694). Fatores do nascimento e do segundo ano de vida das crianças ligados à hipótese da microbiota foram analisados em um modelo teórico tendo como desfecho a variável latente “Sintomas de Asma na Infância” (diagnóstico médico de asma, número de episódios de chiado e emergência médica devido chiado intenso) através da SEM. Observou-se que crianças com maiores valores do escore-z do IMC por idade, da raça negra, com presença de diarreia e com maior idade apresentaram maiores valores para “Sintomas de Asma na Infância”; enquanto que uma melhor situação socioeconômica, sexo feminino e maior tempo de amamentação apresentaram menores valores para “Sintomas de Asma na Infância”. Na presente tese, as análises da variável “Sintomas de Asma na Infância” como uma latente contínua formada por indicadores clínicos de cargas convergentes reduziu o erro de aferição desse desfecho de difícil diagnóstico em crianças. description Asthma is a complex syndrome, of difficult diagnosis in children. The mechanisms involved in the etiopathogeny of the disease have not been completely elucidated. Environmental exposures during the first 1000 days of life (270 days of gestation plus the 365 days of the first year of life and 365 days of the second of life) appear to have a strong influence on epigenetic regulation, increasing predisposition to childhood asthma. The association between early exposures of the prenatal period, birth and the first years of life with asthma symptoms in children involves multicausality and temporality relationships between these variables that can be better studied by structural equation modeling (SEM). Chapter I of this thesis was the original article: The First 1000 Days of Life Factors Associated with “Childhood Asthma Symptoms”: Brisa Cohort, São Luís, Brazil, which aimed to analyze the associations between the environmental factors up to the first 1000 days of life and “Childhood Asthma Symptoms”. It is a prospective study involving three moments of the BRISA cohort in Sao Luis (n = 1140), where the outcome was the latent variable “Childhood Asthma Symptoms”, formed by four indicators with convergent loads: medical diagnosis of asthma, number of wheezing episodes, emergency care visit due to wheezing and medical diagnosis of rhinitis. A theoretical model that included prenatal factors, birth factors and of the second year of life, were analyzed in association with “Childhood Asthma Symptoms” using SEM. It was observed that the higher the pre-gestational BMI, high soft drink consumption in gestation, cesarean section without labor, chill in the first three months of life, carpeted floor and child´s exposure to tobacco were associated with higher values of “Childhood Asthma Symptoms”. In contrast, high birth weight, the age of the child and children exclusively breastfed for six months were associated with lower values of “Childhood Asthma Symptoms”. Chapter II of this thesis was the original article: Environmental factors linked microbiota hypothesis and “Childhood Asthma Symptoms”: BRISA cohort, Ribeirão Preto, Brazil, which aimed to analyze environmental factors linked to the microbiota hypothesis and “Childhood Asthma Symptoms”. It is a prospective study involving population-based sample of the BRISA birth cohort in Ribeirão Preto (n = 3694). Birth and second year life factors linked to the microbiota hypothesis were analyzed on a theoretical model having as outcome the latent variable “Childhood Asthma Symptoms” was formed by three indicators (medical diagnosis of asthma, number of wheezing episodes and emergency care visit due to wheezing) using SEM. It was observed that children with higher BMI values, belonging to the black race, with diarrhea and older presented higher values for “Childhood Asthma Symptoms”; while better economic situation, female gender and longer breastfeeding presented lower values for “Childhood Asthma Symptoms”. In the present thesis, the analyzes of the variable “Childhood Asthma Symptoms” as a continuous latent formed by clinical indicators of convergent loads reduced the error of measurement of this outcome difficult to diagnose in children. Our findings suggest that environmental exposures during the first 1000 days of life, such as: maternal and child overweight, consumption of soft drinks in gestation, cesarean delivery, would be involved in the early programming of asthma children, while breastfeeding would protect from this condition.
