Análise da qualidade de vida do Policial Militar do Maranhão e correlação com: qualidade do sono, índices de obesidade central e estado hemodinâmico de repouso

Resumo: | Objective: to describe and correlate quality of life, quality of sleep, state of drowsiness, Index of Central Obesity, and hemodynamic state of rest of military police (MPs) from Maranhão. Materials and Methods: sample: 404 MPs; 359 males (m; median age 33 years); 45 females (f; median age 32 years). Assessments: quality of life (QOL - WHOQOL-Bref); Pittsburgh Sleep Quality Index (PSQI); Epworth Sleepiness Scale (ESS); Results: MPs-m, had higher medians (p <0.05) of: systolic blood pressure (SBP - 132 vs 117 mmHg); diastolic blood pressure (PAD - 83 vs 75 mmHg). 57.6% of MPs-m were classified as prehypertensive or hypertensive. Higher means (p <0.05) of: body mass index (BMI - 27.4 vs 25.9 kg / m2); waist circumference (WC - 94.0 vs 86.6 cm) and waist-to-hip ratio (WHR - 0.92 vs 0.84 cm). The MPs-f, have higher averages (p <0.05) of: sum of skinfolds (92.6 vs 68.8 mm) and body fat percentage (25.8 vs 17.8% G). For MPs of both sexes, more than 50% are at relative risk for chronic noncommunicable diseases (CNCD), observed by the obesity indexes. Both sexes, presented physical, psychological and social relations domains of approximately 70 points and environment of approximately 60 points. For poor sleep (PSQI), approximately 60% of MPs and sleep disturbance, 13.5% of MPs-m and 8.9% of MPs-f. For ESS, excessive daytime sleepiness (EDS) was observed in 30.1% of MPs-m and 37.8% of MPs-f. In correlations (p <0.05) regardless of sexe: PSQI with physical, psychological, social and environmental domains (r = -0.421; -0.300, -0.280 and - 0.261); MPs-m (r = -0.427; -0.288; -0.275 and -0.283), respectively. MPs-f, PSQI with physical, psychological and social domains (r = -0.422; -0.385 and -0.311) and ESS (r = -0.389; -0.392 and -0.289), respectively. For MPs-m, waist-to-height ratio (WHtR), WHR and conicity index (CI), with age (r = 0.438; 0.490; 0.469) and total working time (TWT) (r = 0.414; 0.460; 0.420), respectively. For MPs-f, BMI with TWT, military work time (MWT), PAD and PAM (r = 0.550; 0.401; 0.542; 0.462); WC with TWT, MWT, military operational working time (MOWT) and PAD (r = 0.440; 0.512; 0.501; 0.511; 0.431), respectively. WHtR with TWT (r = 0.467); CI with TWT (r = 0.404). Discussion: The advancement of age and working time showed a positive correlation with the Index of Central Obesity and arterial hypertension (AH). MPs-m showed higher values and prevalence of these variables, suggesting greater risks for CNCD. For QOL, the domains were classified as regular and showed a negative correlation with sleep quality. Poor sleep affects approximately 60% of the MPs, approximately 10% had sleep disorders and more than 30% had EDS, with the MPs-f in more compromised conditions in this last variable. Conclusion: the results demonstrate health impairment, assessed by blood pressure and obesity rates. In addition, the study demonstrated impaired sleep and QOL, highlighting the need for interventions to reverse this condition. |

Materiais e Métodos: amostra: 404 PMs (359 masculinos (m; mediana idade 33 anos); 45 femininos (f; mediana idade 32 anos). Avaliações: qualidade de vida (QV - WHOQOL-Bref); Índice de Qualidade do Sono de Pittsburgh (PSQI); Escala de Sonolência de Epworth (ESE); Resultados: PMs-m, apresentaram medianas maiores (p<0,05): pressão arterial sistólica (PAS - 132 vs 117 mmHg); pressão arterial diastólica (PAD - 83 vs 75 mmHg). 57,6% dos PMs-m foram classificados com pré-hipertensão ou hipertensão. Médias maiores (p<0,05): índice de massa corporal (IMC - 27,4 vs 25,9 kg/m2); circunferência da cintura (CC - 94,0 vs 86,6 cm) e relação cintura e quadril (RCQ - 0,92 vs 0,84 cm). Os PMs-f, apresentam médias maiores (p<0,05): somatório das dobras cutâneas (92,6 vs 68,8 mm) e percentual de gordura corporal (25,8 vs 17,8 %G). Para os PMs de ambos os sexos, mais de 50% encontram-se em relativo risco para doenças crônicas não transmissíveis (DCNT), observado pelos índices de obesidade. Para QV, PMs de ambos os sexos, apresentaram domínios físico, psicológico e relações sociais de aproximadamente 70 pontos e meio ambiente de aproximadamente 60 pontos. Para sono ruim (PSQI), aproximadamente 60% dos PMs e distúrbio do sono, 13,5% dos PMs-m e 8,9% dos PMs-f. Para a ESE, observou-se sonolência excessiva diurna (SED) em 30,1% dos PMs-m e 37,8% dos PMs-f. Nas correlações (p<0,05) independente do sexo: PSQI com domínios físico, psicológico, social e ambiental (r=-0,421; -0,300, -0,280 e - 0,261); PMs-m (r=-0,427; -0,288; -0,275 e -0,283), respectivamente. PMs-f, PSQI com domínios físico, psicológico e social (r=-0,422; -0,385 e -0,311) e ESE (r=- 0,389; -0,392 e -0,289), respectivamente. Para os PMs-m, relação cintura estatura (RCE), RCQ e índice de conicidade (IC), com idade (r=0,438; 0,490; 0,469) e tempo total de trabalho (TTT) (r=0,414; 0,460; 0,420), respectivamente. Para os PMs-f, IMC com TTT, tempo de trabalho militar (TTM), PAD e PAM (r=0,550; 0,401; 0,542; 0,462); CC com idade, TTT, TTM, tempo de trabalho operacional militar (TTOM) e PAD (r=0,440; 0,512; 0,501; 0,511; 0,431), respectivamente. RCE com TTT (r=0,467); IC com TTM (r=0,404). Discussão: O avançar da idade e tempo de trabalho, apresentaram correlação positiva com os índices de obesidade central e hipertensão arterial (HA). PMs-m apresentaram maiores valores e prevalência destas variáveis, sugerindo maiores riscos para DCNT. Para QV, os domínios foram classificados como regular e demonstraram correlação negativa com qualidade do sono. O sono ruim atinge aproximadamente 60% dos PMs, aproximadamente 10% apresentaram distúrbios do sono e mais de 30% apresentaram SED, com os PMs- f em condições mais comprometidas nesta última variável. Conclusão: os resultados demonstram comprometimento à saúde, avaliado pela pressão arterial e índices de obesidade. Objective: to describe and correlate quality of life, quality of sleep, state of drowsiness, Index of Central Obesity, and hemodynamic state of rest of military police (MPs) from Maranhão. Materials and Methods: sample: 404 MPs; 359 males (m; median age 33 years); 45 females (f; median age 32 years). Assessments: quality of life (QOL - WHOQOL-Bref); Pittsburgh Sleep Quality Index (PSQI); Epworth Sleepiness Scale (ESS); Results: MPs-m, had higher medians (p <0.05) of: systolic blood pressure (SBP - 132 vs 117 mmHg); diastolic blood pressure (PAD - 83 vs 75 mmHg). 57.6% of MPs-m were classified as prehypertensive or hypertensive. Higher means (p <0.05) of: body mass index (BMI - 27.4 vs 25.9 kg / m2); waist circumference (WC - 94.0 vs 86.6 cm) and waist-to-hip ratio (WHR - 0.92 vs 0.84 cm). The MPs-f, have higher averages (p <0.05) of: sum of skinfolds (92.6 vs 68.8 mm) and body fat percentage (25.8 vs 17.8% G). For MPs of both sexes, more than 50% are at relative risk for chronic noncommunicable diseases (CNCD), observed by the obesity indexes. Both sexes, presented physical, psychological and social relations domains of approximately 70 points and environment of approximately 60 points. For poor sleep (PSQI), approximately 60% of MPs and sleep disturbance, 13.5% of MPs-m and 8.9% of MPs-f. For ESS, excessive daytime sleepiness (EDS) was observed in 30.1% of MPs-m and 37.8% of MPs-f. In correlations (p <0.05) regardless of sexe: PSQI with physical, psychological, social and environmental domains (r = -0.421; -0.300, -0.280 and - 0.261); MPs-m (r = -0.427; -0.288; -0.275 and -0.283), respectively. MPs-f, PSQI with physical, psychological and social domains (r = -0.422; -0.385 and -0.311) and ESS (r = -0.389; -0.392 and -0.289), respectively. For MPs-m, waist-to-height ratio (WHtR), WHR and conicity index (CI), with age (r = 0.438; 0.490; 0.469) and total working time (TWT) (r = 0.414; 0.460; 0.420), respectively. For MPs-f, BMI with TWT, military work time (MWT), PAD and PAM (r = 0.550; 0.401; 0.542; 0.462); WC with TWT, MWT, military operational working time (MOWT) and PAD (r = 0.440; 0.512; 0.501; 0.511; 0.431), respectively. WHtR with TWT (r = 0.467); CI with TWT (r = 0.404). Discussion: The advancement of age and working time showed a positive correlation with the Index of Central Obesity and arterial hypertension (AH). MPs-m showed higher values and prevalence of these variables, suggesting greater risks for CNCD. For QOL, the domains were classified as regular and showed a negative correlation with sleep quality. Poor sleep affects approximately 60% of the MPs, approximately 10% had sleep disorders and more than 30% had EDS, with the MPs-f in more compromised conditions in this last variable. Conclusion: the results demonstrate health impairment, assessed by blood pressure and obesity rates. In addition, the study demonstrated impaired sleep and QOL, highlighting the need for interventions to reverse this condition. |
