Congenital syphilis: epidemiology of a city in the state of Espirito Santo, Brazil

Resumo: OBJECTIVE: to describe the epidemiological profile of cases of congenital syphilis reported to the city health department in Serra from 2000 to 2004. METHODS: the collected data come from the forms of the cases investigation reported to the administration of the epidemiological surveillance through the information system of reported injuries of the national health system (Sinan - SUS). The following variables were used: maternal - age, level of schooling, prenatal care, partner's treatment and diagnosed during pregnancy; and the child - gestational age at birth, clinical evidence and final diagnosis. Descriptive analyses were obtained using the Windows TAB WIN program (version 5.2 - 2005). RESULTS: the prevalence of the positive serology for congenital syphilis is still high with 233 cases reported.The mothers' age was between 20 and 29 accounting for 56.7 % of the cases.The level of schooling was from 1 to 3 years of study (34.7%). 72.1% of the mothers said they had done prenatal care and 53.6% of those confirmed were diagnosed for syphilis during pregnancy. Only 26.6% of partners were given treatment. Child's variables were: 77.3% of the babies were born term, asymptomatic in 63.1% and in 78.1% of the cases, the final diagnose was of early congenital syphilis. CONCLUSION: These findings show the characteristics of the congenital syphilis reported cases and indirectly assess the quality of prenatal care given to the pregnant women in Serra, state of Espírito Santo, Brazil.

MÉTODOS: los datos se recogieron de las fichas de investigación de los casos notificados a Vigilancia Epidemiológica a través del sistema de información de la salud (SINAN - SUS). Las variables utilizadas fueron: materna - edad, nivel de instrucción, cuidado prenatal, tratamiento del compañero y realización de diagnóstico durante el embarazo; del niño - edad gestacional, manifestaciones clínicas y diagnóstico final. Se efectuaron análisis descriptivos con el programa Windows (TAB WIN), versión 5.2 - 2005. RESULTADOS: se constató que la prevalencia de serología positiva para sífilis congénita sigue siendo alta, con 233 casos notificados; edad de las madres entre 20 y 29 para 56.7 % de los casos. En relación al nivel de instrucción: predominio de entre 1 y 3 años de estudio (34.7%). 72.1% de las madres informaron haber realizado prenatal y 53.6% de ellas confirmaron que la sífilis había sido diagnosticada durante el embarazo. En sólo 26,6% de los casos el compañero se sometió a tratamiento.Variables del niño: 77.3% de los bebés nacieron a término, 63.1% asintomáticos y en 78.1% de los casos el diagnóstico final era de sífilis congénita temprana. CONCLUSIÓN: Estos resultados permiten conocer las características de los casos notificados de sífilis congénita y evaluar, indirectamente, la calidad del cuidado prenatal brindado a las mujeres embarazadas de Serra/ES.OBJETIVO: descrever o perfil epidemiológico dos casos de sífilis congênita notificados à Secretaria Municipal de Saúde da Serra entre 2000 e 2004. MÉTODOS: os dados coletados provêm das fichas de investigação dos casos notificados à Diretoria de Vigilância Epidemiológica pelo Sistema de Informação de Agravos de Notificação do Sistema Único de Saúde (Sinan - SUS). Foram utilizadas as variáveis: materna - faixa etária, grau de instrução, realização de pré-natal, tratamento do parceiro e realização de diagnóstico na gravidez - e da criança - idade gestacional ao nascer, manifestações clínicas e diagnóstico final. Foram realizadas análises descritivas com base no programa Tabulação para Windows (TAB WIN), versão 5.2 - 2005. RESULTADOS: constatou-se que a prevalência de sorologia positiva para sífilis congênita ainda é alta, tendo sido notificados 233 casos. A faixa etária das mães prevaleceu entre 20-29 anos, sendo 56,7% dos casos. Em relação ao grau de instrução, o predomínio foi de um a três anos de estudo (34,7%). Quanto ao pré-natal, 72,1% das mães disseram que tiveram essa assistência e 53,6% confirmaram ter seu diagnóstico de sífilis estabelecido durante a gestação. Em apenas 26,6% foi realizado tratamento do parceiro. Em relação às variáveis da criança, observou-se que 77,3% dos bebês nasceram a termo, 63,1% assintomáticos e em 78,1% dos casos o diagnóstico final foi de sífilis congênita precoce.
