Limits and possibilities of working with groups within the family health strategy

Resumo: This qualitative study was based on Madeleine Leininger's theory of Culture Care Diversity and Universality. It was carried out in eight basic health care units within a sanitary district of Belo Horizonte-MG. The study aimed to discuss the limits and possibilities of working with groups of diabetic and hypertensive patients within the Family Health Strategy. Nineteen professionals coordinated the groups in the Family Health teams. The results showed that, actually, there are still theoretical and methodological limits for such coordination as well as lack of knowledge to handling situations in which coordinators face difficulties in dealing with human feelings and interactions that emerge from the group. The teams' health professionals' turnover also shows those limits, making it difficult to establish links and accomplish health care. The unregulated demand compromises daily activities of the group, another problem regarding this kind of work. Informants pointed out the need of theory training as a means of improving group coordination, a possibility to respond to the challenges of the demands. It also shows the possibility that informants look for external help in order to keep on with collective educational activities. The search for partnerships has also turned into a cultural activity.

The search for partnerships has also turned into a cultural activity.El estudio cualitativo en base a la Teoría de la Diversidad y Universalidad del Cuidado Transcultural de Madeleine Leininger fue llevado a cabo en ocho unidades básicas de salud de un distrito sanitario de Belo Horizonte, Minas Gerais. El objetivo de la investigación fue discutir las limitaciones y posibilidades para desarrollar trabajos con grupos de diabéticos e hipertensos en la Estrategia de Salud de la Familia. Participaron diecinueve profesionales coordinadores de grupos de los equipos de Salud de la Familia. Los resultados mostraron que en la práctica siguen existiendo limitaciones teórico-metodológicas para la coordinación del grupo. Se constató la falta de preparación de los profesionales para manejar situaciones donde afloran sentimientos y el grupo interactúa. La rotación de estos profesionales de salud también impone limitaciones al comprometer el vínculo y la efectividad del cuidado. La presión de la demanda compromete la programación de actividades grupales y se transforma en un problema para efectivizar este tipo de trabajo. Los informantes expresaron necesidad de capacitación en teorías de grupo para mejorar la coordinación del trabajo, lo cual configura una posibilidad para responder al desafío de la demanda. Quedó en evidencia la posibilidad de que los informantes buscasen refuerzos externos para realizar actividades educativas colectivas y también que la búsqueda de socios para desarrollar trabajos con grupos ya es algo cultural.Trata-se de uma pesquisa qualitativa, fundamentada na Teoria da Diversidade e Universalidade do Cuidado Transcultural, de Madeleine Leininger, realizada em oito Unidades Básicas de Saúde de um Distrito Sanitário de Belo Horizonte-MG. O objetivo com o estudo foi discutir as limitações e as possibilidades no desenvolvimento do trabalho com grupos de diabéticos e hipertensos na Estratégia de Saúde da Família. Participaram da pesquisa dezenove profissionais que atuam como coordenadores de grupos nas equipes de Saúde da Família (SF). Os resultados mostraram que ainda persistem, na prática, limitações teórico-metodológicas para a coordenação do grupo. Constatou-se, também, o despreparo dos profissionais para o manejo de situações em que o coordenador se defronta com dificuldades para lidar com os sentimentos humanos e com as interações que afloram no grupo. A rotatividade dos profissionais de saúde das equipes é outro aspecto que traduz limitações, comprometendo a formação do vínculo e a efetivação do cuidado. A pressão da demanda compromete as agendas de atividades grupais, tornando-se, assim, problema para a efetivação desse tipo de trabalho. Os informantes explicitaram a necessidade de capacitação em teorias de grupo para melhorar a coordenação do trabalho, o que se configura como uma possibilidade para responder ao desafio da demanda.
