Resumo: The Previne Brasil Program comes as a proposal to modify the financing of the health system and brings great challenges to the public service, since it conditions the transfer of funds to the maximum reach of certain health indicators. It is based on this new reality that affects Primary Health Care, that this study brings a strategic possibility of action so that the teams can articulate and seek ways to achieve the goals established by the Federal Government. This is an experience report, lived by residents during the elaboration and implementation of the Project entitled Qualifica HiperDia. It aims to report the construction and implementation of a line of care for people with chronic conditions, with particular emphasis on Systemic Arterial Hypertension and Diabetes Mellitus. The project emerges as an alternative to reduce the difficulties in the team's work process in tracking, monitoring and monitoring the health needs of the community related to Chronic non-Communicable Diseases.

Es a partir de esta nueva realidad que afecta a la Atención Primaria de Salud, que este estudio trae una posibilidad estratégica de acción para que los equipos puedan articularse y buscar caminos para alcanzar las metas establecidas por el Gobierno Federal. Este es un relato de experiencia, vivido por los vecinos durante la elaboración e implementación del Proyecto Califica HiperDia. Tiene como objetivo informar la construcción e implementación de una línea de atención as personas con condiciones crónicas, con especial énfasis en Hipertensión Arterial Sistémica y Diabetes Mellitus. El proyecto surge como una alternativa para reducir las dificultades en el proceso de trabajo del equipo en el seguimiento, acompañamiento y seguimiento de las necesidades de salud de la comunidad relacionadas con las Enfermedades Crónicas no Transmisibles.O Programa Previne Brasil vem como uma proposta de modificação do financiamento do sistema de saúde e traz grandes desafios ao serviço público, uma vez que condiciona o repasse de verbas ao alcance máximo de determinados indicadores de saúde. É com base nessa nova realidade que atinge a Atenção Primária a Saúde, que esse estudo traz uma possibilidade estratégica de atuação para que as equipes possam se articular e buscar meios de alcance das metas estabelecidas pelo Governo Federal. Trata-se de um relato de experiência, vivenciado por residentes durante a elaboração e implementação do Projeto intitulado Qualifica HiperDia. Tem como objetivo relatar a construção e implementação de uma linha de cuidado a pessoas com condições crônicas, priorizando-se especialmente, a Hipertensão Arterial Sistêmica e o Diabetes Mellitus.
