The nurse in the practice of peripherally inserted central catheter in neonatal care

Resumo: In order to improve health care quality provided to premature newborns, the use of peripherally inserted central catheters (PICC) has become more common. This study aims to describe some variables related to the insertion, to the maintenance and to the removal of PICC in neonates who are hospitalized in an Intensive Care Unit. It is a descriptive study with a longitudinal design. Prospective data collection was held in a private institute in upstate São Paulo. Data were obtained through observation and analysis of patients' charts between August and September 2008. The study was previously approved by the Ethics Research Committee. Fourteen newborns who met the criteria for inclusion were evaluated. The most prevalent gestational age was 30 to 34 weeks with weight over 1500 g. Most patients were female (57.2%), and the main indication for catheter insertion was partial parenteral nutrition. The catheter used was silicone 1.9Fr; the average frequency of puncture was 3.1 and in 57.2% of the cases the first punch was successful. The most commonly punctured vein was the basilica (71.2% of the cases). The main reasons for failure were bruises, vascular weakness and difficulty of progression. The mean time of catheter permanence was 10.5 days, and the main indication for removal was end of intravenous therapy (71.4% of the cases). Culture of the catheter tip was required for all catheters with positive result for Staphylococcus aureus.

El objetivo del presente estudio fue de describir algunas de las variables relacionadas con los procedimientos de integración, mantenimiento y retirada del catéter central en recién nacidos internados en la Unidad de Cuidados Intensivos. Se trata de un estudio descriptivo con diseño prospectivo longitudinal realizado en una institución privada del interior de São Paulo. Los datos fueron obtenidos mediante observación y anotados en prontuarios. Antes de comenzar la recogida de datos, realizada entre agosto y septiembre de 2008, el proyecto fue aprobado por el Comité de Ética en Investigación. Los participantes del estudio fueron catorce recién nacidos que atendían a los criterios de inclusión, con edad gestacional predominante de 30 a 34 semanas y peso superior a 1,500 kg. La mayoría (57,2%) era del sexo femenino y la principal indicación para inserción del catéter fue nutrición parenteral parcial. El catéter utilizado era de silicona 1.9Fr, con frecuencia media de punción de 3,1 y primera punción exitosa en 57,2%. La vena preferencial para la punción fue la basílica (71,2%) y las principales razones para los fracasos fueron hematomas, debilidad vascular y dificultad de progresión. El tiempo medio de permanencia del catéter fue de 10,5 días y la principal indicación para su retirada fue término de la terapia intravenosa (71,4%). Fue solicitado cultivo de la punta del catéter para todos los catéteres de los cuales se obtuvo resultado positivo para Estafilococos aureus.Na tentativa de melhorar a qualidade na assistência prestada ao recém-nascido prematuro, tem-se utilizado o cateter central de inserção periférica (PICC). O objetivo com este estudo foi descrever algumas variáveis relacionadas ao procedimento de inserção, manutenção e remoção do cateter central em neonatos internados na Unidade Terapia Intensiva (UTI). Trata-se de um estudo descritivo com delineamento longitudinal e coleta prospectiva realizado em instituição privada no interior de São Paulo. Os dados foram obtidos mediante a observação e coleta nos prontuários. A coleta foi realizada no período de agosto a setembro de 2008. Antes de iniciar a coleta de dados, o projeto foi apreciado e aprovado pelo Comitê de Ética em Pesquisa. Participaram do estudo 14 recém-nascidos que atendiam aos critérios de inclusão. A idade gestacional predominante foi de 30 a 34 semanas e peso maior que 1.500 g. A maioria (57,2%) era do sexo feminino, sendo que a principal indicação da inserção do cateter foi nutrição parenteral parcial. O cateter utilizado foi de silicone 1.9 French, a média de frequência de punção foi de 3,1 com sucesso na primeira punção de 57,2%. A veia preferencial para punção foi a basílica; 71,2% e os principais motivos de insucessos foram hematomas, fragilidade vascular e dificuldade de progressão. O tempo médio de permanência do cateter foi de 10,5 dias e a principal indicação de remoção foi término da terapia intravenosa, 71,4%.
