Main reasons for removal of the peripherally inserted central venous catheter (PICC) in the neonatal ICU of a military hospital in Rio de Janeiro

Resumo: Introduction: The use of the peripherally inserted central venous catheter (CCIP) in the Neonatal Intensive Care Unit (ICU) has been of great importance, due to the necessary procedures to be performed due to prematurity, avoiding the need for frequent venous punctures and reducing the exposure of the newborn to pain. However, its use is not free from complications. Objective: To identify the main causes for removal of the PICC catheter in the Neonatal ICU of a military hospital located in the city of Rio de Janeiro. Methods: This is a cross-sectional, descriptive and analytical study with data collection through the analysis of medical records of newborns who underwent PICC catheter insertion in the Neonatal ICU of Hospital Naval Marcílio Dias (HNMD) between June 2018 and June 2021. They were included in the study preterm newborns (PTNB) with a gestational age (GA) of less than 37 weeks and/or with a birth weight of less than 2000g. Preterm infants with PICC permanence for less than 5 days due to treatment suspension or death were excluded. Results: Detailed data analysis revealed an unexpectedly high rate of phlebitis occurrences (9.8%), even when the majority of indications for catheter removal were related to the end of treatment (78.4%). Conclusion: This scenario emphasizes the urgency of implementing solid care indicators, comprehensive checklists/bundles and standard operating procedures (SOP) to improve the standardization of care practices, aiming to improve the safety and quality of care for neonatal patients.

Objetivo: Identificar las principales causas de la eliminación de catéter PICC en la UTI Neonatal de un hospital militar en la ciudad de Río de Janeiro. Métodos: Estudio transversal, descriptivo y analítico con recolección de datos através del análisis de las historias clínicas de los neonatos sometidos a la inserción del catéter PICC en la UCI Neonatal del Hospital Naval Marcílio Dias (HNMD) entre junio de 2018 y junio de 2021. Se incluyeron en el estudio los recién nacidos prematuros (RNPT) con edad gestacional (AG) inferior a 37 semanas. Se excluyeron los recién nacidos con permanencia del PICC por menos de 5 días o muerte. Results: El análisis detallado de los datos reveló una tasa inesperadamente alta de ocurrencias de flebitis (9,8%), incluso cuando la mayoría de las indicaciones para la retirada del catéter estaban relacionadas con el final del tratamiento (78,4%). Conclusión: Este escenario enfatiza la urgencia de implementar indicadores de atención, listas de verificación / paquetes integrales y procedimientos operativos para mejorar la estandarización de las prácticas de atención, con el objetivo de mejorar la calidad de atención a pacientes neonatales.Introdução: A utilização do cateter venoso central de inserção periférica (PICC) na Unidade de Terapia Intensiva Neonatal (UTIN) tem sido de grande importância, em virtude dos procedimentos necessários a serem realizados em decorrência da prematuridade, evitando a necessidade de punções venosas frequentes e reduzindo a exposição do recém-nascido à dor. No entanto, seu uso não está livre de complicações. Objetivo: Identificar as principais causas para remoção do cateter PICC na UTI Neonatal de um hospital militar localizado no município do Rio de Janeiro. Métodos: Estudo transversal, descritivo e analítico com coleta dos dados por meio da análise de prontuários de neonatos submetidos à inserção do cateter PICC na UTI Neonatal do Hospital Naval Marcílio Dias (HNMD) entre junho de 2018 e junho de 2021. Foram incluídos no estudo recém-nascidos pré-termos (RNPT) com idade gestacional (IG) inferior a 37 semanas e/ou com peso de nascimento inferior a 2000g. Foram excluídos os RNPT com permanência de PICC por período inferior a 5 dias por motivo de suspensão de tratamento ou óbito. Resultados: A análise detalhada dos dados revelou uma taxa inesperadamente elevada de ocorrências de flebite (9,8%), mesmo quando a maioria das indicações para a remoção do cateter estava relacionada ao término do tratamento (78,4%).
