One to look at of the adolescents on the changes in the pregnancy: promoting the mental health in the basic attention

Resumo: This is a qualitative research based on a convergent-care methodology with ten pregnant teenagers' patients at a Primary Healthcare Unit. It seeks to identify the teenagers' perception on social, physical and emotional changes during pregnancy, how to face them, and the support received. The data was collected from August to September 2009 through semi-structured interview and participant observation until data saturation. The data was interpreted by content analysis that resulted in the following categories: facing the new, social, pregnancy physical and emotional changes, and support network. The results indicated the teenagers appeared unprepared and immature to carry the gestation on. The pregnancy led to alterations such as body changes, concerns about the image, childbirth and the baby, more responsibility, sadness, joy, stress, fear, detachment from family and friends, changes in lifestyle and drop out of school. It trigged personal, family and partner reactions such as elaboration, acceptance, and denial, rejection, culminating in abortion and suicide attempts. The support network proved to be essential for it focused mainly on the mother and less on the health professionals. This study gave adolescents an opportunity to express their experiences during pregnancy. It reinforced the need of an interdisciplinary, integrated and customized health care service to the pregnant teenager as well as the support network and mental health promotion actions. It contributed as well to the production of knowledge on the subject and it can subsidize health care changes on primary care practices.

Tiene el propósito de conocer la opinión de las adolescentes acerca de los cambios sociales, físicos y emocionales durante el embarazo, comportamientos para enfrentarlos y ayuda recibida. La recogida de datos ocurrió en agosto y septiembre de 2009, por medio de entrevista semiestructurada y observación participante hasta la saturación de los datos que fueron interpretados por el análisis de contenido y originaron las siguientes categorías: Confrontación con lo nuevo, cambios sociales, físicos y emocionales ante el embarazo y red de ayuda. Los resultados indican falta de preparación e inmadurez de las adolescentes para asumir el embarazo. El embarazo provoca alteraciones corporales, preocupación con la imagen , partoy el bebé, más responsabilidad, tristeza, alegría, estrés, miedo, alejamiento de la familia y de los amigos, cambios en el estilo de vida y abandono escolar. Causa reacciones personales, familiares y del compañero tales como elaboración, aceptación, negación, rechazo, culminando con la tentativa de aborto y suicidio. La red de ayuda mostró ser fundamental, centrándose básicamente en la madre y poco en los profesionales de salud. Este estudio les ofreció a las adolescentes la oportunidad de expresar sus vivencias durante el embarazo. Refuerza la necesidad de brindar atención interdisciplinaria, integral y personalizada a la adolescente embarazada, fortalecer la red de apoyo y de acciones de promoción de la salud mental además de contribuir a la producción de conocimiento y servir de subsidio para transformar las prácticas de salud en la atención básica.Pesquisa qualitativa na modalidade convergente assistencial com dez adolescentes grávidas, usuárias de uma Unidade Básica de Saúde. A finalidade foi conhecer a percepção das adolescentes sobre as mudanças sociais, físicas e emocionais na gravidez, condutas para enfrentá-las e o apoio recebido. A coleta de dados deu-se em agosto e setembro de 2009, por meio de entrevista semiestruturada e observação participante até a saturação de dados. Os dados interpretados pela análise de conteúdo deram origem às categorias: confronto com o novo, mudanças sociais, físicas e emocionais diante da gravidez e rede de apoio. Os resultados indicam despreparo e imaturidade das adolescentes para assumir a gestação. A gravidez gerou mudanças tais como: alterações corporais, preocupações com a imagem, o parto e o bebê, maior responsabilidade, tristeza, alegria, estresse, medo, distanciamento da família e amigos, alterações do estilo de vida e abandono escolar. Causou reações pessoais, familiares e do companheiro como: elaboração, aceitação, negação, rejeição, culminando com a tentativa de aborto e suicídio. A rede de apoio mostrou-se fundamental, centrando-se mais na mãe e pouco nos profissionais de saúde. Este estudo dá voz e vez às adolescentes para expressarem suas vivências durante a gravidez.
