Pressure ulcers in neonates and children: epidemiological and clinical profile

Resumo: | The objective of this study was to evaluate the pressure ulcer incidence in neonates and children at the Hospital das Clinicas at Federal University of Parana (HC/UFPR), to characterize the sociodemographic profile of patients with pressure ulcers in an established period of time, and perform a clinical evaluation of pressure ulcers and identify its degree of seriousness. This was a transversal study developed from April to May of 2009 in the HC/UFPR pediatric units. Each neonate and child was chosen by the responsible registered nurse of each pediatric unit. A specific instrument was utilized under sociodemographic characteristics, anatomical location and clinical evaluation of pressure ulcers. It was utilized the SPSS Statistics program, 17.0 version for statistical analysis. Five neonates/children were identified with pressure ulcers, with an 8, 06% prevalence rate. The average age was 2, 25 years (SD=3, 02), the average time in hospital was 21, 4 days (SD=19, 08), varying from 5 to 50 days. The hospitalization causes were acute respiratory insufficiency associated with cardiopathy (1), bronchopneumonia (1), late postoperative with central venous catheterrelated infection (1) and myelomeningoceles (2). Regarding the severity of the pressure ulcers, two were First-Degree, one Second-Degree, one Third-Degree and one unidentified Degree, located at occipital region (20%), temporal (20%), nasal (20%), dorsal (20%) and thumb (20%). It has been observed that a significant prevalence of pressure ulcers exists in pediatric patients. Prevention strategies and further new incidence and prevalence studies in this age group might decrease these numbers |

MÉTODOS: estudio cruzado desarrollado en el periodo de abril a mayo de 2009 en las unidades de internación pediátrica del HC/UFPR. La indicación de los neonatos y niños evaluados fue realizada por el enfermero responsable de cada unidad pediátrica. Se utilizó un instrumento específico bajo las características sociodemográficas, localización anatómica y evaluación clínica de la úlcera. Para el análisis estadístico se empleó el programa SPSS Statistics, versión 17.0. RESULTADOS: Fueron identificados cinco neonatos/niños portadores de úlcera por presión, con prevalencia de 8,06%. El promedio de edad fue de 2.25 años (DP=3,02), tiempo promedio de hospitalización fue de 21,4 días (DP=19,08), variación de cinco a 50 días. Causas de hospitalización: insuficiencia respiratoria aguda asociada a cardiopatía (1) y bronconeumonía (1), pos operatorio tardía con infección en acceso venoso central (1) y Mielomeningocele (2). Con relación a la gravedad, dos úlceras eran de Grado I, una de Grado II, una de Grado III y una de Grado indefinido, localizadas en la región occipital (20%), temporal (20%), nasal (20%), dorsal (20%) y pulgar (20%). CONCLUSIÓN: Se observa prevalencia significativa de úlcera por presión en neonatos y niños. Estrategias de prevención y la realización de nuevos estudios de incidencia y prevalencia en esa edad pueden disminuir esos númerosOs objetivos com este trabalho foram avaliar a prevalência de úlcera por pressão em neonatos e crianças no Hospital de Clínicas da Universidade Federal do Paraná (HC/UFPR), caracterizar o perfil sociodemográfico dos portadores de úlceras por pressão no período estabelecido, realizar a avaliação clínica das úlceras por pressão e identificar sua gravidade. Este é um estudo transversal desenvolvido no período de abril a maio de 2009 nas unidades de internação pediátrica do HC/UFPR. A indicação dos neonatos e crianças avaliadas foi realizada pelo enfermeiro responsável de cada unidade pediátrica. Utilizou-se um instrumento específico sobre as características sociodemográficas, localização anatômica e avaliação clínica da úlcera. Observou-se a prevalência significativa de úlcera por pressão em neonatos e crianças.
