Evaluation of cytopathology findings in women attended at a gynecology nursing service

Resumo: The cytopathology test is used for screening precursory lesions of cervical cancer. A professional nurse collects cytological material and is, therefore, responsible for the quality of the sample and for any collecting factors that can alter the results. The objective of this study is to evaluate cytopathology findings in women attended at gynecology service. It was carried out through a retrospective descriptive documental methodology with a quantitative focus. One hundred and ninety one charts were analyzed according to a structured formulary with objective questions. Data was organized in tables and graphics. Most women (44%) were older than 30 years; 51.6% underwent screening test regularly, 44.4% started having sex at the age of 16-19 years; 20.1% used no contraceptive method and the same percentage had an oral contraceptive method. The most common gynecologic complain was about having a white secretion. Most part of the exams (72.2%) was considered efficient. The diagnosis of squamous metaplasia with inflammation was relevant and Gardnerella vaginalis was the most prevalent etiological agent (28.3%). Cytological changes of unknown significance were found in 3% of the exams. We conclude that the sexual profile described should be considered alarming since there are risk factors for precursory lesions of cervical cancer. The quality of the sample was considered satisfactory and showed to be an extremely important factor for a successful diagnosis and an adequate treatment. Moreover, the screening test has a strategic value to diminish morbid mortality of cervical cancer.

El enfermero, a través de una consulta de enfermería, recoge el material citológico por lo cual se vuelve responsable de la calidad del material y de los factores que influyen en su diagnóstico. El objetivo de este trabajo fue evaluar el citodiagnóstico de mujeres atendidas en el servicio de Ginecología del Centro de Parto Natural. Se trata de una investigación descriptiva documentaria retrospectiva desde el enfoque cuantitativo. La muestra se constituye de 194 prontuarios de mujeres que contestaron formularios estructurados con preguntas objetivas. Los datos se organizaron en tablas y gráficos. La mayoría (44,4%) tenía más de 30 años; 51,6% realizaban examen preventivo con frecuencia; 44,9% iniciaron la vida sexual entre los 16 y los 19 años; 20,1% no usaba ningún método anticonceptivo y el porcentaje equivalente anticonceptivos orales La queja más frecuente fue secreción blanca. El equivalente al 72,2% de los estudios fue satisfactorio; se destacó el diagnóstico metaplasia escamosa con inflamación y el agente que más prevaleció fue Gardnerella Vaginalis (28,3%). Las alteraciones celulares con significado indeterminado representaron el 3 % de los estudios. Se concluyó que el perfil sexual trazado era preocupante por la presencia de factores de riesgo para que surgieran lesiones precursoras del cáncer. La adecuación de la muestra del material analizado, considerada satisfactoria en este estudio, es de suma importancia para que el diagnóstico y el tratamiento sean exitosos y sirve como estrategia para disminuir las estadísticas de morbimortalidad del cáncer cervical.O exame citopatológico é usado como método de rastreamento para detectar lesões precursoras do câncer de colo uterino. Realizou-se pesquisa descritiva, documental retrospectiva, com abordagem quantitativa. A amostra foi composta de 194 prontuários, à qual se aplicou formulário estruturado contendo questões fechadas. Os dados foram organizados em tabelas e gráficos. A maioria das mulheres (44,4%) tinha idade superior a 30 anos; 51,6% realizavam o exame preventivo freqüentemente; 44,9% iniciaram a vida sexual entre 16-19 anos; 20,1% não usavam método contraceptivo; e a porcentagem equivalente usava como método o contraceptivo oral. A queixa ginecológica mais referida foi o corrimento branco. A maioria dos exames realizados foi considerada satisfatória (72,2%), o diagnóstico metaplasia escamosa com inflamação se sobrepôs e Gardnerella vaginalis (28,3%) foi o agente etiológico mais prevalente. As alterações celulares de significado indeterminado representaram 3,0% dos exames. Concluiu-se que o perfil sexual traçado foi considerado preocupante dada a presença dos fatores de risco para o surgimento de lesões precursoras para o câncer.
