Between sick life and healthy death: orthotanasia in the experience of nurses in an intensive care unit

Resumo: | Considering that passive euthanasia is a controversial term with insufficient ethical and legal debates, a research was carried out aiming to know the perception of professional nurses about such practice in the hospital context. It is an exploratory research, based on a qualitative approach, developed in an adult ICU of a public hospital in the state of Rio Grande do Sul, through interviews with five nurses. Semi-directed interviews were conducted from January to March 2008. Ethical aspects involved in the research were appropriately observed. After content analysis, the following categories emerged: the doctor decides; patient is not worth investing the effort; the family participation; death with dignity; and hidden passive euthanasia. Dealing with such a theme as passive euthanasia is an attempt to contextualize its existence and its practice, considering a routine that tries to hide or to mask it, since there is no legal or medical support to its practice. Even though passive euthanasia is considered a new subject, it is, in fact, an ancient practice which takes place in the hospital environment. According to the professionals who were interviewed, it is a viable possibility to treat death as a constant and irremediable process opposed to the extension of human suffering. Professionals involved on ICU assistance should be encouraged to participate in discussions on themes related to death, since talking about it helps to gradually de-stigmatize it and face it in a new way. |

Se trata de una investigación exploratoria con enfoque cualitativo realizada en un Centro de Tratamiento Intensivo (CTI) adulto de un hospital público del Estado de Rio Grande del Sur, habiendo sido entrevistadas cinco enfermeras. Se optó por entrevistas semiestructuradas realizadas de enero a marzo de 2008. Los aspectos éticos involucrados en la investigación fueron debidamente respetados. Después de realizar el análisis de contenido emergieron las siguientes categorías: el médico es quien decide; el paciente no merece el esfuerzo invertido en él; la participación de los familiares; el morir con dignidad; y la ortotanasia escondida. Así, tratar del asunto ortatanasia es un intento de contextualizar su existencia y actuación frente a una rutina que trata de ocultarla y mascararla, justamente por el hecho de no existir amparos para su práctica, tanto de la jurisdicción legal como de la propia medicina. La llamada nueva ortatanasia es, en realidad, una antigua práctica instalada en el ambiente hospitalario y que, de acuerdo con los profesionales entrevistados, es una posibilidad viable de tratar la muerte como un proceso irremediable y constante, de forma contraponerse al prolongamiento del sufrimiento humano. Los profesionales involucrados en la asistencia en CTI deberían ser estimulados a participar de espacios que les permitan debatir asuntos relacionados a la muerte puesto que hablando sobre ella se la puede, poco a poco, desestigmatizar y comenzar a encarar de otra forma.Considerando que a ortotanásia diz respeito a um termo por vezes controverso e com insuficientes discussões éticas e jurídicas, realizou-se uma pesquisa objetivando conhecer a percepção de profissionais enfermeiros sobre essa prática no contexto hospitalar. Trata-se de uma pesquisa de caráter exploratório, pautada na abordagem qualitativa, desenvolvido em uma UTI adulto de um Hospital público do Rio Grande do Sul, tendo sido entrevistadas cinco enfermeiras. Optouse pela entrevista semidirigida, realizada de janeiro a março de 2008. Os cuidados relacionados aos aspectos éticos envolvidos na pesquisa foram devidamente cumpridos. Após a análise de conteúdo, emergiram as categorias: é o médico quem decide; o paciente não é investível; a participação dos familiares; o morrer com dignidade; e a ortotanásia oculta. Tratar do assunto da ortotanásia é uma tentativa de contextualizar sua existência e atuação diante de uma rotina que tenta escondê-la ou mascará-la, justamente pelo fato de não existir amparos para sua prática, tanto da jurisdição legal como da própria medicina. A ortotanásia, dita nova, é, na verdade, uma antiga prática instalada no ambiente hospitalar e que, de acordo com os profissionais entrevistados, é uma possibilidade viável de tratar a morte como um processo irremediável e constante, de forma a contrapor-se ao prolongamento do sofrimento humano.
