Association of haemostatic and inflammatory biomarkers with nephropathy in type 1 diabetes mellitus

Resumo: This study aimed at investigating the association between haemostatic biomarkers, proinflammatory, and anti-inflammatory cytokines with chronic kidney disease in type 1 diabetic patients. Patients were divided into two groups: with nephropathy (albuminuria ≥ 30 mg/g and/or GFR < 60 mL/min/1.73 m2 ), = 65; and without nephropathy (albuminuria < 30 mg/g and GFR ≥ 60 mL/min/1.73 m2 ), = 60. INF-, IL-6, IL-10, and TNF- plasma levels were determined by flow cytometry. VWF, ADAMTS13 antigen, and D-Dimer plasma levels were determined by enzyme-linked immunosorbent assay and ADAMTS13 activity was assessed by fluorescence resonance energy transfer assay. Elevated levels of INF-, VWF, ADAMTS13 antigen, D-Dimer, and reduced ADAMTS13 activity/antigen ratio were observed in patients with nephropathy as compared to those without nephropathy ( = 0.001, < 0.001, < 0.001, < 0.001, and < 0.001, resp.). Cytokines and haemostatic biomarkers remained associated with nephropathy after adjustments (use of statin, acetylsalicylic acid, angiotensin converting enzyme inhibitor, and angiotensin antagonist). INF-, TNF-, and IL-10 significantly correlated with haemostatic biomarkers. Inflammatory and hypercoagulability status are associated with nephropathy in type 1 diabetes mellitus and an interrelationship between them may play an important role in pathogenesis of diabetic nephropathy.

Os pacientes foram divididos em dois grupos: com nefropatia (albuminúria ≥ 30 mg/ge/ou TFG < 60 mL/min/1,73 m2), = 65; e sem nefropatia (albuminúria < 30 mg/g e TFG ≥60 mL/min/1,73 m2), = 60. Os níveis plasmáticos de INF-, IL-6, IL-10 e TNF- foram determinados por citometria de fluxo. VWF, ADAMTS13 antígeno e os níveis plasmáticos de D-Dímero foram determinados por ensaio imunossorvente ligado a enzima e a atividade de ADAMTS13 foi avaliada por ensaio de transferência de energia de ressonância de fluorescência. Níveis elevados de INF-, VWF, antígeno ADAMTS13, D-Dimer e redução da relação atividade/antígeno de ADAMTS13 foi observada em pacientes com nefropatia em comparação com aqueles sem nefropatia ( = 0,001, < 0,001, < 0,001, < 0,001 e < 0,001, respectivamente). Citocinas e biomarcadores hemostáticos permaneceram associados com nefropatia após ajustes (uso de estatina, ácido acetilsalicílico, inibidor da enzima conversora de angiotensina e antagonista). INF-, TNF- e IL-10 correlacionaram-se significativamente com biomarcadores hemostáticos.
