Estima-se que 50% dos casos de insuficiência hepática aguda são devidos ao abuso de acetaminofeno (APAP). Em doses mais elevadas, APAP causa necrose de hepatócitos e liberação de moléculas intracelulares que - uma vez no ambiente extracelular - desencadeiam uma potente resposta inflamatória. Diferentes moléculas celulares já foram descritas como padrões moleculares associados ao dano (DAMPs) que são relevantes no contexto da inflamação estéril, incluindo ATP, proteínas high motility group box (HMGB)-1, DNA, histonas e também IL-33. A IL-33 sinaliza através de um único receptor (ST2 ou IL1RL1) e a participação desta via nas doenças hepáticas ainda são controversas na literatura. Aqui, demonstramos que a sinalização de IL-33 via ST2 é fundamental no agravamento da inflamação do fígado desencadeada por necrose. Os camundongos que não possuíam o receptor ST2 eram resistentes aos efeitos tóxicos de APAP e o transplante de medula óssea de progenitores hematopoiéticos deficientes em ST2 reverteu a falência hepática em camundongos de tipo selvagem. Os neutrófilos são os principais sensores para IL-33 extracelular, sugerindo uma possível estratégia para futuras abordagens terapêuticas destinadas a abrandar a sinalização de IL-33 em células imunes durante lesão estéril do fígadoLiver injury is a major health concern and one of the most frequent causes of hepatic failure is the abusive and unsupervised intake of drugs. It is estimated that 50% of the cases of acute liver failure is due to acetaminophen (APAP) abuse. At higher doses, APAP causes hepatocyte necrosis and release of intracellular molecules that – once in the extracellular environment – will trigger a robust inflammatory response. Different cell molecules were already described as damage associated molecular patterns (DAMPs) that are relevant in the context of sterile inflammation, including ATP, high motility group box (HMGB)-1, DNA, histones and also IL-33. IL-33 signals via a single receptor (ST2, or IL1RL1) and the participation of this pathway in liver diseases is still controversial in the literature. Here we demonstrated that IL- 33 signalling via ST2 is central in aggravation of necrosis-triggered liver inflammation. Mice lacking ST2 receptor were resistant to APAP toxic effects, and bone marrow transplantation of ST2-deficent hematopoietic progenitors rescued hepatic failure in wild type mice.
