Utilização de serviços de saúde por imigrantes haitianos na grande Cuiabá, Mato Grosso

Resumo: The immigration phenomenon has highlighted the fragility of the structure and the country's lack of preparation in health care for immigrants and also the lack of public policies for this population. There are few studies that evaluate the health status of this population group, as has being a recent phenomenon. Therefore, there is the need for the profile analysis of the health services use which, in turn, would contribute to the planning of actions the health needs efficiently, respecting the uniqueness of this people. This study aims to analyze the use of health services by resident Haitian immigrants in Cuiabá, Mato Grosso, Brazil. A cross-sectional exploratory study was developed, with a probabilistic sample of 452 Haitian immigrants residents in Cuiaba and Várzea Grande. Haitians answered a questionnaire with demographic, health and use of health services information. The following variables were analyzed: age (measured in years and classified into age groups); individual income (in reais, for the last month); education (in years); languages you speak/understand; How long lived in Brazil; If you ever needed to use any health service (yes/no); and what types of services and their nature (public/private). Data collection took place between December 2014 and February 2015 and was approved by the University Hospital of the Ethics Committee Julius Muller UFMT. The SPSS software was used for data analysis. Four hundred fifty-two Haitians was interviewed (82.5% male). Approximately half of the respondents earned between one and two minimum wages (46.0%) and a significant proportion reported having not got no income (30.1%). The highest percentage of women who reported having used some health service in Brazil when compared to men (65.8% vs 41.3, p <0.01). In addition, there was a greater prevalence of use of health services among those with higher income, with better understanding of the Portuguese, who dominated Haitian Creole and two or more languages and living more time in Brazil (p <0.05 for all variables). Regarding the types of services and their nature, 18.6% used public emergency care service; 21.5% used Basic Unit of Health Services (UBS) Public; only 1.2% said they had used private emergency care. It was noted that there are inequalities between genders regarding the use of health services, higher demand among women. Thus, there is a need for actions aimed at male emphasizing the importance of prevention of diseases and disorders. Some theorists suggest that individuals with better understanding of Portuguese, most use the health services, however, we must consider that the language domain is directly associated with the residence time in the host country, was found in this study that the Portuguese domain It was not associated with the use of health services, after adjusting for the residence time in Brazil. Thus, there is the need for empowerment of migrants on their rights and also on how it is organized health services network in Brazil. Furthermore, continuing education activities for health professionals on health care in relation to immigrant and develop other forms of communication beyond verbal language can assist in better access of this population group to health services.

Por ser um fenômeno recente, ainda são poucos os estudos que avaliam a situação de saúde deste grupo populacional. Portanto, destaca-se a necessidade da análise do perfil de utilização dos serviços de saúde que, por sua vez, contribuirá para o planejamento de ações voltadas as necessidades de saúde de forma resolutiva, respeitando as singularidades deste povo. Este estudo tem como objetivo analisar a utilização de serviços de saúde por imigrantes haitianos residentes na Grande Cuiabá, Mato Grosso. Desenvolveu-se um estudo exploratório, de delineamento transversal com uma amostra probabilística de 452 imigrantes haitianos residentes na Grande Cuiabá. Os haitianos responderam a um questionário com informações sociodemográficas, condições de saúde e utilização de serviços de saúde. Para o seguinte trabalho, as seguintes variáveis foram analisadas: idade (avaliada em anos completos e classificadas em faixas etárias); renda individual (em reais, referente ao último mês); escolaridade (em anos completos com aprovação); idiomas que fala/compreende; há quanto tempo reside no Brasil; se já precisou utilizar algum serviço de saúde (sim/não); e quais os tipos de serviços e sua natureza (público/privado). A coleta de dados ocorreu entre dezembro de 2014 e fevereiro de 2015 e foi aprovada pelo Comitê de Ética do Hospital Universitário Júlio Muller da UFMT. O software SPSS foi utilizado para a análise dos dados. Foram entrevistados 452 haitianos (82,5% do sexo masculino). Aproximadamente metade dos entrevistados tinham entre um e dois salários mínimos (46,0%) e uma importante proporção relataram não ter obtido nenhuma renda (30,1%). Foi maior a proporção de mulheres que relataram ter utilizado algum serviço de saúde no Brasil quando comparados aos homens (65,8% vs 41,3, p<0,01). Além disso, foi maior a prevalência de utilização de serviço de saúde entre aqueles com maior renda, com melhor entendimento do português, que dominavam Crioulo haitiano e outros dois ou mais idiomas e com maior tempo no Brasil (p<0,05 para todas as variáveis). Em relação aos tipos de serviços e a sua natureza, 18,6% utilizaram serviço de atendimento de emergência público; 21,5% utilizaram serviços de Unidade Básica de Saúde (UBS) pública; apenas 1,2% disse ter utilizado atendimento de emergência privada. Notou-se que há desigualdades entre gêneros quanto a utilização dos serviços de saúde, sendo maior a procura entre as mulheres. Desta forma, destaca-se a necessidade de ações voltadas para o sexo masculino ressaltando a importância da prevenção de doenças e agravos. É importante ressaltar que o domínio do idioma está diretamente associado ao tempo de residência no país de acolhida, sendo verificado neste estudo que o domínio do português não se manteve associado à utilização dos serviços de saúde, após ajuste para o tempo de residência no Brasil. Destaca-se ainda, a necessidade de empoderamento dos imigrantes sobre os seus direitos e também sobre como está organizada a rede de serviços de saúde no Brasil.
