Narrativa como mediadora de vivências infantis no contexto hospitalar : as representações sociais sobre o cuidado em uma enfermaria pediátrica, segundo equipe de saúde e as significações infantis

Resumo: This theoretical work is based on the idea that the children elaborated knowledge about hospital occurs in the inter-subjective context. It considers that when they are inserted in this context they are faced with a pre-structuring of the social environment that organizes the interactions in which the children take part. It understands that the promotion of interaction with narratives that introduce meanings about this scenario can foment the meaning process of the children on the hospital care, helping them to design their role in relation to these practices. It is a study that is based in the Social Representations Theory, elaborated by Moscovici (1978, 2012) listing some possible points of articulating with the Cultural-historical perspective of human development proposed by Vigotski (2010). Among the Bruner’s studies (2001, 2002, 2008, 2014) on narratives, it seeks the understand about the imbrication of this type of knowledge with the interaction contexts that people experience. It exhibits a survey of cultural meanings that permeate the hospital history and the medical knowledge, helping in the understanding about different traditions on the children care in the hospital context. Thus, in the theoretical aspect, this work seeks to assist in the texture of approximations between the Social Representation Theory, the Cultural - Historical Theory and the studies on narratives. Therefore, it takes as analyses object the social representation of the care in a pediatric Ward, starting from the negotiation between different meanings network shared in this scenario: the health staff narrative, the Binje narrative (FREIRE, 2013) and the children narratives elaborated from the dialogue with the tasks developed in the context of the extension project “Binje: in the search of children authorship in the hospital context”.This research presents the lineament of an ethnographic study, involving an investigation and a two-axis analysis: 1. The context of the professionals in which it was used a participant observation and semi-structured interviews; and 2. The context of the practices developed by the extension project mentioned above with the children, involving the participant observation of the activities and in some moments, a video recording and the semistructured interview.The eleven interviews carried out with the professionals were processed by the IRAMUTEQ computer program that allowed the analysis of the Descendent Hierarchical Classification, favoring the identification of different classes in the presented narratives. Otherwise, the material produced through the observation were analyzed from the interpretative axes which sought to show the articulation between the notebook notes and the theoretical references. Regarding the children, twelve cases were selected, comprehensively analyzed by the means of the interpretative axes as well. As a result, the social dimension of this study pointed out the presence of meaning’s network about the health care. One of them related to the Flexner’s medicine and another one associated to the humanization movement. From this point, in the subjective dimension, the narratives presented by the health professionals and hospitalized children allowed to perceive different negotiations regarding this social knowledge as a mean of assigning meanings to the children care in the hospital scenario, as a way of interaction guidance and of justifying the actions. In the inter-subjective dimension of this study, it has identified that the sharing of the Binje narrative (FREIRE, 2013), by offering the contact with different constructs about the hospitalization, favored the children’s identification with some of the narrative elements, presenting themselves as a support of meaning expression and elaboration about the hospital health experience. The storytelling sections indicated the interaction of the children with value meta-systems associated with the National Humanization Politics indicating the importance of a protagonist posture, of dialogue and co-responsibility towards the health care, which in some of the analyzed cases showed that they were relevant to the change of children behavior in relation the hospital care.

Considera que ao serem inseridas nesse contexto, elas se defrontam com uma pré-estruturação do ambiente social que organiza as interações nas quais as crianças tomam parte. Entende-se que a promoção de interação com narrativas que apresentem significados sobre esse cenário podem favorecer os processos de significação das crianças sobre o cuidado hospitalar, auxiliando-as no delineamento do seu papel em relação a essas práticas. É um estudo que se fundamenta na Teoria das Representações Sociais, elaborada por Moscovici (1978, 2012), elencando pontos possíveis de articulação com a perspectiva Histórico-Cultural do desenvolvimento humano proposta por Vigotski (2010). Juntamente com os estudos de Bruner (2001, 2002, 2008, 2014) sobre a narrativa, busca a compreensão sobre a imbricação desse tipo de conhecimento com os contextos de interação que as pessoas vivenciam. Apresenta um levantamento sobre significações culturais que permeiam a história do hospital e do saber médico, ajudando na compreensão acerca de diferentes tradições sobre o cuidado à criança no contexto hospitalar. Assim, no aspecto teórico, este trabalho busca auxiliar na tessitura de aproximações entre Teoria das Representações Sociais, Teoria Histórico-Cultural e os estudos sobre narrativas. Para tanto, toma como objeto de análise a representação social do cuidado em uma Enfermaria Pediátrica, a partir das negociações entre diferentes redes de significados compartilhadas nesse cenário: as narrativas da equipe de saúde, a narrativa Binje (FREIRE, 2013) e as narrativas infantis elaboradas a partir do diálogo com as atividades desenvolvidas no contexto do projeto de extensão “Binje: em busca de autorias infantis no contexto hospitalar”. Esta pesquisa apresenta contornos de um estudo do tipo etnográfico, envolvendo a investigação e análise de dois eixos: 1. O contexto dos profissionais no qual se utilizou a observação participante e a realização de entrevistas semiestruturadas; e 2. O contexto das práticas desenvolvidas pelo projeto de extensão acima citado com as crianças, envolvendo a observação participante das atividades e em alguns momentos a videogravação e a realização de entrevista semiestruturada. As 11 entrevistas realizadas junto aos profissionais foram processadas pelo programa computacional IRAMUTEQ que permitiu a realização da análise de Classificação Hierárquica Descendente, favorecendo a identificação de diferentes classes nas narrativas apresentadas. Já o material produzido por meio da observação foi analisado a partir de eixos interpretativos que buscavam apresentar articulações das notas de caderno de campo com os referenciais teóricos. Em relação às crianças, foram selecionados 12 casos, analisados compreensivamente, também por meio da configuração de eixos interpretativos. Como resultado, a dimensão social deste estudo indicou a presença de redes de significações sobre o cuidado em saúde. Uma delas relacionada à Medicina Flexneriana e a outra associada aos movimentos de humanização. A partir disso, na dimensão subjetiva, as narrativas apresentadas pelos profissionais de saúde e crianças hospitalizadas permitiram perceber diferentes negociações em relação a esses saberes sociais como forma de atribuírem significações ao cuidado da criança no cenário hospitalar, como forma de orientar interações e justificar ações. Na dimensão intersubjetiva desse estudo, o trabalho identificou que a partilha da narrativa Binje (FREIRE, 2013), ao oferecer o contato com diferentes constructos acerca da hospitalização, favoreceu a identificação das crianças com elementos da narrativa, se apresentando como um suporte para a elaboração e expressão de significações sobre a vivência do cuidado hospitalar.
