A terapia comunitária integrativa e a enfermagem : revisão integrativa de literatura

Resumo: | With social movements, the Psychiatric Reform, and the creation of the Unified Health System (UHS), there was the consolidation of the rights of users in psychological distress. In 2008 the Ministry of Health instituted the National Policy for Integrative Practices in Mental Health, recognizing Integrative Community Therapy (ICT), created by Adalberto Barreto, as one such practice. The present study aimed to gather and identify the evidence available in the scientific literature about ICT as a nursing care practice, thus producing an Integrative Literature Review, which followed the PRISMA recommendations for research and selection of articles for analysis. The methodological approach chosen was exploratory and descriptive, by qualitative research. The study were categorized through thematic analysis, resulting in five thematic axes: What is Integrative Community Therapy?; Who are the community therapists? The TCI method (step by step); Nursing and ICT; TCI's contributions to those involved. The results indicate that the ICT constitutes a space of welcome and listening, which provides the involved improvements such as increased self-esteem, self-knowledge, and overcoming daily problems. In Primary Care it has proved to be an important tool for bringing health professionals closer to users, enabling and strengthening actions for the promotion and prevention of mental health (MH). Nurses showed significant participation in the training of community therapists, and in the implementation of ICT as care in MH at different levels of care, with varied public. It has also been shown that nursing has the most number of scientific publications in the area. It is concluded that ICT is a light-weight care technology, contrary to most studies that classify it as a light technology without considering Merhy's definitions of care technology; and that is in line with SUS guidelines and integrative practices policies. It is characterized as a care action that promotes changes in the quality of health care, the strengthening of communities, and the recovery of people in psychological distress. However, currently there is a tendency to get back mental health care asylum in Brazil. The Technical Note 11/2019, which provides clarification on changes in the National Mental Health Policy and the National Drug Policy Guidelines, threatens the rights gained as they bring back Psychiatric Hospitals and compulsory hospitalization, thus contrary to the global trends in care in MH and the guiding and organizational principles of UHS. Nursing, which has been evidenced as a strengthening category of integrative care practices, should not omit from this setback and organize itself for the struggle in defense of the rights conquered by the Brazilian people. |

Em 2008, o Ministério da Saúde instituiu a Política Nacional de Práticas Integrativas em Saúde Mental, de modo a reconhecer a Terapia Comunitária Integrativa (TCI), criada por Adalberto Barreto, como uma dessas práticas. O presente estudo objetivou reunir e identificar as evidências disponíveis na literatura científica sobre a TCI como prática de cuidado de enfermagem. Dessa forma, foi produzida uma Revisão Integrativa de Literatura, que seguiu as recomendações PRISMA para busca e seleção dos artigos para análise. A abordagem metodológica escolhida foi de caráter exploratório e descritivo, de natureza qualitativa. Os dados foram categorizados por meio de análise temática, que resultou em cinco eixos temáticos: O que é a Terapia Comunitária Integrativa? Quem são os terapeutas comunitários? O método da TCI (passo a passo); A Enfermagem e a TCI; As contribuições da TCI para os envolvidos. Os resultados apontam que a TCI se constitui como um espaço de acolhida e escuta, que proporciona aos envolvidos melhorias como o aumento da autoestima, o autoconhecimento e a superação de problemas cotidianos. Na Atenção Básica, demonstrou ser uma importante ferramenta de aproximação dos profissionais de saúde com os usuários, possibilitando e fortalecendo as ações de promoção e prevenção da saúde mental (SM). Os enfermeiros mostraram significativa participação na formação de terapeutas comunitários e na implementação da TCI como cuidado em SM nos diversos níveis de atendimento, com variados públicos. Demonstrou-se também que a enfermagem detém a maior quantidade de publicações científicas na área. Conclui-se que a TCI é uma tecnologia de cuidado leve-dura, contrariando a maioria dos estudos que a classificam como tecnologia leve sem levar em consideração as definições de tecnologia em cuidado de Merhy; e que está em consonância com as diretrizes do SUS e das políticas de práticas integrativas. Caracteriza-se como uma ação de cuidado que promove mudanças na qualidade da assistência em saúde, no fortalecimento das comunidades, e na recuperação das pessoas em sofrimento psíquico. No entanto, atualmente há uma tendência de remanicomialização do cuidado em saúde mental no Brasil. A Nota Técnica nº 11/2019, que traz esclarecimentos sobre as mudanças na Política Nacional de Saúde Mental e nas Diretrizes da Política Nacional sobre Drogas, ameaça os direitos conquistados, uma vez que traze de volta os Hospitais Psiquiátricos e a internação compulsória, sendo, assim, contrária às tendências mundiais em cuidado na SM e aos princípios norteadores e organizacionais do SUS.
