Resumo: | ABSTRACT: Work plays an important role in the social life of individuals, as well as being a means of growth and building personal identity. Different approaches have been developed to evaluate the impact of high demand and control over nursing work, among which Karasek Demand versus Control theory stands out. The imbalance between psychological demands and control can lead to work overload, loss of skills and interest and may result in occupational stress. The objective of this study was to characterize the work condition and to identify, through a specific instrument, the level of stress of nurses, through the relation between psychological demand and control over work, and to evaluate social support as a factor to protect the effects presumed of the demand / control relationship, in a federal public teaching hospital, in Cuiabá-MT. This is a descriptive study with a cross-sectional design and a quantitative data approach, the sample of which was composed of full-time and in-service nurses in the nursing division of the referred hospital. Data collection took place in the first half of 2017, where two instruments were applied, the sociodemographic and professional questionnaire followed by the work stress scale, validated for Brasil (Job Stress Scale). The project was approved by the Committee of Ethics in Research with Human Beings, with the opinion 252/2016. Data were tabulated and analyzed using absolute (n) and relative (%) frequencies. Among the 85 nurses, 75 (88,3%) are female; 42 (49,4%) reported living with a partner. Age ranged from 24 to 55 years, with the mean age of 31 years (10,6%). Total of 48 (56%) reported having children. As to employment, 68 (80%) said they had only one, followed by 16 (18,8%) with two ties, and only one worker reported three links. Regarding the level of education, 66 (77,6%) reported having at least one specialization in the health area and only 13 (15.3%) have a master's degree. Regarding the health profile at work, 17 (20%) of the total state that they have some chronic disease; nine (10,6%) reported having suffered some type of work accident; and six (7,1%) reported having occupational disease, the most frequently cited being repetitive strain injuries (RSI). Of the 85 nurses, 37 (43,5%) were classified more expressively in the B2 economic classification. Regarding the demand and control characteristics, there was a prevalence of workers with a high labor demand (62,2%) and high control (60%), which in general characterized active work. Social support was high, with 48 (52,2%) among workers. It should be considered that the results bring relevant contributions, because to characterize the working conditions through the prognosticators that define the relation demand, control and social support, allows the managers and the workers themselves to seek measures to soften them and, thus improve the quality of life at work and patient care. |

Diferentes enfoques han sido desarrollados para evaluar el impacto de la alta demanda y control sobre el trabajo de enfermería y, entre ellas se destaca la teoría Demanda versus Control de Karasek. El desequilibrio entre demandas psicológicas y control puede proceder en sobrecarga de trabajo, pérdidas de habilidades e interés pudiendo resultar en estrés ocupacional. El presente estudio tuvo como objetivo de caracterizar la condición de trabajo e identificar por medio de instrumento específico el nivel de estrés de los enfermeros, por medio de la relación demanda psicológica y control sobre el trabajo, y evaluar el apoyo social como un factor de protección a los efectos presumibles de la relación demanda / control, en un hospital público federal de enseñanza, en Cuiabá-MT. Se trata de un estudio descriptivo con delineamiento transversal y abordaje cuantitativo de los datos, cuya muestra fue constituida por enfermeros concurridos y en ejercicio en la división de enfermería del referido hospital. La recolección de datos ocurrió en el primer semestre de 2017, donde se aplicaron dos instrumentos, el cuestionario sociodemográfico y profesional seguido de la escala de estrés en el trabajo, validada para Brasil (Job Stress Scale). El proyecto fue aprobado por el Comité de Ética en Investigación con Seres Humanos, con el dictamen 252/2016. Los datos fueron tabulados y analizados, por medio de frecuencias absoluta (n) y relativa (%). Entre los 85 enfermeros, 75 (88,3%) son del sexo femenino; 42 (49,4%) declaran vivir con compañero (a). La edad varía entre 24 y 55 años, con la edad media de 31 años (10,6%). Un total de 48 (56%) declaran tener hijos. En cuanto a los vínculos laborales, 68 (80%) afirmó tener sólo uno, seguido de 16 (18,8%) con dos vínculos, y sólo un trabajador relató tres vínculos. En cuanto al nivel de formación, 66 (77,6%) mencionó poseer por lo menos una especialización en el área de la salud y apenas 13 (15,3%) poseen maestría. En relación al perfil de salud en el trabajo, 17 (20%) del total declaran poseer alguna enfermedad crónica; nueve (10,6%) declararon haber sufrido algún tipo de accidente de trabajo; y seis (7,1%) declarar poseer enfermedad ocupacional, siendo la más citada las Lesiones por esfuerzo repetitivo (LER / DORT). De los 85 enfermeros, 37 (43,5%) fueron clasificados de forma más expresiva en la clasificación económica nivel B2. En cuanto a las características de Demanda y Control, hubo prevalencia de trabajadores que poseen alta demanda en el trabajo 53 (62,2%) y alto control 51 (60%), lo que caracterizó en general un trabajo activo. El apoyo social se mostró alto, con 48 (52,2%) entre los trabajadores. Se debe considerar que los resultados traen contribuciones relevantes, pues a caracterizar las condiciones de trabajo por medio de los pronosticadores que definen la relación demanda, control y apoyo social, permite que los gestores y los propios trabajadores busquen medidas para suavizarlos y, así, mejorar la calidad de vida en el trabajo y la asistencia a los pacientes.CAPESO trabalho desempenha importante papel na vida social dos indivíduos, além de ser um meio de crescimento e construção de identidade pessoal. Diferentes abordagens têm sido desenvolvidas para avaliar o impacto da alta demanda e controle sobre o trabalho de enfermagem e, dentre elas se destaca a teoria Demanda versus Controle de Karasek. O desequilíbrio entre demandas psicológicas e controle pode proceder em sobrecarga de trabalho, perdas de habilidades e interesse podendo resultar em estresse ocupacional. O presente estudo teve como objetivo caracterizar a condição de trabalho e identificar mediante instrumento específico o nível de estresse dos enfermeiros, por meio da relação demanda psicológica e controle sobre o trabalho, e avaliar o apoio social como um fator de proteção aos efeitos presumíveis da relação demanda/controle. Trata-se de estudo descritivo com delineamento transversal e abordagem quantitativa, cuja amostra foi constituída por enfermeiros lotados em exercício na divisão de enfermagem. A coleta de dados aconteceu no primeiro semestre de 2017, onde foram aplicados dois instrumentos, o questionário sociodemografico e profissional seguido da escala de estresse no trabalho, (Job Stress Scale). O projeto foi aprovado pelo Comitê de Ética em Pesquisa com Seres Humanos, com o parecer 252/2016. Os dados foram tabulados e analisados, por meio de frequências absoluta (n) e relativa (%). Entre os 85 enfermeiros, 75(88,3%) são do sexo feminino; 42(49,4%) declaram viver com companheiro (a). A idade variou entre 24 e 55 anos, com a idade média de 31 anos (10,6%). Um total de 48(56%) declaram ter filhos.Quanto aos vínculos empregatícios, 68(80%) afirmou ter apenas um, seguido de 16(18,8%) com dois vínculos, e apenas um trabalhador relatou três vínculos. Quanto ao nível de formação, 66(77,6%) referiu possuir pelo menos uma especialização na área da saúde e apenas 13(15,3%) possuem mestrado. Em relação ao perfil de saúde no trabalho, 17(20%) do total declaram possuir alguma doença crônica; nove (10,6%) declararam ter sofrido algum tipo de acidente de trabalho; e seis (7,1%) declararm possuir doença ocupacional, sendo a mais os Disturbios Osteomusculares relacionados ao trabalho (DORT). Dos 85 enfermeiros, 37(43,5 %) foram classificados de modo mais expressivos na classificação econômica nível B2. Em relação às características de Demanda e Controle, houve prevalência de trabalhadores que possuem alta demanda no trabalho 53(62,2%), e alto controle 51 (60%), o que caracterizou no geral, um trabalho ativo. O apoio social mostrou-se alto, com 48(52,2%) entre os trabalhadores.
