Resumo: | Rede Cegonha is the most recent strategy proposed by the Brazilian Ministry of Health and aims at humanized assistance to maternal and child health with a focus on pregnancy, childbirth and the puerperium. Brazil is a country with notable regional differences, therefore, the health policies, programs and strategies proposed at the national level lack creative and viable adaptations to reg ional demands. This dissertation proposes a Social Technology conceived from the demand of municipal managers and leaders of health institutions in interior cities of the state of Rio Grande do Sul, for the improvement of management and assistance to mater nal and child health. The research developed and the Social Technology conceived and implemented, observe the Research Priorities Agenda of the Ministry of Health: Axis 14 Maternal and Child Health, as it provides for the analysis of the sociodemographic profile of pregnant women and indicators of care during childbirth, up to monitoring children from 0 to 5 years old and improving health care through continuing education. The objective was to conceive and implement a maternal and child health care strate gy for interior cities, through the analysis of municipal health indicators and the perception of managers and health professionals about the weaknesses and potentialities in ma ternal and child health care. Methodological path: the strategy was designed wi th a focus on three interior towns in the state of Rio Grande do Sul, namely Sobradinho, São Gabriel and Tupanciretã. It included a survey on the health indicators of the third municipality, through official documents. The perception of managers and health professionals was collected through four different questionnaires. 113 responses were obtained, analyzed in seven stages, starting from four dimensions, namely, structural, operational, assistance and professional training. The analysis of the data includ es the tabulation of the answers in the Excel Software, regarding their frequency and correlation, based on a heat map that highlighted potentialities, weaknesses and opportunities for improvement, with results greater than the 50% quartile. Starting from this initial stage, the design and implementation of the Sowing Strategy consists of six more stages: 2nd development of actions to meet priority needs, organized in four fronts of intervention, with actions aimed at life projects of adolescent girls, redu ction of teenage pregnancy rates, implementation of a protocol for prenatal care, creation of a Center for Permanent Education in Health in the care network and systematization of multidisciplinary monitoring of the growth and development of children up to five years of age; 3rd Systematization of actions in a manual; 4th Presentation of the proposal to the municipal health and education managers; 5th Implementation of actions in person; 6th Adaptation of the Sowing Strategy to the virtual environment, in v iew of the context of the pandemic by COVID 19; 7th Evaluation of the strategy by health managers, health professionals and users. The results showed that the greatest weaknesses identified by health professionals and managers are found in the operational and professional training dimensions. Actions were created to meet these demands and the Semear Strategy confirmed itself capable of responding efficiently, presenting a consistent adaptive potential for different locations. The strategy was fully availabl e on a website that brings together videos, podcasts, texts, a group with pregnant women and a course aimed at professionals in maternal and child care. Final considerations: The systematization of the health indicator survey processes, their most differen t articulations and the educational fronts, stood out for reaching health professionals, pregnant women and adolescent girls. The COVID 19 pandemic revealed a positive potential for the Strategy, in its flexibility and consistency when adapted to the virtu al path. The Sowing Strategy product of this work is configured, in short, as a management tool, with enormous potential to favor inland municipalities at the national level. |

O Brasil é um país com notáveis diferenças regionais, portanto, as políticas, programas e estratégias de saúde propostas a nível nacional carecem de adaptações criativas e viáveis às demandas regionais. A presente dissertação propõe uma Tecnologia Social concebida a partir de demanda de gestores municipais e lideranças de instituições de saúde de municípios interioranos do estado Rio Grande do Sul, para o aprimoramento da gestão e da assistência à saúde materno infantil. A pesquisa desenvolvida e a Tecnologia Social concebida e implementada, observam à Agenda de Priorid ades de Pesquisa do Ministério da Saúde: Eixo 14 Saúde Materno Infantil, pois prevê desse a análise do perfil sociodemográfico de gestantes e de indicadores de atenção ao parto, até o acompanhamento de crianças de 0 a 5 anos e o aprimoramento da atenção em saúde por meio da educação continuada. Objetivou se conceber e implementar uma estratégia de atenção à saúde materno infantil para municípios interioranos, por meio da análise de indicadores de saúde municipais e da percepção de gestores e profissionais da saúde sobre as fragilidades e as potencialidades na atenção à saúde materno infantil. Percurso metodológico: a estratégia foi concebida com foco em três municípios interioranos do estado do Rio Grande do Sul, a saber, Sobradinho, São Gabriel e Tupancir etã. Incluiu o levantamento quanto aos indicadores de saúde do terceiro município, por meio de documentos oficiais. A percepção dos gestores e dos profissionais da saúde foi coletada por meio de quatro diferentes questionários. Foram obtidas 113 respostas, analisadas em sete etapas, partindo de quatro dimensões, quais sejam, dimensão estrutural, operacional, assistencial e de capacitação profissional. A análise dos dados compreende a tabulação das respostas no Software Excel, quanto à sua frequência e corre lação, a partir de um mapa de calor que salientou potencialidades, fragilidades e oportunidades de melhorias, com resultados maiores do que o quartil 50%. Partindo dessa etapa inicial, a concepção e a implementação da Estratégia Semear constitui se de mais seis etapas: 2o desenvolvimento de ações para o atendimento de necessidades prioritárias, organizadas em quatro frentes de intervenção, com ações voltadas para projetos de vida de meninas adolescentes, redução dos índices de gravidez na adolescência, impl antação de um protocolo de atenção ao pré natal, criação de um Núcleo de Educação Permanente em Saúde na rede de atenção e a sistematização do acompanhamento multidisciplinar do crescimento e desenvolvimento da criança até cinco anos de idade; 3o Sistemati zação das ações em um manual; 4o Apresentação da proposta para os gestores municipais de saúde e educação; 5o Implementação das ações presencialmente; 6o Adaptação da Estratégia Semear ao ambiente virtual, tendo em vista o contexto de pandemia por COVID 19 7o Avaliação da estratégia por gestores de saúde, profissionais de saúde e usuários. Os resultados evidenciaram que as maiores fragilidades identificadas por profissionais da saúde e gestores encontram se nas dimensões operacional e de capacitação profis sional. Foram criadas ações para atender a essas demandas e a Estratégia Semear se confirmou capaz de responder de maneira eficiente, apresentando um consistente potencial adaptativo para diferentes localidades. A estratégia foi totalmente disponibilizada em um site que reúne vídeos, podcasts, textos, grupo com gestantes e um curso voltado para profissionais da atenção materno infantil. Considerações finais: A sistematização dos processos de levantamento de indicadores de saúde, as suas mais diferentes arti culações e as frentes de caráter educacional, destacaram se por alcançar profissionais de saúde, gestantes e meninas adolescentes. A pandemia de COVID 19 revelou um potencial positivo da Estratégia, em sua flexibilidade e consistência quando adaptada à via virtual. Final considerations: The systematization of the health indicator survey processes, their most differen t articulations and the educational fronts, stood out for reaching health professionals, pregnant women and adolescent girls. The COVID 19 pandemic revealed a positive potential for the Strategy, in its flexibility and consistency when adapted to the virtu al path. The Sowing Strategy product of this work is configured, in short, as a management tool, with enormous potential to favor inland municipalities at the national level. |
