Resumo: | Cesarean delivery rate in Brazil is approximately 55%. Spinal anesthesia is preferred, because it's associated with less maternal complication rates, hasten the post anesthesia recovery and avoid fetal exposition to anesthetic drugs. General anesthesia (GA) is chosen when neuraxial anesthesia is contraindicated or when spinal anesthesia technique fails. Moreover, GA is chosen in emergent situations, when there's not sufficient time to perform neuraxial block. GA indications are been refined as new evidence support or refute this technique. There are plenty international scientific papers on GA in cesarean delivery, but none conducted in Brazil. Research articles rely on data and information accurately inserted in clinical records. However, oftenly, information is wrongly added to clinical records, compromising research results. General Objectives: Assess GA rate in cesarean deliveries that took place in high risk Pregnancy Center in southern Brazil. Specific Objectives: Identify main complications of GA in cesarean deliveries; Create a database software for research purposes, with complete and correct data; Methods: Retrospective analysis of medical record on deliveries with GA in a period of four consecutive years. After literature review, a form was elaborated with main information used on this research topic relevant to the current study. The collected data were analyzed using descriptive statistics. The database was developed by an information technology company based on the forms with medical records information. Results: During the researched period, 5,425 cesarean deliveries were performed, 109 under GA (2.0%). The most common indications where: Obstetric emergencies (45%), failure of neuraxial technique (32%) and contraindications to neuraxial anesthesia (23%). Pulmonary aspiration of gastric contents took place once and one maternal death after the delivery was observed, but not related to the anesthesia. Conclusions: 2% of GA in cesarean deliveries is low when compared with North American literature (5%). There is a global effort to reduce GA for cesarean deliveries, because, when compared with neuraxial anesthesia, it is associated with higher rates of anesthesia related 8 complications. The most frequent indication of GA was related to obstetric emergencies, in line with international literature. Product and applicability: The database software will be able to produce high quality results, as it will properly choose how and what data should be included, minimize data loss and can be updated periodically. This software can be shared with other facilities, collaborating to better understanding the obstetric practices in our country. |

A anestesia neuroaxial (ANA) é a técnica de primeira escolha, pois está associada a menores taxas de complicações maternas, acelera o processo de recuperação pós-operatória e evita a exposição fetal aos anestésicos gerais. A anestesia geral (AG) é utilizada quando há contraindicação ou falha da anestesia neuroaxial e principalmente em cesarianas de emergência, quando há a percepção de tempo insuficiente para a realização das técnicas neuroaxiais. As indicações da AG veem sendo aprimoradas à medida que evidências científicas que as sustentam ou refutam são produzidas. Há grande número de publicações internacionais sobre anestesia geral em cesarianas, entretanto o mesmo não ocorre no Brasil. Evidências científicas dependem de dados e informações corretamente inseridos nos prontuários clínicos, entretanto não é incomum o preenchimento incompleto destes, podendo compromete a qualidade dos resultados de pesquisas. Objetivo geral: Conhecer a taxa de utilização assim como as principais indicações e condições relacionadas ao uso da AG nas cesarianas realizadas em um hospital terciário e referência em gestação de alto risco no sul do Brasil. Objetivos específicos: Identificar as principais complicações relacionadas ao uso da AG nas cesarianas do período estudado; Elaborar um software para banco de dados, específico para cesarianas sob AG, afim de proporcionar pesquisas futuras com dados completos e corretamente inseridos. Método: Avaliação retrospectiva dos prontuários clínicos de todas gestantes submetidas à cesariana sob AG referente a um período de 4 anos consecutivos. Após revisão bibliográfica sobre o tema, elaborou-se um formulário com os principais dados utilizados nas pesquisas desta revisão pertinentes ao atual estudo proposto. Os dados coletados foram analisados através de estatística descritiva. O software foi elaborado por uma empresa de tecnologia da informação a partir dos dados incluídos no formulário utilizado na revisão dos prontuários da pesquisa. Resultados: Durante o período estudado foram realizadas 5.425 cesarianas, sendo 109 sob anestesia geral (2,0%). As indicações mais comuns foram emergências obstétricas (45%), falha da técnica neuroaxial (32%) e contraindicações à anestesia neuroaxial (23%). Como complicação grave relacionada à AG ocorreu em um caso de broncoaspiração do conteúdo gástrico durante a indução anestésica. Houve um óbito materno no período pós-operatório, porém sem relação ao ato anestésico. Conclusão: A taxa de 2% de utilização de AG em cesarianas é baixa quando comparada com a encontrada na literatura norte-americana que é de 5%. Há um esforço global para a redução da AG para cesarianas, pois quando comparada às técnicas neuroaxiais está associada a maiores taxas de complicações relacionadas a anestesia. As indicações mais 7 frequentes da AG foram as relacionadas às emergências obstétricas, em consonância com o observado na literatura internacional. Produto e aplicabilidade: O software para banco de dados será capaz de produzir resultados de alta qualidade, pois definirá de maneira apropriada como e quais dados deverão ser incluídos, minimizará a perda desses dados e poderá ser atualizado periodicamente caso necessário.
