Resumo: Introduction: The diabetic foot is considered one of the main chronic complications of Diabetes Mellitus (DM) and the prime cause of ulceration and amputation of lower limbs. However, several of these complications could be prevented through educational approach and periodic examination. Nurses, among the professionals working in primary health care services, play an important role on the prevention and early detection of complication of the diabetic feet; they are directly involved in the daily care for people with this disease. To this end, it is vital those professionals have knowledge and technical skills to carry out the examination of the feet, in order to prevent and identify such complications. Objective: To analyze the effects of an educational intervention on theoretical-practical knowledge of primary care nurses, face care for diabetic foot examination and prevention. Method: The study of multiple methods developed in two steps: (1) Study methodology with quantitative approach to construction and validation of knowledge Research questionnaire of nurses on diabetic foot (QICEPeD); (2) Intervention study, quasi-experimental, before and after type, quantity and qualitative design. The educational intervention was operationalized according to the steps of the methodology of Problematization with Maguerez arc: Observation of reality, key points, theorizing, chances of solution and application to reality. For data collection there were used two strategies: Application of QICEPeD tool, before and after the educational intervention; and the focal group technique. The empirical material was analyzed quantitatively using SPSS and qualitatively by the technique of content Analysis of the Thematic type. The study was approved by the Committee of Ethics in Research, Opinion N 0577/2015. Results: (1) The process of content validation of the instrument was carried out by a group of 10 judges in two steps. All the items assessed in the second stage they obtained Content Validity Index = 0.90. Analysis of reliability and internal consistency of the total instrument was high (a = 0.860); (2) The reality observed during the educational intervention, indicated that the examination of the feet is not a practice of care usually held during the nursing consultation to people with diabetes, most often is made only when the user requests or concerns a complaint. There was consensus among focus groups that the little knowledge and lack of professional training on the subject are the main factors that hinder the realization of examination of the feet by nurses in primary health care. At the end of the intervention, there were found some changes in nurses’ perception about the need to give more importance to the examination of the feet of diabetic people. There was a significant increase of the minimum and maximum score, and average of hits for the issues investigated in theoretical dimensions I and II, after the educational intervention (p < 0.01). Conclusions: Educational intervention proved effective to promote changes in the theoretical-practical knowledge of these professionals, evidenced by the significant increase in the average of hits, especially in issues related to clinical evaluation of the foot at risk, responding like this, the hypothesis of the study.

Sin embargo, varias de estas complicaciones podrían evitarse a través del enfoque educativo y del examen periódico de los pies. La enfermera, entre los profesionales que trabajan en los servicios de atención primaria, juega un rol importante en la prevención y en la atención a esta complicación; están directamente implicadas en la atención a las personas con esta enfermedad. Para ellas es imprescindible que este profesional tenga conocimiento y habilidades técnicas para llevar a cabo el examen de los pies con el fin de prevenir e identificar tales complicaciones. Objetivo: Analizar los efectos de una intervención educativa acerca del conocimiento teórico práctico de las enfermeras de la atención primaria frente a los cuidados con el pie diabético en su evaluación y prevención. Método: Se trata de un estudio llevado a cabo con métodos múltiples, desarrollado en dos pasos: (1) El estudio metodológico con enfoque cuantitativo a la construcción y validación de encuesta de investigación de conocimiento del personal de enfermería acerca del pie diabético (QICEPeD); (2) Un estudio intervencionista, casi experimental, como antes y después, con cantidades y diseño cualitativo. La intervención educativa fue puesta en marcha siguiendo los pasos de la Metodología de la Problematización con Arco de Maguerez: Observación de la realidad, Puntos Clave, Teorización, Posibilidades de Solución y Aplicación a la Realidad. Para la recolección de datos fueron usados dos estrategias: La aplicación del instrumento de QICEPeD, antes y después de la intervención educativa; y la técnica de grupo focal. El material empírico se analizó cuantitativamente utilizando SPSS y cualitativamente por la Técnica de Análisis de Contenido del Tipo Temático. El estudio fue aprobado por el Comité de Ética en Investigación, Opinión número 0577/2015. Resultados: (1) El proceso de validación de contenido del instrumento se llevó a cabo por un grupo de 10 jueces en dos pasos. Todos los elementos evaluados en la segunda etapa obtuvieron el Índice de Validez de Contenido = 0,90. El análisis de fiabilidad y consistencia interna del instrumento total fue alta (a = 0,860); (2) La realidad observada durante la intervención educativa indicó que el examen de los pies no es una práctica de atención generalmente llevada a cabo durante la consulta de enfermería a personas con diabetes, pero, a menudo, se hace solamente cuando el usuario solicita o refiere a una queja. Fueron el consenso de los grupos de enfoque de poco conocimiento y la falta de formación profesional acerca del tema los principales factores que impidieron la realización del examen de los pies por las enfermeras en la Atención Primaria de Salud. Al final de la intervención, identificaron algunos cambios en la percepción del personal de enfermería de la necesidad de dar más importancia al examen de los pies de las personas con diabetes. Hubo un aumento significativo de la puntuación mínima y máxima y promedio de aciertos por los temas investigados en dimensiones teóricas I y II, después de la intervención educativa (p < 0.01). Conclusiones: La intervención educativa mostró se eficaz en la promoción de cambios en el conocimiento teórico práctico de estos profesionales, evidenciado el aumento significativo del promedio de aciertos, especialmente en los temas relacionados con la evaluación clínica del pie en riesgo; respondiendo así, la hipótesis del estudio.NenhumaIntrodução: O pé diabético é considerado uma das principais complicações crônicas do Diabetes Mellitus (DM) e a maior causa de ulceração e amputações de membros inferiores. Entretanto, várias dessas complicações poderiam ser prevenidas por meio da abordagem educativa e do exame periódico dos pés. O enfermeiro, entre os profissionais que atuam nos serviços de Atenção Primária à Saúde, desempenha um papel importante na prevenção e rastreamento dessa complicação, por estar diretamente envolvido na prestação de cuidados às pessoas com a doença. Para tanto, é imprescindível que esse profissional tenha conhecimento e habilidades técnicas para realizar o exame dos pés de forma a prevenir e identificar complicações. Objetivo: Analisar os efeitos de uma intervenção educativa sobre o conhecimento teórico-prático de enfermeiros da atenção primária, frente aos cuidados para prevenção e avaliação do pé diabético. Método: Tratase de um estudo de métodos múltiplos, desenvolvido em duas etapas: (1) estudo metodológico com abordagem quantitativa para construção e validação do Questionário de investigação do conhecimento de enfermeiros sobre pé diabético (QICEPeD); (2) estudo de intervenção, quase experimental, do tipo antes e depois, com desenho quanti e qualitativo. A intervenção educativa foi operacionalizada de acordo com as etapas da Metodologia da Problematização com o Arco de Maguerez: Observação da realidade, Pontos-Chave, Teorização, Hipóteses de Solução e Aplicação à realidade. Para a coleta de dados foram utilizadas duas estratégias: aplicação do instrumento QICEPeD, antes e após a intervenção educativa; e a técnica de grupo focal. O material empírico foi analisado quantitativamente utilizando o SPSS e qualitativamente pela técnica de Análise de Conteúdo do tipo Temática. O estudo foi aprovado pelo Comitê de Ética em Pesquisa, Parecer n. 0577/2015. Resultados: (1) O processo de validação de conteúdo do instrumento foi realizado por um grupo de 10 juízes em duas etapas. Todos os itens avaliados na segunda etapa obtiveram Índice de Validade de Conteúdo = 0,90. A análise da confiabilidade e da consistência interna total do instrumento foi considerada elevada (a=0,860). (2) A realidade observada durante a intervenção educativa indicou que o exame dos pés não é uma prática de cuidado costumeiramente realizada durante a consulta de enfermagem às pessoas com diabetes, na maioria das vezes é feita apenas quando o usuário solicita ou refere alguma queixa. Foi consenso entre os grupos focais que o pouco conhecimento e a falta de capacitação profissional sobre a temática são os principais fatores que dificultam a realização do exame dos pés pelo enfermeiro na atenção primária. Ao final da intervenção, identificaram-se algumas transformações na percepção dos enfermeiros quanto à necessidade de dar mais importância ao exame dos pés das pessoas com diabetes. Observou-se um expressivo aumento da pontuação mínima e máxima, e da média de acertos para as questões investigadas nas dimensões teóricas I e II, após a intervenção educativa (p<0,01).
