Análise da utilização do incentivo financeiro do PMAQ-AB sob a ótica do gestor municipal

Resumo: INTRODUCTION The historical trajectory of health policies in Brazil has leveraged the construction of Sus and effectiveness as a current model, strengthening official assistance and the largest country in the world to use a public health system based on the principles of universality, equity and integrality. The processes that promote the quality of care are important instruments for advances in SUS, with a view to improving quality, producing knowledge about reality. The PMAQ-AB proposes to expand the population's access to ABS services, with quality of services provided, through practices of management practices transformed in the work processes in the Basic Health Units. This research was organized by the organization: how do municipal managers in the state of Paraíba use the advanced financial resources of the PMAQ? OBJECTIVE To analyze how advanced financial resources were used for the Program to Improve Access and Quality of Primary Care, from the perspective of managers in the state of Paraíba during three cycles. METHODOLOGY: this is an exploratory, descriptive study with a quantitative approach, which used a questionnaire addressing standards, transfer and reflexes from financial resources, applied with municipal health managers. The study was submitted to the Ethics Committee on Research with Humans at the Health Sciences Center of the Federal University of Paraíba, under number CAAE: 90331418.6.0000.8069. The study sample consists of 219 participants. Descriptive data analysis, statistical inference and hierarchical cluster analysis were performed. RESULTS AND DISCUSSION: the analysis covered the three cycles of the program. It showed that most of the participants occupied the position of health secretary, an important fact for being the decision maker. Existence of a municipal law regulating the transfer of financial resources from PMAQ since cycle 1. The variables that analyzed the transfer of resources to the teams showed greater similarities in the decision making of managers for cycle 2 and 3. Within the reflexes of decision making in the transfer of resources, problems were identified among the members of the health teams and between them and the management, although they see improvements in all the categories analyzed. FINAL CONSIDERATIONS: as an innovative practice, the PMAQ brought essential aspects for the strengthening of access and quality, considering the self-assessment / M & A and the P4P modality as an instrument of practices that foster bases for assistance with excellence. The decision-making manager has the potential to collaborate with the construction of knowledge in the field of essential improvements for ABS.

Os processos que promovem qualidade da assistência são instrumentos importantes para os avanços no SUS com vistas à melhoria da qualidade, produzindo conhecimento sobre a realidade. O programa de melhoria de acesso e qualidade da atenção básica (PMAQ-AB) propôs ampliar o acesso da população aos serviços da atenção básica à saúde, com qualidade dos cuidados prestados, através de estratégias de práticas de gestão transformadoras dos processos de trabalho nas Unidades Básicas de Saúde. A presente pesquisa foi norteada pela indagação: de que forma os gestores municipais do Estado da Paraíba utilizaram os recursos financeiros advindos do PMAQ-AB? OBJETIVO Analisar o modo como foram utilizados os recursos financeiros advindos do Programa de Melhoria do Acesso e Qualidade da Atenção Básica a partir da perspectiva dos gestores do estado da Paraíba durante os três ciclos. METODOLOGIA: trata-se de um estudo exploratório, descritivo e analítico, com abordagem quantitativa, que utilizou questionário abordando normativas, repasse e reflexos provenientes dos recursos financeiros, aplicados com gestores municipais de saúde. O estudo foi submetido da comitê de Ética em pesquisa com seres humanos no Centro de Ciências da Saúde da Universidade Federal da Paraíba, sob número CAAE: 90331418.6.0000.8069. A amostra do estudo é composta por 219 gestores municipais do estado da Paraíba. Foi realizado análise descritiva dos dados, inferência estatística e análise de agrupamento hierárquico. RESULTADOS E DISCUSSÃO: a análise contemplou os três ciclos do programa. Mostrou que a maioria dos participantes ocupavam o cargo de secretário de saúde, fato importante por ser o tomador de decisões. Existência de lei municipal regulamentando repasse de recursos financeiros do PMAQ-AB desde o ciclo 1. As variáveis que analisaram o repasse de recursos para as equipes demostraram maiores similaridades na tomada de decisão dos gestores para o ciclo 2 e 3. Dentro dos reflexos das tomadas de decisões no repasse dos recursos, foram identificados problemas entre os membros das equipes de saúde e entre estes e a gestão, embora vislumbrem melhorias em todas as categorias analisadas. CONSIDERAÇÕES FINAIS: como prática inovadora, o PMAQ-AB trouxe vertentes essenciais para o fortalecimento do acesso e qualidade, haja vista o compomente de monitoramento e avaliação e a modalidade de pagamento por performance como instrumentos de práticas que fomentam bases para uma assistência com excelência.
