Análise da progressão e da reversão de lesões artificiais de cárie através de espectroscopia Raman e tomografia de coerência óptica

Resumo: | There are several protocols for the development of artificial carious lesions, and pH-cycling models, involving the exposure of substrates to the combination of demineralization and remineralization, are the most used. Usually, the methodologies analysis of the lesions obtained based on surface changes, and no information is obtained for deeper layers. The aim of this study was to analyze the progression and the reversal of artificial carious lesions in human enamel. Fifty third molars were used to obtain one hundred enamel blocks. Treatment consisted of a demineralizer pH cycling for different periods of time: G1 (1 day), G2 (3 days), G3 (5 days), G4 (7 days), G5 (9 days) and G6 (11 days). The remineralizing treatment used pH cycling for 7 days and neutral 2% fluoride gel applications, on alternate days. The samples were analyzed before and after treatments using Raman Spectroscopy (RS) for the analysis of the content of calcium phosphate and Optical Coherence Tomography (OCT) for the analysis of morphological characteristics. After the demineralization treatment it was observed greater loss of calcium phosphate for G1 compared to the other groups. The results showed that G1 and G2 presented an unchanged surface after demineralization treatment; in G3 and G4 it was observed surface irregularities, indicating initial erosion of the enamel surface, which progressed, resulting in exposure of the underlying dentin (G6). Beneath of the surface there was a progressive demineralization, according to increasing periods of time, and the cervical region showed greater demineralization than the occlusal region. In respect to the loss and gain of calcium phosphate, it was observed mineral loss for G1 and G2 after the remineralization treatment, while G3, G4, G5 and G6 showed gain of calcium phosphate. The morphological analysis after remineralization showed that there were reversals of the process only for G5. Longer storage periods showed a gradual demineralization effect on the surface and subsurface of the enamel. The cervical region of the specimens was more sensitive to demineralization than did the occlusal region. Shorter periods of storage showed no response to remineralization treatment. |

Shorter periods of storage showed no response to remineralization treatment.Coordenação de Aperfeiçoamento de Pessoal de Nível SuperiorExistem vários protocolos para o desenvolvimento de lesões artificiais de cárie, no entanto, modelos de ciclagem de pH, envolvendo a exposição de substratos dentários à combinação de desmineralização e remineralização são os mais utilizados, por mimetizar melhor a dinâmica de perda e ganho de minerais. As metodologias empregadas para analisar as lesões obtidas costumam basear-se em análise da superfície, não fornecendo informações sobre alterações ocorridas nas camadas mais profundas. O objetivo do presente estudo foi analisar características de progressão e reversão de lesões artificiais de cárie em esmalte humano. Para tanto, utilizou-se 50 terceiros molares inclusos, dos quais se obteve cem blocos de esmalte. O tratamento desmineralizador consistiu em ciclagem de pH por diferentes períodos de tempo: G1(1 dia), G2(3 dias), G3(5 dias), G4(7 dias), G5(9 dias) e G6(11dias). E o tratamento remineralizador em aplicações de flúor gel neutro a 2%, em dias alternados, concomitante a ciclagem de pH, durante 7 dias. As amostras foram analisadas antes e após os tratamentos, utilizando Espectroscopia Raman (ER) para análise de conteúdo de fosfato de cálcio e Tomografia de Coerência Óptica (TCO) para análise das características morfológicas. Após tratamento desmineralizador observou-se perda de fosfato de cálcio menor em G1 do que nos demais grupos. Os resultados mostraram que em G1 e G2 a superfície apresentou-se inalterada após tratamento desmineralizador; em G3 e G4 observaram-se irregularidades, denotando erosão inicial da superfície do esmalte, as quais progrediram, resultando em exposição da dentina subjacente em G6. Abaixo da superfície houve progressão da desmineralização, de acordo com os períodos crescentes de tempo, sendo que a região cervical apresentou maior desmineralização do que a oclusal. Quantificando perda e ganho de fosfato de cálcio, observou-se para G1 e G2 perda mineral após o tratamento remineralizador, enquanto G3, G4, G5 e G6 apresentaram ganho. Com relação à análise morfológica após remineralização, observou-se reversão do processo apenas para G5. Períodos maiores de armazenamento apresentaram efeito gradativo na desmineralização da superfície e da subsuperfície do esmalte. A região cervical apresentou-se mais sensível à desmineralização e exposição dentinária do que a oclusal.
