Quantitative cross epidemiological study held at the Municipal Secretary of Guarapuava-Paraná State–Brazil. The objective was: Describe the information contained in “HIPERDIA” files. 5,500 registration forms were researchedwith a sample of 612 forms. Data were collected during the month of April/2005 and the studied period was from May 2002until December 2004. Information was entered after the creation of a database program in Microsoft Excel and analyzedafter being coded and presented using the statistical program Statistical Package For The Social Sciences (SPSS). Theresults show that “HIPERDIA” Health Information System was created to help with care reorganization of people sufferingfrom hypertension and diabetes mellitus. Some data required by the Ministry of Health (MS) are missing in some records:race, educational level, marital status, waist, weight, height, date of consultation and signing of the service responsible. Itis considered that the omission of such data may interfere with the final processing of forms, and not supplement them,negatively influencing on possible studies of the epidemiological profile of this population as well as making impossiblethe monitoring and implementation of strategies that could transform the current condition of these individuals .
