Resumo: This study aimed at analysing registered information in the Patient Health Record and to identify the profile of first time pregnant women related to demographic, admission physical examination, and obstetrical data. It is a descriptive-exploratory study about Patient Health Record registries of first time pregnant women who had normal childbirth in a referral hospital in the city of São Paulo. One hundred and twelve health records constituted the sample. Of the data under analysis, obstetrical data had major incidence of irregular registries, being the variables specular examination, amniotic sac integrity, presentation of the sac, and data of the pelvic basin the most unregistered data. It was concluded that the fact of not registering these data can be a negative factor in the decision making process during normal childbirth.

Trata-se de um estudo descritivo-exploratório sobre os registros de informações em prontuário de mulheres primigestas que evoluíram para o parto normal em um hospital de referência na cidade de São Paulo. A amostra foi constituída por 112 prontuários. Dos registros analisados, os dados obstétricos foram os que tiveram maior incidência de registro irregular, sendo que as variáveis exame especular, integridade da bolsa amniótica, apresentação da bolsa e dados da bacia foram os que tiveram menor índice de registro. Conclui-se que o não registro destas variáveis pode ser um fator negativo no processo de tomada de decisão durante o trabalho de parto.Este estúdio objectivó analisar las informaciones registradas em lo prontuário del paciente y identificar los datos del perfil demográfico, del exame admisional y los datos del exame obstétrico. Tratase de un estúdio descritpivo-exploratorio sobre los registros de mujeres primigestas que tuvieron parto normal en un hospital de referencia en la cuidad de São Paulo. La amostra fué constituyda por 112 prontuários. De los datos analisados, los datos obstetricos tuvieron mayor incidencia de registros iregulares, siendo las variables exame especular, integridad de la bolsa amniotica, presentación de la bolsa y datos de la bacia los que tuvieron la más baja taja de registro. Se concluye que no registrarse estos datos puede ser un factor negativo en el proceso de toma de decisiones en el parto normal.
