Defesa: Curitiba, 06/04/2017Inclui referências : f. 84-96Resumo: O Diabetes mellitus (DM) é caracterizado por um quadro hiperglicêmico decorrente da deficiência na síntese, na ação da insulina, ou ambos. O diagnóstico precoce do diabetes, atrelado ao adequado tratamento e monitoramento, permite minimizar e postergar o aparecimento das complicações associadas (nefropatia, retinopatia, doença cardiovascular, entre outras), melhorando a qualidade de vida dos afetados e reduzindo os custos do sistema de saúde pública. Atualmente, os critérios diagnósticos para o diabetes ainda são baseados na glicemia em jejum, teste oral de tolerância a glicose ou dosagem de hemoglobina glicada. Considerando a complexidade e as complicações da doença e que o diagnóstico é realizado apenas na medição de glicose e na dosagem de hemoglobina glicada, é de extrema importância que novos biomarcadores confiáveis para o diagnóstico, tratamento e prognóstico da patologia sejam pesquisados. Portanto, o objetivo do trabalho foi a detecção e identificação de proteínas de membrana e do citoplasma de eritrócitos de pacientes com DM do tipo 1, DM do tipo 2 com mau e bom controle glicêmico e indivíduos sem diabetes a fim de avaliar se o perfil proteico entre esses grupos apresentaria diferenças densitométricas. Para isso, as proteínas dos eritrócitos, tanto de membrana quanto do citosol foram extraídas, separadas por eletroforese SDSPAGE, analisadas por densitometria e identificadas por espectrometria de massa do tipo MALDI-TOF. Do total de 11 bandas proteicas visualizadas no gel de poliacrilamida, 4 bandas apresentaram diferenças no valor de volume relativo e essas bandas foram identificadas como proteína 4.1, gliceraldeído-3-fostato desidrogenase (G3PDH), banda 3 ou mistura de proteína 4.1 e banda 3 ou mistura de G3PDH e banda 3. No grupo DM1 com bom controle glicêmico e no grupo controle, os volumes relativos da banda proteica identificada como mistura de proteína 4.1 e banda 3 foram respectivamente superiores e estatisticamente significativos (18% ± 5 e 23% ± 6, P<0,05) em relação ao grupo com mau controle glicêmico (13% ± 2). O volume relativo da mistura de gliceraldeído-3-fosfato desidrogenase e banda 3 (43% ± 11) no grupo DM1 com mau controle glicêmico foi estatisticamente superior (P<0,05) em relação ao volume da banda 3 (26% ± 4) na condição de bom controle glicêmico e no controle não houve detecção de banda. Para o grupo DM2 com mau e bom controle glicêmico, os volumes relativos da enzima G3PDH não apresentaram diferença com significância estatística (P<0,05). No entanto, a G3PDH não foi identificada no grupo controle. Diante dos resultados obtidos para o grupo DM1, os volumes relativos tanto da proteína 4.1 e a G3PDH foram superiores na condição de mau controle glicêmico, enquanto para o grupo DM2, o volume relativo de G3PDH foi superior na condição de mau e bom controle glicêmico em relação ao controle. Portanto, é possível inferir que os eritrócitos de pacientes DM1 e DM2 com mau controle glicêmico possam apresentar propriedades reduzidas de deformabilidade e estabilidade mecânica, uma vez que a proteína 4.1 participa na regulação do citoesqueleto. Já a enzima citosólica G3PDH pode indicar uma condição de estresse oxidativo celular, uma vez que nessa condição, a enzima se associa ao domínio citosólico da proteína integral de membrana, a banda 3.Abstract: Diabetes mellitus (DM) is characterized by a hyperglycemic condition due to deficiency in synthesis, insulin action, or both. The early diagnosis of diabetes, coupled with the adequate treatment and monitoring, allows minimizing and delaying the appearance of associated complications (nephropathy, retinopathy, cardiovascular disease, among others), improving the quality of life of those affected and reducing the costs of the public health system . Currently, the diagnostic criteria for diabetes are still based on fasting blood glucose, oral glucose tolerance test or glycated hemoglobin dosage. Considering the complexity and complications of the disease and that the diagnosis is only made in the measurement of glucose and in the measurement of glycated hemoglobin, it is of the extreme importance that new reliable biomarkers for the diagnosis, treatment and prognosis of the pathology be investigated. Therefore, the objective of the study was to detect and identify erythrocyte membrane and cytoplasm proteins of patients with type 1 DM, type 2 DM with poor and good glycemic control and individuals without diabetes in order to assess whether the protein profile between these groups would present densitometric differences. For this, both erythrocyte membrane and cytosol proteins were extracted, separated by SDS-PAGE electrophoresis, analyzed by densitometry and identified by MALDI-TOF type mass spectrometry. From the total of 11 protein bands visualized on the polyacrylamide gel, 4 bands presented differences in the relative volume value and these bands were identified as protein 4.1, glyceraldehyde-3-phosphate dehydrogenase (G3PDH), band 3 or protein 4.1 and band 3 or mixture of G3PDH and band 3. In the DM1 group with good glycemic control and in the control group, the relative volumes of the protein band identified as a mixture of protein 4.1 and band 3 were respectively higher and statistically significant (18% ± 5 and 23% ± 6, P <0.05) in relation to the group with poor glycemic control (13% ± 2). The relative volume of the mixture of glyceraldehyde- 3-phosphate dehydrogenase and band 3 (43% ± 11) in the DM1 group with poor glycemic control was statistically superior (P <0.05) in relation to the volume of band 3 (26% ± 4) in the condition of good glycemic control and there was no band detection in the control. For the DM2 group with poor and good glycemic control, the relative volumes of G3PDH enzyme did not present difference with statistical significance (P <0.05). However, G3PDH was not identified in the control group. Considering the results obtained for the DM1 group, the relative volumes of both protein 4.1 and G3PDH were higher in the condition of poor glycemic control, while for the DM2 group, the relative volume of G3PDH was higher in the condition of poor and good glycemic control in relation to control group. Therefore, it is possible to infer that the erythrocytes of DM1 and DM2 patients with poor glycemic control may have reduced deformability and mechanical stability properties, since protein 4.1 participates in cytoskeletal regulation.
