The clinical and social vulnerability of children with special healthcare needs.

Resumo: | Children with special healthcare needs (CSHCN) require increasing care by relatives and health services. Due to the ‘invisibility’ of this group, a qualitative studied was carried out to analyze and to discuss the dimension of physical care provided to CSHCN. The studied included the analyses of the patients’ records, interviews of family caretakers in a university hospital in southern Brazil, and the application of a sensitive creative method. Findings showed that these children are clinically and socially vulnerable. Consequently, during home care, their caretakers face challenges related to care complexity and to the fact of being women. The presence of the State is scarce, and follow-up nursing after discharge is absent. It is recommended that these caretakers are taught some nursing knowledge, and that actions are taken to improve prenatal care services as a preventive measure. |

La invisibilidad del grupo exigió un estudio con el objetivo de analizar y discutir la dimensión del cuidado físico de los CSHCN. El estudio cualitativo consistió en el análisis de prontuarios, entrevista con cuidadoras familiares de CSHCN en un hospital universitario del sur de Brasil y la aplicación del método creativo sensible, indicando que ellas son clínicamente frágiles y socialmente vulnerables. Por consiguiente, las cuidadoras de estos niños enfrentan en el cuidado domiciliario, desafíos relacionados con la complejidad del cuidar y con la condición femenina. Hay poca presencia del Estado y ausencia de la enfermería en el seguimiento post alta. Se recomienda la incorporación de los conocimientos de enfermería al cuidado prestado por estas cuidadoras, así como acciones que califiquen los servicios de atención prenatal como forma de prevención.As crianças com necessidades especiais de saúde (CRIANES) apresentam demandas de cuidados crescentes para as famílias e serviços de saúde. A invisibilidade do grupo requereu um estudo com o objetivo de analisar e discutir a dimensão do cuidado físico às CRIANES. O estudo qualitativo consistiu de análise de prontuários, entrevista com cuidadoras familiares de CRIANES em um hospital universitário do sul do Brasil e aplicação do método criativo sensível. Os achados indicaram que elas são clinicamente frágeis e socialmente vulneráveis. Conseqüentemente, suas cuidadoras familiares enfrentam, no cuidado domiciliar, desafios relacionados à complexidade do cuidar e à condição feminina. Há pouca presença do Estado e ausência da enfermagem no seguimento pós-alta.
