The nursing work in the production of health care in the clinical model

Resumo: The subject of this research study is the technological and social organization of health work in hospital, considering the position of nursing practice within production of care. The aim is to analyze the work process at a University Hospital to apprehend the configuration of practices, knowledges, social relations and technologies that are operated by agents in intervention on the body of an ill person. Using a dialectical approach, empirical data were collected by direct observation of the work process in a medical in-patient unit and by semi-structured interviews with professionals. It was found that the production of care is developed based on collective work, that is structured by the division in parts and fragmentation of the actions of the different agents, in order to produce a diagnosis and therapy. The agents articulate their actions around the medical act which takes up a central position in the production of care. The importance of broadening the clinical model to contemplate the social and subjective dimension of the health/disease process is pointed out in a therapeutical project shared by different health care team professionals. It is proposed to change the work process associated with the construction of new management models in health care institutions.

Se tiene como objetivo analizar el proceso de trabajo en un hospital universitário, buscando aprehender la configuración de las prácticas, de los saberes, de las relaciones sociales y de las tecnologías que son utilizadas por los agentes para la intervención en el cuerpo enfermo. Se utilizó un abordaje dialético y la recoleta de los datos empíricos se procedió utilizándose la observación directa del proceso de trabajo en una unidad de internación y entrevistas semi-estructuradas con los profesionales. Se constató que la producción de cuidados se desarrolla a partir de un trabajo colectivo, que se estructura por parcelamiento y fragmentación de las acciónes de los diferentes agentes, para se obtener como producto final el diagnóstico y la terapéutica. Los agentes se articulan al redor del acto médico, que tiene posición central en la producción de los cuidados. Se apunta la importancia de la ampliación del modelo clínico, con la inclusión de la dimensión social y subjetiva en el proceso salud/enfermedad, en un proyecto terapéutico compartido por los diferentes profesionales del equipo de salud. Se propone la transformación del proceso de trabajo asociada a la construcción de nuevos modelos de gestión de las instituciones de salud.O objeto desta pesquisa é a organização tecnológica e social do trabalho em saúde no hospital, privilegiando o espaço da prática de enfermagem na produção de cuidados. Tem-se como objetivo analisar o processo de trabalho em um hospital universitário, procurando apreender a configuração das práticas, dos saberes, das relações sociais e das tecnologias que são operadas pelos agentes para intervenção sobre o corpo doente. Utilizou-se uma abordagem dialética, procedendo-se à coleta dos dados empíricos através de observação direta do processo de trabalho em uma unidade de internação de clínica médica e de entrevistas semi-estruturadas com os profissionais. Constatou-se que a produção de cuidados desenvolve-se a partir de um trabalho coletivo, que se estrutura através do parcelamento e da fragmentação das ações dos diferentes agentes, para obter como produto final o diagnóstico e terapêutica. Todos os agentes se articulam em torno do ato médico, que tem posição central na produção de cuidados. Aponta-se a importância da ampliação do modelo clínico, contemplando a dimensão social e subjetiva do processo saúde/doença, em um projeto terapêutico compartilhado pelos diferentes profissionais da equipe de saúde.
