Avaliação dos critérios convencionais preditivos de desmame de suporte ventilatório mecânico em pacientes idosos durante a ventilação mecânica com tubo T

Resumo: Aging causes strutural and functional changes in respiratory system. Changes in the lung parenchyma and chest wall can be observed in elderly patients. There is no evidence, however, that these changes could influence the predictors for weaning from mechanical ventilation.The objective of this study was to evaluate if the predictors of weaning outcome could change in elderly patients. It was a cross-sectional study in wich collected data from 21 ICU patients (eleven with less than 60 and 10 ³ 60 years). All patients were mechanically ventilated in the pressure support mode and clinically stable to wean, before the trial with t- tube was done. It was evaluated: tidal volume (VT), volume exalated (VE), respiratory rate(f), rapid shallow breathing index, gasometric data and APACHE II (acute physiology chronic health evaluation). The statistic analises was done with the student-t test. All predictable index had no statistic significant diference between the studied groups.However the APACHE II was higher for the elderly (p=0.02). In conclusion this study suggested that in the studied groups there were no differences in the conventional criteria for predicting successful weaning comparing adults and eldrely patients.
