Caracterização da terapia nutricional enteral em pacientes críticos com alteração glicêmica

Resumo: | Introduction: Critically ill patients often present with glycemic changes and adequate enteral nutritional therapy favors the reduction of physiological stress and maintenance of immunity. The objective of this study was to describe the characteristics of the enteral diet and glycemic control in critically ill patients. Methods: A cross-sectional study with patients admitted to the Intensive Care Unit (ICU) of the Onofre Lopes University Hospital (HUOL) in Natal / RN. Project approved by the Ethics and Research Committee / HUOL, with CAAE: 41416814.5.0000.5292. Data were collected with a questionnaire specific to the study. Results: The final sample consisted of 67 patients, mean age of 58 (15.02) years and 50.7% of males. It was observed that 28.4% presented a diagnosis of Type 2 Diabetes Mellitus, 50.7% Sepsis and 28.9% died during ICU admission. Regarding caloric needs, 76.1% met the minimum needs, 65.7% reached the average and 14.9% managed to reach the maximum need, according to the recommendation of ESPEN and ASPEN. Regarding proteins, 64.2% reached the minimum requirement, 28.4% the mean and no patient was able to reach the maximum need. It was recorded that 79.1% received polymeric diets, in which 70.1% were hypercaloric and hyperproteic and 9% were normocaloric and normoproteic and 20.9% received oligomeric diets. Blood glucose levels above 180mg / dL were observed in 49.3% of the 67 patients who received a hyperprotein diet, 53.1% of the 49 patients who received a hypoglycemic diet, and 38.8% of the 18 patients who received a normoglycemic diet. Conclusion: The highest number of diets administered were polymeric diets and there was alteration of glycemic control. |

Objetivou-se descrever as características da dieta enteral e o controle glicêmico em pacientes críticos. Métodos: Estudo transversal com pacientes internados na Unidade de Terapia Intensiva (UTI) do Hospital Universitário Onofre Lopes (HUOL) em Natal/RN. Projeto aprovado pelo Comitê de Ética e Pesquisa/HUOL, com CAAE: 41416814.5.0000.5292. Coletou-se dados com questionário especifico para o estudo. Resultados: A amostra final foi de 67 pacientes, idade média de 58 (15,02) anos e 50,7% do sexo masculino. Observou-se que 28,4% apresentavam diagnóstico de Diabetes Mellitus tipo2, 50,7% Sepse e 28,9% foram a óbito durante internação na UTI. Em relação as necessidades calóricas, 76,1% atingiram as necessidades mínimas, 65,7% alcançaram a média e 14,9% conseguiram atingir a necessidade máxima, de acordo com a recomendação da ESPEN e ASPEN. Em relação as proteínas, 64,2% alcançaram a necessidade mínima, 28,4% a média e nenhum paciente conseguiu alcançar a necessidade máxima. Registrou-se que 79,1% receberam dietas poliméricas, no qual 70,1 % eram hipercalóricas e hiperproteicas e 9% eram normocaloricas e normoproteicas e 20,9% receberam dietas oligoméricas. Verificou-se glicemias acima de 180mg/dl em 49,3% dos 67 pacientes que receberam dieta hiperprotéica, 53,1% dos 49 que receberam dieta hipoglicídica e 38,8% dos 18 que receberam dietas normoglicidica.
