Abordagem cirúrgica em tumores malignos de glândulas salivares menores: uma revisão sistemática

Resumo: Minor salivary gland neoplasms may represent between 41.9% and 61.8% of all salivary gland tumors, with being the most common cystic adenoid carcinoma followed by mucoepidermoid carcinoma and polymorphous adenocarcinoma. In general, surgical resection is widely accepted as a primary treatment followed by post-operative radiotherapy especially in cases of unfavorable prognosis. The aim of this study was to evaluate which surgical approach is most appropriate for malignant tumors of the minor salivary glands. This is a systematic review, in which searches were performed in the databases PubMed / Medline, LILACS, SciELO and BBO. Studies that aimed to evaluate minor salivary gland neoplasms and which surgical treatment is most appropriate for each case, in the Portuguese, English and Spanish languages with a publication date up to 10 years were included. Studies that addressed larger salivary glands neoplasms or whose approach was directed to only clinical and pathological aspects besides studies that are case reports or duplicates from one database to another were excluded.532 articles were found and 11 were selected for analysis. It was concluded that the most recent studies are still quite divergent as to the best way of treating these lesions despite the fact that there is a predilection for a surgical approach associated with post-operative radiotherapy which is already recommended in the literature.

No geral, a ressecção cirúrgica é amplamente aceita como uma forma primária de tratamento, seguida da radioterapia pós-operatória, principalmente em casos de prognóstico desfavorável. O objetivo desse estudo é avaliar qual abordagem cirúrgica é mais indicada para os tumores malignos de glândulas salivares menores.Trata-se de uma revisão sistemática, na qual foram realizadas buscas nas bases de dados eletrônicas PubMed/Medline, LILACS, SciELO e BBO. Foram incluídos estudos que tiveram como objetivo avaliar neoplasias de glândulas salivares menores e qual tratamento cirúrgico mais indicado para cada caso, nos idiomas português, inglês e espanhol, com data de publicação de até 10 anos. Foram excluídos estudos que abordassem neoplasias de glândulas salivares maiores, ou cuja abordagem fosse direcionada para apenas aspectos clínicos e patológicos, além de estudos que sejam relatos de casos ou duplicados de uma base para outra. Foram encontrados 532 artigos, dos quais 11 foram selecionados para análise.
