Resumo: The elderly population arouses great interest in the present, due to its growth, and it is increasingly necessary to elucidate the clinical conditions that may indicate the appearance of cognitive decline in this population. This dissertation was elaborated from two articles. The first presents an investigation in the literature, based on the systematic review model, with the objective of investigating the cognitive decline in aging and the use of executive and functional instruments. With this review, we observed that the executive functions are related to the competences in the daily actions, and when associated to the functional instruments contribute to a better understanding of the underlying alterations in the cognitive decline, evidencing the clinical diagnosis. For this study, 2070 documents were cataloged and after a careful examination, 30 documents categorized by combination of instruments were selected for qualitative analysis: 13 studies with executive and functional instruments, 09 studies with executive instruments and 08 studies with instruments functionality. The second study presents an experimental study carried out in the geriatric outpatient clinic of the University Hospital Clementino Fraga Filho, in which a descriptive and inferential analysis of 63 patients attended in this outpatient clinic was carried out using the following measures used in the Comprehensive Geriatric Assessment: Mini Mental State Examination (MEEM), Clock Drawing Test (TDR), Semantic Verbal Fluency (VF), Clinical Dementia Scale (CDR), Katz Scale and Lawton Scale. Neuropsychological measures aimed at assessing executive functioning were aligned with these instruments, namely: Digit Testing, Number and Letter Sequence Testing, Cube Testing, Part A Track Testing (TMTA) and Part B (TMTB). The studied sample showed that 76.2% are women, the mean age of this group is 77.8 years and the schooling represented in years of studies showed that 30.2% had between 05 and 08 years of studies. The results obtained in the MMSE in this sample indicated that 52.4% of the participants did not present cognitive alterations to the examination, 28.6% did not present alterations in the VF test and in the TDR 28.6% did not present failures in the task, the result of the CDR showed that 90.5% of subjects had questionable dementia. The results obtained in the executive tests indicated that TMT B is affected by schooling and aging, and the Lawton Scale presented the highest average for the group with higher schooling.

Esta disserta??o foi elaborada a partir de dois artigos. O primeiro apresenta uma investiga??o na literatura, baseada no modelo de revis?o sistem?tica, com o objetivo de investigar o decl?nio cognitivo no envelhecimento e o uso de instrumentos executivos e de funcionalidade. Com esta revis?o, observamos que as fun??es executivas est?o relacionadas ?s compet?ncias nas a??es cotidianas, e quando associadas aos instrumentos funcionais contribuem para um melhor entendimento das altera??es subjacentes no decl?nio cognitivo, evidenciando o diagn?stico cl?nico. Para a realiza??o deste estudo, foram catalogados 2070 documentos e ap?s exame criterioso, foram selecionados para an?lise qualitativa 30 documentos categorizados por combina??o de instrumentos, sendo encontrados: 13 estudos com instrumentos executivos e de funcionalidade, 09 estudos com instrumentos executivos e 08 estudos com instrumentos de funcionalidade. O segundo estudo apresenta um estudo experimental realizado no ambulat?rio de geriatria do Hospital Universit?rio Clementino Fraga Filho, no qual foi realizada uma an?lise descritiva e inferencial de 63 pacientes atendidos neste ambulat?rio, utilizando as seguintes medidas utilizadas na Avalia??o Geri?trica Ampla: Mini Exame de estado mental (MEEM), Teste de Desenho do Rel?gio (TDR), Flu?ncia Verbal Sem?ntica (FV), Escala de Dem?ncia Cl?nica (CDR), Escala Katz e Escala Lawton. As medidas neuropsicol?gicas que visam avaliar o funcionamento executivo foram alinhadas com esses instrumentos, s?o eles: Teste de D?gitos, Teste de Sequ?ncia de N?mero e Letra, Teste de Cubo, Teste de Trilha da Parte A (TMTA) e Parte B (TMTB). A amostra estudada evidenciou que 76,2% ? composta por mulheres, a m?dia de idade deste grupo ? de 77,8 anos e a escolaridade representada em anos de estudos mostrou que 30,2% tinham entre 05 e 08 anos de estudos. Os resultados obtidos no MEEM nesta amostra indicaram que 52,4% dos participantes n?o apresentaram altera??es cognitivas ao exame, 28,6% n?o apresentaram altera??es no teste de VF e na TDR 28,6% n?o apresentaram falhas na tarefa, o resultado da CDR mostrou que 90,5% dos sujeitos tinham dem?ncia question?vel. description The elderly population arouses great interest in the present, due to its growth, and it is increasingly necessary to elucidate the clinical conditions that may indicate the appearance of cognitive decline in this population. This dissertation was elaborated from two articles. The first presents an investigation in the literature, based on the systematic review model, with the objective of investigating the cognitive decline in aging and the use of executive and functional instruments. With this review, we observed that the executive functions are related to the competences in the daily actions, and when associated to the functional instruments contribute to a better understanding of the underlying alterations in the cognitive decline, evidencing the clinical diagnosis. For this study, 2070 documents were cataloged and after a careful examination, 30 documents categorized by combination of instruments were selected for qualitative analysis: 13 studies with executive and functional instruments, 09 studies with executive instruments and 08 studies with instruments functionality. The second study presents an experimental study carried out in the geriatric outpatient clinic of the University Hospital Clementino Fraga Filho, in which a descriptive and inferential analysis of 63 patients attended in this outpatient clinic was carried out using the following measures used in the Comprehensive Geriatric Assessment: Mini Mental State Examination (MEEM), Clock Drawing Test (TDR), Semantic Verbal Fluency (VF), Clinical Dementia Scale (CDR), Katz Scale and Lawton Scale. Neuropsychological measures aimed at assessing executive functioning were aligned with these instruments, namely: Digit Testing, Number and Letter Sequence Testing, Cube Testing, Part A Track Testing (TMTA) and Part B (TMTB). The studied sample showed that 76.2% are women, the mean age of this group is 77.8 years and the schooling represented in years of studies showed that 30.2% had between 05 and 08 years of studies. The results obtained in the MMSE in this sample indicated that 52.4% of the participants did not present cognitive alterations to the examination, 28.6% did not present alterations in the VF test and in the TDR 28.6% did not present failures in the task, the result of the CDR showed that 90.5% of subjects had questionable dementia. The results obtained in the executive tests indicated that TMT B is affected by schooling and aging, and the Lawton Scale presented the highest average for the group with higher schooling.
