Resumo: The present study aimed to evaluate the tumescence technique with lidocaine for mastectomy in cats and to compare it with continuous infusion of intravenous fentanyl. Twenty cats with breast tumors were selected for the radical unilateral mastectomy procedure. All animals were acclimated for 30 minutes, and then subjected to a physical examination to record baseline values (HR, f and rectal temperature). After trichotomy of the left thoracic limb, venoclysis was performed for fluid therapy with lactated ringer's solution at the rate of 3 mL.kg-1.h-1, the animal was sent to the operating room, where anesthetic induction with propofol (5 mg.kg-1) was performed intravenously (IV) and then orotracheal intubation and anesthetic maintenance with isoflurane. The animals were randomly assigned to two groups of 10 animals. The study was random and covered, composed of three anesthetists, and only the first anesthetist had knowledge of the instituted analgesic technique. The tumescence group (GT) received 15 mL.kg-1 of 0.32% lidocaine tumescent solution infiltrated in the subcutaneous tissue. The fentanyl group (GF) received an initial bolus of 2.5 ?g.kg-1 by slow intravenous fentanyl citrate and immediately afterwards, they were kept in continuous infusion at the rate of 10 ?g.kg-1.h-1. The physiological variables [heart rate (HR), respiratory rate (f), systolic blood pressure (SBP), rectal temperature, pulse oximetry (SpO2), capnography (EtCO2)] and expired anesthetic gases were assessed throughout the procedure. After the end of the surgical procedure, the animals' pain scores were evaluated at hourly intervals using the multidimensional pain scale proposed by UNESP-Botucatu until the time of analgesic rescue. The analgesic rescue was performed with 0.3 mg.kg-1 of methadone hydrochloride (IM) associated with meloxican at a dose of 0.15 mg.kg-1 by the subcutaneous route (SC). The mean time for analgesic rescue was 4 hours in the GT and 1.5 hours in the FG. We conclude that when compared, the two analgesic techniques were equally effective in decreasing the requirement for inhalation anesthetic and promoting analgesia at the trans-operative moment, however, the tumescence technique with 0.32% lidocaine showed more prolonged postoperative analgesia. The residual analgesia produced by the infusion of fentanyl was not sufficient for the postoperative period in cats submitted to unilateral mastectomy. Additionally, tumescence for promoting the hydrodivulsion effect facilitated the excision of the mammary chain and decreased the trans-operative bleeding, without interfering in the surgical healing.

Foram selecionadas 20 gatas com tumores mam?rios, sem restri??o de peso e idade, para o procedimento de mastectomia unilateral radical. Todos os animais foram aclimatados por 30 minutos e, em seguida, submetidos a um exame f?sico para os registros dos valores basais atrav?s da aferi??o da frequ?ncia card?aca (FC), respirat?ria (f), e temperatura retal. Na sequ?ncia, o animal foi transferido ao centro cir?rgico, e, ap?s a tricotomia do membro tor?cico esquerdo, foi realizada a ven?clise para a fluidoterapia com solu??o de ringer com lactato na taxa de 3 mL.kg-1.h-1. Imediatamente depois, foi realizada a indu??o anest?sica com propofol (5 mg.kg-1) pela via intravenosa (IV) e, em seguida, a intuba??o orotraqueal e manuten??o anest?sica com isoflurano. Os animais foram distribu?dos aleatoriamente em dois grupos de 10 animais. O estudo foi aleat?rio e encoberto, composto por tr?s anestesistas, e somente o primeiro anestesista possu?a conhecimento da t?cnica analg?sica institu?da. O grupo tumesc?ncia (GT) recebeu 15 mL.kg-1 de solu??o tumescente de lidoca?na a 0,32% infiltrada no tecido subcut?neo. No grupo fentanil (GF) os animais receberam um bolus inicial de cloridrato de fentanil na dose de 2.5 ?g.kg-1 dilu?dos em 3 mL no tempo fixo de 2 minutos, por via intravenosa e, logo em seguida, foram mantidos em infus?o cont?nua na taxa de 10 ?g.kg-1.h-1. As vari?veis fisiol?gicas [frequ?ncia card?aca (FC), frequ?ncia respirat?ria (f), press?o arterial sist?lica (PAS), temperatura retal, oximetria de pulso (SpO2), capnografia (EtCO2)] e gases anest?sicos expirados foram avaliados durante todo o procedimento. Ap?s o t?rmino do procedimento cir?rgico, os animais tiveram seus escores de dor avaliados em intervalos de uma hora atrav?s da escala de dor multidimensional, proposta pela UNESP-Botucatu, at? o momento de resgate analg?sico. O resgate analg?sico foi feito com 0,2 mg.kg-1 de cloridrato de metadona (IM) associado ao meloxicam na dose de 0,15 mg.kg-1 pela via subcut?nea (SC). O tempo m?dio de resgate analg?sico foi de 4 horas no GT e de 1,5 hora no GF. Conclui-se que, quando comparadas, as duas t?cnicas analg?sicas se mostraram igualmente eficazes para diminuir o requerimento de anest?sico inalat?rio e promover analgesia no momento trans-operat?rio. Entretanto, a t?cnica de tumesc?ncia com lidoca?na a 0,32% apresentou analgesia p?s-operat?ria mais prolongada. description The present study aimed to evaluate the tumescence technique with lidocaine for mastectomy in cats and to compare it with continuous infusion of intravenous fentanyl. Twenty cats with breast tumors were selected for the radical unilateral mastectomy procedure. All animals were acclimated for 30 minutes, and then subjected to a physical examination to record baseline values (HR, f and rectal temperature). After trichotomy of the left thoracic limb, venoclysis was performed for fluid therapy with lactated ringer's solution at the rate of 3 mL.kg-1.h-1, the animal was sent to the operating room, where anesthetic induction with propofol (5 mg.kg-1) was performed intravenously (IV) and then orotracheal intubation and anesthetic maintenance with isoflurane. The animals were randomly assigned to two groups of 10 animals. The study was random and covered, composed of three anesthetists, and only the first anesthetist had knowledge of the instituted analgesic technique. The tumescence group (GT) received 15 mL.kg-1 of 0.32% lidocaine tumescent solution infiltrated in the subcutaneous tissue. The fentanyl group (GF) received an initial bolus of 2.5 ?g.kg-1 by slow intravenous fentanyl citrate and immediately afterwards, they were kept in continuous infusion at the rate of 10 ?g.kg-1.h-1. The physiological variables [heart rate (HR), respiratory rate (f), systolic blood pressure (SBP), rectal temperature, pulse oximetry (SpO2), capnography (EtCO2)] and expired anesthetic gases were assessed throughout the procedure. After the end of the surgical procedure, the animals' pain scores were evaluated at hourly intervals using the multidimensional pain scale proposed by UNESP-Botucatu until the time of analgesic rescue. The analgesic rescue was performed with 0.3 mg.kg-1 of methadone hydrochloride (IM) associated with meloxican at a dose of 0.15 mg.kg-1 by the subcutaneous route (SC). The mean time for analgesic rescue was 4 hours in the GT and 1.5 hours in the FG. We conclude that when compared, the two analgesic techniques were equally effective in decreasing the requirement for inhalation anesthetic and promoting analgesia at the trans-operative moment, however, the tumescence technique with 0.32% lidocaine showed more prolonged postoperative analgesia. The residual analgesia produced by the infusion of fentanyl was not sufficient for the postoperative period in cats submitted to unilateral mastectomy. Additionally, tumescence for promoting the hydrodivulsion effect facilitated the excision of the mammary chain and decreased the trans-operative bleeding, without interfering in the surgical healing.
