Imunofenotipagem comparada dos plasmocitomas extramedulares, histiocitomas e linfomas cutâneos em cães

Resumo: A literatura sobre plasmocitomas extramedulares caninos (PEMCs) é ampla e com informações divergentes. Devido sua grande variação morfológica, essa neoplasia é comumente confundida com outros tumores benignos e malignos na rotina de diagnóstico histopatológico; principalmente com o histiocitoma cutâneo canino (HCC), o sarcoma histiocítico canino (SHC) e o linfoma canino (LC). Com o objetivo de fornecer informações adicionais sobre a imunofenotipagem dos PEMCs, testes de imuno-histoquímica foram realizados em microarranjo de tecidos (TMA) com cores de 2mm, usando os marcadores convencionais CD79a (em duplicata), CD3 (em duplicata), MUM1 (em duplicata), Iba1 e cadeias leves λ e κ de imunoglobulina em amostras de 39 PEMCs, 27 HCCs e 8 LCs. Para fins de comparação e controle, o estudo também incluiu amostras de 2 SHCs, 2 mastocitomas cutâneos (MCC), 2 melanomas orais, 1 mieloma múltiplo e espécimes de linfonodo, baço, medula óssea e pele caninos. Além disso, foi feita uma investigação inédita com o anticorpo monoclonal antireceptor α da interleucina 3 (CD123) aplicado a cães. O CD123 tem sido amplamente estudado na medicina humana como eficiente marcador de algumas neoplasias linfoides, de mastócitos e de células dendríticas plasmocitoides blásticas, mas é praticamente inexplorado na medicina veterinária. Com o objetivo de avaliar a eficiência do microarranjo de tecidos (TMA) para alguns dos marcadores, realizou-se um estudo imuno-histoquímico piloto nas seções histológicas convencionais com MUM1 e CD79a em 27 amostras de PEMCs e 10 de HCCs, com os anticorpos anti-CD3 e CD79a em 8 amostras de LCs (5 cutâneos e 3 nodais) e com o anti-CD123 em amostras de 7 PEMCs e 2 HCCs. As conclusões mais importantes foram: 1) MUM1 é muito sensível (89,7%) para detectar PEMCs e eficiente para diferenciar os PEMCs de neoplasias histiocíticas. 2) Linfomas de células T podem expressar MUM1. 3) O CD123 é expresso por PEMCs, MCCs e SHCs, além de plasmócitos normais, endotélio e possivelmente por células dendríticas plasmocitoides caninas. 4) A expressão do antígeno CD123 em neoplasias malignas como MCC e SHC deve ser melhor investigada. 5) O anticorpo Iba1 teve 100% de sensibilidade para detectar HCCs e SHCs e, é muito específico para essas neoplasias.

Due its great morphological variety, this neoplasm is commonly confused with others benign and malignant tumors in histopathologic diagnostic routine; mainly with Canine Cutaneous Histiocytoma (CCH), Canine Histiocytic Sarcoma (CHS) and Canine Lymphoma (CL). Aiming to compile information to investigate and clarify aspects regarding the immunophenotyping of CEPs, immunohistochemistry tests were performed in tissue microarray (TMA) with 2mm colors, using the conventional markers CD79a (in duplicate), CD3 (in duplicate), MUM1 (in duplicate), Iba1 and immunoglobulin λ and κ light chains in 39 CEPs, 27 CCHs and 8 CLs. For comparison and control purposes, the study also included samples from 2 CHSs, 2 cutaneous mast cell tumors (MCT), 2 oral melanomas and 1 multiple myeloma; in addition to normal and reactive tissues from canine lymph nodes, spleen, bone marrow and skin. Furthermore, an analysis unprecedented was carried out with the antibody anti- Interleukin 3 alpha receptor (CD123) applied in dogs. The CD123 has been widely studied in human medicine as an efficient marker of some lymphoid neoplasms, mast cells and blastic plasmacytoid dendritic cells, but it is practically unexplored in medicine veterinary. To evaluate the efficiency of the tissue microarray (TMA) for some markers used, a pilot immunohistochemical study was carried out in the conventional histological sections with MUM1 and CD79a in 27 samples of CEPs and 10 of CCHs, with antibodies anti-CD3 and CD79a in 8 samples of CLs (5 cutaneous and 3 nodal) and with anti-CD123 in samples of 7 CEPs and 2 CCHs. description A literatura sobre plasmocitomas extramedulares caninos (PEMCs) é ampla e com informações divergentes. Devido sua grande variação morfológica, essa neoplasia é comumente confundida com outros tumores benignos e malignos na rotina de diagnóstico histopatológico; principalmente com o histiocitoma cutâneo canino (HCC), o sarcoma histiocítico canino (SHC) e o linfoma canino (LC). Com o objetivo de fornecer informações adicionais sobre a imunofenotipagem dos PEMCs, testes de imuno-histoquímica foram realizados em microarranjo de tecidos (TMA) com cores de 2mm, usando os marcadores convencionais CD79a (em duplicata), CD3 (em duplicata), MUM1 (em duplicata), Iba1 e cadeias leves λ e κ de imunoglobulina em amostras de 39 PEMCs, 27 HCCs e 8 LCs. Para fins de comparação e controle, o estudo também incluiu amostras de 2 SHCs, 2 mastocitomas cutâneos (MCC), 2 melanomas orais, 1 mieloma múltiplo e espécimes de linfonodo, baço, medula óssea e pele caninos. Além disso, foi feita uma investigação inédita com o anticorpo monoclonal antireceptor α da interleucina 3 (CD123) aplicado a cães. O CD123 tem sido amplamente estudado na medicina humana como eficiente marcador de algumas neoplasias linfoides, de mastócitos e de células dendríticas plasmocitoides blásticas, mas é praticamente inexplorado na medicina veterinária. Com o objetivo de avaliar a eficiência do microarranjo de tecidos (TMA) para alguns dos marcadores, realizou-se um estudo imuno-histoquímico piloto nas seções histológicas convencionais com MUM1 e CD79a em 27 amostras de PEMCs e 10 de HCCs, com os anticorpos anti-CD3 e CD79a em 8 amostras de LCs (5 cutâneos e 3 nodais) e com o anti-CD123 em amostras de 7 PEMCs e 2 HCCs. As conclusões mais importantes foram: 1) MUM1 é muito sensível (89,7%) para detectar PEMCs e eficiente para diferenciar os PEMCs de neoplasias histiocíticas. 2) Linfomas de células T podem expressar MUM1. 3) O CD123 é expresso por PEMCs, MCCs e SHCs, além de plasmócitos normais, endotélio e possivelmente por células dendríticas plasmocitoides caninas. 4) A expressão do antígeno CD123 em neoplasias malignas como MCC e SHC deve ser melhor investigada. 5) O anticorpo Iba1 teve 100% de sensibilidade para detectar HCCs e SHCs e, é muito específico para essas neoplasias.
